Adjusting for bias due to incomplete case ascertainment in case-control studies of birth defects.

Case-control studies of birth defects might be subject to selection bias when there is incomplete ascertainment of cases among pregnancies that are terminated after a prenatal diagnosis of the defect. We propose a simple method to estimate inverse probability of selection weights (IPSWs) for cases ascertained from both pregnancies that end in termination and those that do not end in termination using data directly available from the National Birth Defects Prevention Study and other published information. The IPSWs can then be used to adjust for selection bias analytically. We can also allow fo…

Quantifying the bias due to observed individual confounders in causal treatment effect estimates.

It is often of interest to use observational data to estimate the causal effect of a target exposure or treatment on an outcome. When estimating the treatment effect, it is essential to appropriately adjust for selection bias due to observed confounders using, for example, propensity score weighting. Selection bias due to confounders occurs when individuals who are treated are substantially different from those who are untreated with respect to covariates that are also associated with the outcome. A comparison of the unadjusted, naive treatment effect estimate with the propensity score adjuste…

Prepregnancy body mass index and spina bifida: Potential contributions of bias.

BACKGROUND: Epidemiologists have consistently observed associations between prepregnancy obesity and spina bifida in offspring. Most studies, however, used self-reported body mass index (potential for exposure misclassification) and incompletely ascertained cases of spina bifida among terminations of pregnancy (potential for selection bias). We conducted a quantitative bias analysis to explore the potential effects of these biases on study results. METHODS: We included 808 mothers of fetuses or infants with spina bifida (case mothers) and 7,685 mothers of infants without birth defects (contro…

Misclassifying the indications for prostate-specific antigen testing may bias case-control studies of the efficacy of prostate cancer screening.

BACKGROUND AND OBJECTIVE: In the absence of data from randomized controlled trials, prostate cancer (CaP) screening recommendations may be based on observational studies that contrast exposure to screening between cases and controls. We evaluated the potential bias from mis-classifying indications for PSA testing in observational studies of CaP screening. METHODS: We randomly selected men undergoing PSA testing and obtained data on PSA results and prostate biopsies. Data were linked with a tumor registry to identify incident and prevalent cases of CaP. We abstracted medical records for 45 inc

Oral contraceptives and venous thromboembolism: an epidemiological review.

New epidemiological studies have demonstrated a higher risk of venous thromboembolism among users of oral contraceptives (OCs) with third-generation progestogens desogestrel or gestodene compared with users of OCs with second-generation progestogens levonorgestrel or norgestrel. As the absolute risk of venous thromboembolism among users of OCs in general is lower in the new studies than in previously published studies, and as several important potential confounders are not controlled for in the new studies, the real difference in risk of venous thromboembolism between users of third- compared

Dyspepsia--how noisy are gallstones? A meta-analysis of epidemiologic studies of biliary pain, dyspeptic symptoms, and food intolerance.

BACKGROUND: In contrast to earlier beliefs, it is nowadays assumed that no relation exists between dyspeptic symptoms and the presence of gallstones. Although many studies indeed failed to show a relation, each has been too small to exclude it. The aim of this meta-analysis is to systematically review epidemiologic studies and to synthesize the data. METHODS: Twenty-one controlled studies on the association between gallstones and dyspeptic symptoms were analyzed after critical appraisal with regard to selection bias, blinding, confounding, and data presentation. Most of them had reasonable va…

Cognitive screening biases in a secondary prevention Alzheimer's disease clinical trial.

INTRODUCTION: Alzheimer's disease (AD) prevention trials have multiple steps to identify cognitively unimpaired individuals with AD biomarker evidence. Cognitive/functional screening tests may be biased in ethnoracial minorities, impacting trial eligibility. METHODS: A total of 6669 participants screened for the Anti-Amyloid Treatment in Asymptomatic Alzheimer's (A4) study were grouped by ethnoracial background and testing language. Ethnoracial/language differences in ineligibility reason, cognitive/functional test performance, and amyloid positivity rates were examined. RESULTS: Ethnoracial

Now or Later? An fMRI study of the effects of endogenous opioid blockade on a decision-making network.

Previously, we found that distinct brain areas predict individual selection bias in decisions between small immediate ("Now") and larger delayed rewards ("Later"). Furthermore, such selection bias can be manipulated by endogenous opioid blockade. To test whether blocking endogenous opioids with naltrexone (NTX) alters brain activity during decision-making in areas predicting individual bias, we compared fMRI BOLD signal correlated with Now versus Later decision-making after acute administration of NTX (50 mg) or placebo. We tested abstinent alcoholics and control subjects in a double-blind two…

Evaluating potential sources of bias in linear growth rate analysis of renal tumors: case study in Von-Hippel Lindau disease.

Accurate tumor growth rate estimation is vital for linking disease progression to patient characteristics and outcomes, especially in rare diseases where limited data increase bias risk. We evaluated two potential sources of bias in linear growth rate (LGR) estimation using data from a prior natural history (NH) study of von Hippel-Lindau (VHL) renal cell carcinoma (RCC), a rare hereditary disorder. A Quantitative bias analysis (QBA) assessed selection bias from differential follow-up, while a Bayesian analysis (BA) examined shrinkage bias from a frequentist linear mixed-effects approach. In t…

Exclusion of elderly women from published randomized trials of stress incontinence surgery.

OBJECTIVE: More than 20,000 women, aged 70 years or older, undergo surgery for stress urinary incontinence each year. Our objective was to review the published randomized trials of the surgical treatment of stress urinary incontinence and estimate the proportion of women 70 years or older enrolled in those trials. DATA SOURCES: MEDLINE and Cochrane Databases of Clinical Trials were searched from January 1966 through December 2003 with the terms "urinary incontinence," "stress incontinence," "urethropexy," "needle suspension," "pubovaginal sling," "tension-free vaginal tape," "urethral injecti

Parental occupational exposures and risk of childhood cancer: a review.

We reviewed the literature in order to summarize the present knowledge on the association between parental occupational exposures to chemicals and the risk of childhood malignancy. The 32 studies pertaining to this topic were evaluated by considering various study qualities such as sample size, specificity of outcome, confounding, exposure specificity, and control selection. When evaluating the findings from any epidemiologic study, the potential sources of bias have to be considered. The selection of subjects, misclassification of exposure or outcome, and confounding from extraneous factors c

Multiple bias analysis using logistic regression: an example from the National Birth Defects Prevention Study.

PURPOSE: Exposure misclassification, selection bias, and confounding are important biases in epidemiologic studies, yet only confounding is routinely addressed quantitatively. We describe how to combine two previously described methods and adjust for multiple biases using logistic regression. METHODS: Weights were created from selection probabilities and predictive values for exposure classification and applied to multivariable logistic regression models in a case-control study of prepregnancy obesity (body mass index ≥30 vs. <30 kg/m2) and cleft lip with or without cleft palate (CL/P) using …

Optimising the case-crossover design for use in shared exposure settings.

With a case-crossover design, a case's exposure during a risk period is compared to the case's exposures at referent periods. The selection of referents for this self-controlled design is determined by the referent selection strategy (RSS). Previous research mainly focused on systematic bias associated with the RSS. We additionally focused on how RSS determines the number of referents per risk, sensitivity to overdispersion and time-varying confounding. We illustrated the consequences of different RSS using a simulation study informed by data on meteorological variables and Legionnaires' diseas
