American cutaneous leishmaniasis associated with degradation of native forest, regardless of economic, social and infrastructure vulnerability.

American cutaneous leishmaniasis (ACL) is a notifiable dermatozoonosis with relevant morbidity. The present study aimed to evaluate the epidemiological aspects of cases of ACL reported in Northern Paraná (2007-2016), as well as to georeference and analyse the influence of economic, social and environmental variables. Data from ACL notification forms were obtained and the patients' houses were georeferenced. Descriptive statistics, calculations of disease incidence and proportion of vegetation cover by municipality, spatial analysis, multiple linear regression and vulnerability analysis by cens

Social Vulnerability in Congenital Syphilis Case Mothers: Qualitative Assessment of Cases in Indiana, 2014 to 2016.

BACKGROUND: Congenital syphilis occurs when a pregnant woman with syphilis is not diagnosed or treated and the infection is passed in utero, causing severe infant morbidity and mortality. Congenital syphilis is easily prevented if women receive timely and adequate prenatal care. Cases of congenital syphilis are considered indicators of problems in the safety net. However, maternal social and behavioral factors can impede women's care, even when providers follow guidelines. METHODS: We reviewed case interviews and maternal records for 23 congenital syphilis cases reported to the Centers for Di

Testing the diathesis-stress model: 5-HTTLPR, childhood emotional maltreatment, and vulnerability to social anxiety disorder.

Regarding the development of social anxiety disorder (SAD), a diathesis-stress paradigm including biological vulnerabilities and environmental stressors can be assumed. However, studies dealing with the etiology of SAD did not integrate both aspects so far. We examined a particular diathesis-stress model for SAD in which we included a functional polymorphism of the serotonin transporter (5-HTTLPR) as a genetic vulnerability factor and childhood emotional maltreatment (CEM) as an environmental stressor. Current analyses were based on individuals who participated in the Study of Health in Pomera

Cleft Palate Repair in a Socially Vulnerable Area: Analysis of Sociodemographic Differences on Surgical Timing.

INTRODUCTION: Timely cleft palate repair is essential for optimal speech development and reducing long-term morbidity. Children from socially vulnerable or underserved populations often experience delays in surgical care. This study examines the impact of race, insurance, and social vulnerability index (SVI) on the timing of primary cleft palate repair in a state with significant health care disparities. METHODS: A retrospective review was conducted for 147 patients who underwent primary palatoplasty between 2011 and 2021 at the state's only ACPA-approved center. Demographic variables, cleft

Suicide prevention in primary health care among socially vulnerable populations: a narrative review.

INTRODUCTION: Suicide remains a major public health challenge worldwide, particularly among socially vulnerable populations who experience disproportionate exposure to social determinants associated with suicide risk and barriers to mental healthcare. Primary health care (PHC) is often the first point of contact with healthcare services and therefore plays a critical role in suicide prevention. METHODS: This narrative review was conducted according to the SANRA guidelines. A structured literature search of PubMed/MEDLINE, Scopus, and Web of Science was performed to identify evidence on suicid

Barriers to access to over-the-counter oral contraceptives by neighborhood social vulnerability in Boston, Massachusetts.

Oral contraceptives are widely used in the US, but prescription requirements are an access barrier, particularly for adolescents and young adults. Over-the-counter availability of norgestrel 0.075 mg (OPill) was anticipated to improve contraceptive access. This cross-sectional study assessed OPill availability in 17 Boston zip codes, stratified by Social Vulnerability Index (SVI). Among 33 pharmacies, 75.8% presented at least one barrier to access. OPill was available in all stores in low-SVI neighborhoods but not stocked in 13.6% of pharmacies in high-SVI neighborhoods, compounding low access

The Invisible Controller: A Decade-Long Missed Diagnosis of HIV in a Socially Vulnerable Elite Controller.

Elite controllers (ECs) are rare individuals living with HIV who maintain viral suppression without antiretroviral therapy (ART). Their unique immune responses have contributed significantly to HIV cure research. However, in populations burdened by psychiatric illness and housing insecurity, such cases may go undetected, raising important ethical, clinical, and public health concerns. We present the case of a 62-year-old African American woman with schizophrenia, bipolar disorder, traumatic brain injury, and unstable housing who was hospitalized after a ground-level fall. During routine testin

[Mental health and social vulnerability. The case of women living on the street at Buenos Aires, Argentine].

INTRODUCTION: The purpose of this research is to investigate the possible relationship between homelessness and mental pathology in a population of women housed in a Social Inclusion Center of the Autonomous City of Buenos Aires, Argentina. There could be a clinical bias that characterizes homelessness as a consequence of a mental illness. METHODS: A group of women between 18 and 65 years of age, socially assisted in the Social Inclusion Center for women alone or with minor children “Azucena Villaflor” of the City of Buenos Aires, who in turn consult or have consulted in the past about their

Exploring Transportation Challenges and Opportunities for Mobility-Vulnerable Populations Through the Social-Ecological Model.

Community mobility is a vital activity of daily living. Mobility-vulnerable populations-individuals with incomes below the federal poverty threshold, minorities, individuals with disabilities, and older adults-face transit disparities. Advanced technologies (e.g., ride-sharing) may mitigate transportation inequities, but limited evidence considers mobility-vulnerable populations. Utilizing the social-ecological model, the team examined challenges and opportunities associated with transportation availability, accessibility, affordability, acceptability, and adaptability among two socioeconomic

Telemedicine and Access to Elective Cholecystectomy for Socially Vulnerable Adults: A Pilot Randomized Clinical Trial.

IMPORTANCE: Socially vulnerable patients with symptomatic cholelithiasis are more likely to face barriers to accessing surgical care. This barrier to access can lead to delays in treatment, the need for emergent cholecystectomy, and worse outcomes. OBJECTIVES: To determine the effectiveness of telemedicine vs in-person surgical consultation on access to elective cholecystectomy in socially vulnerable populations and to evaluate the association of scheduling navigation with access to elective cholecystectomy in these populations. DESIGN, SETTING, AND PARTICIPANTS: This pilot randomized clinic

Assessment of Social Vulnerability in Pediatric Head and Neck Cancer Care and Prognosis in the United States.

IMPORTANCE: Prior investigations in social determinants of health (SDoH) in pediatric head and neck cancer (HNC) have only considered a narrow scope of HNCs, SDoH, and geography while lacking inquiry into the interrelational association of SDoH with disparities in clinical pediatric HNC. OBJECTIVES: To evaluate the association of SDoH with disparities in HNC among children and adolescents and to assess which specific aspects of SDoH are most associated with disparities in dynamic and regional sociodemographic contexts. Time trend, social vulnerability, and identification of risk areas for tuberculosis in Brazil: An ecological study.

INTRODUCTION: Tuberculosis is one of the ten leading causes of death and the leading infectious cause worldwide. The disease represents a challenge to health systems around the world. In 2018, it is estimated that 10 million people were affected by tuberculosis, and approximately 1.5 million people died due to the disease worldwide, including 251,000 patients coinfected with HIV. In Brazil, the disease caused 4,490 deaths, with rate of 2.2 deaths per 100,000 inhabitants. The objective of this study was to analyze the time behavior, spatial, spatial-temporal distribution, and the effects of soc

Social Determination of HIV: Women's Relationship Work in the Context of Mass Incarceration and Housing Vulnerability.

We contrast a typical "social determinants of health" framing with a more dynamic and complex "social determination of health" framing to analyze HIV-related sexual risk among women in low-income, segregated neighborhoods in New Haven, CT. Using an abductive approach, we analyze repeated, longitudinal qualitative interviews conducted over a 2-year period with a sample of 14 HIV-negative women who engaged in sex with men during the study period. Three case studies are presented to demonstrate how behaviors and sexual practices typically described as HIV "risks" can be understood as part of the

Conceptualising the social networks of vulnerable children and young people: a systematic review and narrative synthesis.

PURPOSE: The relationship between social networks and health and wellbeing is increasingly demonstrated in vulnerable adult populations. This relationship for vulnerable children and young people has not hitherto been systematically reviewed. This narrative synthesis aims to consolidate research to provide a foundational basis for future health-related social network research and interventions for children and young people. METHODS: This mixed methods systematic review synthesises research investigating whole, egocentric social networks of 32 vulnerable child groups with a mean age below 18.

Cost-effectiveness analysis of HPV vaccination: comparing the general population with socially vulnerable individuals.

BACKGROUND: After the WHO recommended HPV vaccination of the general population in 2009, government support of HPV vaccination programs was increased in many countries. However, this policy was not implemented in Korea due to perceived low cost-effectiveness. Thus, the aim of this study was to analyze the cost-utility of HPV vaccination programs targeted to high risk populations as compared to vaccination programs for the general population. MATERIALS AND METHODS: Each study population was set to 100,000 people in a simulation study to determine the incremental cost-utility ratio (ICUR), then

Predicting asthma prevalence across US communities by integrating social vulnerability and environmental risk with machine learning.

BACKGROUND: Asthma is one of the most common chronic conditions in the United States, disproportionately affecting racial and ethnic minority populations and low-income communities. Although both social and environmental factors are known contributors to asthma prevalence, the extent to which they interact to shape geographic disparities remains insufficiently understood. OBJECTIVE: This study aimed to explore how these factors vary across regions with differing asthma prevalence and thereby identify patterns that may inform targeted public health responses. METHODS: We used the US Centers f

Association of community-level social vulnerability with county-level clinical trial access among ovarian cancer patients.

OBJECTIVES: Clinical trials are essential to therapeutic advancement, yet access to trials in ovarian cancer may not be equitably distributed. This study evaluated associations between community-level social vulnerability and county-level availability of interventional ovarian cancer clinical trials in the United States. METHODS: U.S.-based ovarian cancer clinical trials (2008-2021) identified from the Aggregate Analysis of ClinicalTrials.gov database were aggregated by county and year. Trial availability was defined as ≥1 open interventional trial in a given county-year. County-level availab

Social vulnerability shapes deep clinical phenotypes and brain health in aging and dementia across Latin America.

INTRODUCTION: Adverse social conditions across the life course influence brain aging and dementia, yet their compounded impact on clinical phenotypes remains underexplored, particularly in Latin America, where social inequality and dementia burden are high. METHODS: We studied 3941 individuals from six Latin American countries, including cognitively unimpaired controls (CU), Alzheimer's disease (AD), and frontotemporal lobar degeneration (FTLD). A life-course questionnaire captured eight domains of social vulnerability, used to derive a social vulnerability index and latent vulnerability prof
