Inconsistency in Shoulder Arthrometers for Measuring Glenohumeral Joint Laxity: A Systematic Review.

There is no consensus on how to measure shoulder joint laxity and results reported in the literature are not well systematized for the available shoulder arthrometer devices. This systematic review aims to summarize the results of currently available shoulder arthrometers for measuring glenohumeral laxity in individuals with healthy or injured shoulders. Searches were conducted on the PubMed, EMBASE, and Web of Science databases to identify studies that measure glenohumeral laxity with arthrometer-assisted assessment. The mean and standard deviations of the laxity measurement from each study w…

Ototoxicity associated with extended dalbavancin treatment for a shoulder prosthetic joint infection.

BACKGROUND: Dalbavancin is a lipoglycopeptide antibiotic approved for treatment of skin and soft tissue infections, administered as a single or two-dose treatment. The extended half-life, good penetration into bone and synovial fluid, and bactericidal activity against gram-positive bacteria, including those in biofilm, make dalbavancin an appealing choice for treatment of bone and joint infections in outpatient settings. However, we present a rare case of ototoxicity associated with off-label extended dalbavancin treatment of a prosthetic joint infection. CASE PRESENTATION: A 55-year-old man

Shoulder Joint Infections with Negative Culture Results: Clinical Characteristics and Treatment Outcomes.

BACKGROUND: The incidence of septic arthritis of the shoulder joint is increasing as the population ages. The prevalence of shoulder infection is also increasing because of the growing use of arthroscopy and expansion of procedures in the shoulder. However, cultures do not always identify all microorganisms, even in symptomatic patients. The incidence of negative cultures ranges from 0% to 25%. Few studies have reported clinical features and treatment outcomes of culture-negative shoulder infections. This cohort study addresses culture-negative shoulder joint infections in nonarthroplasty pati…

Massive Lymphedema Necessitating Disarticulation of Shoulder Joint Following Treatment for Carcinoma Breast.

We report a case of massive upper-limb edema necessitating disarticulation, in a cancer survivor who tended to ignore instructions regarding preventive exercises since 2002 after completion of her treatment for breast cancer. However, she was in a family situation where she was constrained to physically involve in daily chores. Consequently, she presented 14 years later with a lymphedematous arm which weighed over 31 kg, necessitating disarticulation. We highlight the role of preventive exercises in postmastectomy patients and also the efficacy of continuous nerve block techniques in preventin

Acute septic arthritis of shoulder joint caused by Aeromonas veronii biotype sobria.

We present the case of a 91-year-old woman who presented with a 2‑day history of progressive pain and immobility of the right shoulder joint after fever of unknown etiology. Aeromonas sobria was isolated from a culture of purulent synovial fluid. The clinical condition gradually improved with the application of appropriate antibiotics and no surgical intervention was necessary. This report indicates that acute septic arthritis may result from Aeromonas veronii biotype sobria infections in healthy people. This case is contrary to the previous reports due to the absence of obvious risk factors.

Shoulder joint synovial chondromatosis presenting as multiple axillary masses: A case report.

Synovial chondromatosis is a rare, benign, proliferative cartilaginous lesion arising from the synovial tissue, tenosynovium, or bursal lining. We describe the case of a patient who initially presented with multiple axillary masses. Breast ultrasound (US) was requested due to the concern of a breast tumor with axillary lymph node metastases. US study was helpful and provided adequate information to suggest the diagnosis.

Anterosuperior labrum lesions of the shoulder joint: pathogenesis, arthroscopic treatment, and results.

The anterosuperior labrum lesion of the shoulder without biceps anchor involvement is a rare injury, and it is unclear whether the refixation has advantages over resection. We describe the pathophysiology and treatment, and compare the functional outcome of these procedures of refixation and resection in 21 patients (median follow-up 6 months, range 5-16). The labrum was refixated in 11 cases. Clinical evaluation used the Constant score. Synovialitis in the area of the labral tear was detected in all cases. In refixation patients there was a significant postoperative improvement in Constant sc

Assessment of virulence traits in Cutibacterium acnes from shoulder periprosthetic joint infections.

BACKGROUND: Cutibacterium acnes is the most common pathogen in shoulder periprosthetic joint infection (PJI) but remains difficult to distinguish from contamination due to its low virulence and presence as a skin commensal. Diagnostic ambiguity and variable antibiotic resistance further complicate management. METHODS: We analyzed 87 C acnes isolates from patients undergoing evaluation for shoulder PJI across 2 institutions. Isolates were obtained from intraoperative cultures and stored for subsequent analysis. Hemolysis, aerotolerance, and growth rate were assessed in vitro. Antimicrobial sus

Comparative Study of Shoulder Joint Stiffness During Dynamic Movement and Static Posture.

Characterizing human shoulder joint stiffness provides insight into how the shoulder contributes to arm stability under varying task demands. This study characterizes shoulder joint stiffness under two different task conditions-dynamic movement and static posture-in 3D space, and examines differences in stiffness between male and female participants. During dynamic movement trials, participants performed flexion-extension movements while wearing a parallel-actuated shoulder exoskeleton robot, whereas during static posture trials, they maintained fixed arm postures. In both conditions, the robo…

Assessment of shoulder joint derangements with magnetic resonance imaging in adult Nigerians.

OBJECTIVES: Shoulder pain secondary to various aetiologies is a common musculoskeletal complaint worldwide, and Magnetic Resonance Imaging (MRI) is the most accurate imaging method for evaluating shoulder pain in all age groups. While the patterns of shoulder MRI abnormalities in various demographics have been reported, data on sub-Sahara African populations are still sparse. This study aims to describe the imaging features and spectrum of shoulder joint pathologies on MRI in adult Nigerians. MATERIALS AND METHODS: This was a retrospective review of the shoulder MRI of 100 adult Nigerians (wi…

When should reverse total shoulder arthroplasty be considered in glenohumeral joint arthritis?

Anatomical total shoulder arthroplasty (TSA) has been used widely in treatment of glenohumeral osteoarthritis and provides excellent pain relief and functional results. Reverse total shoulder arthroplasty (RSA) was created to treat the complex problem of rotator cuff tear arthropathy. RSA also has been performed for glenohumeral osteoarthritis even in cases where the rotator cuff is preserved and has shown good results comparable with TSA. The indications for RSA are expanding to include tumors of the proximal humerus, revision of hemiarthroplasty to RSA, and revision of failed TSA to RSA. The

System Identification to Characterise Shoulder Joint Dynamics in Two Degrees of Freedom.

In this study, we present a new design of a shoulder perturbation robot that can characterise the dynamics of the shoulder in two degrees of freedom. It uses two linear electric motors to perturb the shoulder joint in internal/external rotation and abduction/adduction, and force and position sensors to measure the corresponding torque and angular displacement about the joint. System identification techniques are used to estimate the dynamics of the muscles around the joint. The advantage our apparatus offers over the existing ones is that it can efficiently transfer torque to the joint and mea…

Safety and efficacy of shoulder arthroplasty following lower extremity periprosthetic joint infection.

BACKGROUND: The safety of arthroplasty in patients with a previous periprosthetic joint infection (PJI) remains unclear. The objective of this study was to determine whether patients with a history of a properly treated PJI complicating lower extremity arthroplasty can safely undergo shoulder arthroplasty without an increased risk of shoulder infection or compromise in their clinical outcomes. METHODS: Between 2002 and 2013, 36 patients with successful treatment of an infected lower extremity arthroplasty underwent a subsequent primary shoulder arthroplasty. Three were lost to follow-up. A rare case of ipsilateral shoulder and thumb CMC joint neuropathic arthropathy.

Neuropathic arthropathy (Charcot joints) most frequently affect the weight-bearing joints of the body, are commonly associated with a variety of medical and neurological conditions and are notoriously difficult to treat due to the nature of the underlying pathology. We present a case of ipsilateral shoulder and thumb carpometacarpal (CMC) joint neuropathic arthropathy secondary to cervical syringomyelia. To our knowledge, this is the first reported case in the literature of this rare association.

OBJECTIVES: To assess the degree of impairment of shoulder proprioceptive acuity in individuals with chronic rotator cuff pathology (CRCP), … in intensity was significantly greater (P<.01). CONCLUSIONS: This study demonstrated impairment of shoulder joint position sense in CRCP. The degree of proprioceptive impairment was greatest at higher elevations in the setting of increased shoulder impingement and pain, which may serve to perpetuate the pathology. These findings provide a theoretic rationale fo …

Intra-articular fibroma of tendon sheath of the shoulder joint: synovial fibroma.

The case of a 30-year-old woman with multiple loose bodies in the shoulder joint is presented. The patient complained of discomfort of her right shoulder. Conventional radiographs failed to reveal any abnormality of the shoulder. On MRI numerous loose bodies were detected that resembled synovial chondromatosis. Histologically, the loose bodies were composed of a benign fibroblastic lesion with diagnostic features of fibroma of tendon sheath, each surfaced by synoviocytes. The clinicopathologic features of intra-articular fibroma of tendon sheath are reviewed. We propose using the term "synovia…
