BACKGROUND: In some rare cases, Shigella flexneri infection may be associated with fulminating encephatopathy.
CASE REPORT: A 6-year-old girl was admitted with fever, diarrhea and consciousness abnormalities. Brain CT scan was normal. Shigella flexneri type 2 was isolated from the stools. Despite antibiotic therapy, the encephalopathy was lethal. Two siblings were also infected, the first with only diarrhea, the second with headache and mild consciousness abnormalities: both children had a favorable course.

Although acute renal failure secondary to infections is relatively common in adult patients, uremia requiring dialysis has not previously been reported in an adult patient with shigella enterocolitis. Our patient, infected with S flexneri, had severe renal failure without any evidence of sepsis, rhabdomyolysis, or the hemolytic-uremic syndrome. Antibiotic therapy with trimethoprim-sulfamethoxazole appeared to play a role in his eventual recovery.

Except in gas production, S. flexneri 6 is generally similar to other S. flexneri. S. flexneri 6 contains both aerogenic and anaerogenic strains which are identical serologically. It is not surprising that S. flexneri 6 is not separable from other S. flexneri types on the basis of DNA relatedness (overall genetic makeup) (4). A new serovar of Shigella flexneri has been isolated with increasing frequency in the USSR. It has the antigenic formula IV:7,8. We suggest that it be incorporated into the classification scheme as S. flexneri subserovar 4c. The 212 strains of this proposed subserovar examined to date display biochemical and serological properties typical of the species, are sensitive to the genus-specific bacteriophage, and cause keratoconjunctivitis in the Sereny test.
