Effect of transfer status on outcomes for necrotizing soft tissue infections.

BACKGROUND: Whether patients with necrotizing soft tissue infections (NSTI) who presented to under-resourced hospitals are best served by immediate debridement or expedited transfer is unknown. We examined whether interhospital transfer status impacts outcomes of patients requiring emergency debridement for NSTI. METHODS AND MATERIALS: We conducted a retrospective review studying patients with an operative diagnosis of necrotizing fasciitis, Fournier's gangrene, or gas gangrene in the 2010-2015 American College of Surgeons National Surgical Quality Improvement Program Participant Use Data Fil…

Epidemiology and Outcomes of Hospitalized Children With Necrotizing Soft-Tissue Infections.

OBJECTIVE: The purpose of this study was to evaluate the epidemiology and outcome of hospitalized children with a diagnosis of necrotizing soft-tissue infections (NSTIs). METHODS: Demographic and outcome data of children 1 month to 18 years of age with a diagnosis of NSTI (International Classification of Diseases, 9th revision diagnosis codes 728.86 and 729.4) were extracted from the Kids' Inpatient Database 2009 and 2012. Univariate and multivariate analyses were done to determine the factors affecting mortality. The sample data were weighted to get national estimates. RESULTS: A total of 4

Necrotising soft tissue infections: the effect of hyperbaric oxygen on mortality.

In a single-centre, retrospective, case-controlled study of patients attending the Alfred Hospital in Prahran, Victoria, we assessed the effect of hyperbaric oxygen therapy (HBOT) in reducing mortality or morbidity in patients with necrotising fasciitis (NF) over a 13-year period from 2002 to 2014. A total of three hundred and forty-one patients with NF were included in the study, of whom 275 received HBOT and 66 did not. The most commonly involved sites were the perineum (33.7%), lower limb (29.9%) and trunk (18.2%). The commonest predisposing factor was diabetes mellitus (34.8%). Polymicrobi

Hospitalist perspective on the treatment of skin and soft tissue infections.

The prevalence of skin and soft tissue infections (SSTIs) has been increasing in the United States. These infections are associated with an increase in hospital admissions. Hospitalists play an increasingly important role in the management of these infections and need to use hospital resources efficiently and effectively. When available, observation units are useful for treating low-risk patients who do not require hospital admission. Imaging tools may help to exclude abscesses and necrotizing soft tissue infections; however, surgical exploration remains the principal means of diagnosing necro…

Treatment with beta-lactam antibiotics in skin and soft tissue infections.

Beta-lactam antibiotics are highly effective against most bacteria causing skin and soft tissue infections. Good tissue penetration further makes beta-lactam antibiotics useful against skin and soft tissue infections. Drugs with a narrow spectrum should be among the first choice. Broader spectrum beta-lactam antibiotics may be needed in reduced bacterial susceptibility. Low molecular weight compounds (monobactams, penems, penicillinase inhibitors) in addition to their high antibacterial effect have shown excellent tissue penetration. In easy and moderately severe anaerobic infections not treat…

Skin and soft tissue polymicrobial infections from intravenous abuse of drugs.

Skin and soft tissue infections were studied in 21 seriously ill narcotic addicts who had been admitted to hospital. Subcutaneous abscesses were present in 14 patients; cellulitis was noted in 3, pyomyositis in 2 and necrotizing fasciitis in 2. In four patients there was septicemia. Infections in 14 patients (66.6 percent) were associated with anaerobic bacteria, which were the exclusive isolates in 6 patients. In seven patients (33.3 percent) isolates were exclusively aerobic bacteria and in eight both aerobes and anaerobes were present. The anaerobic isolates were clostridia (six), peptostre…

Ureaplasma urealyticum necrotizing soft tissue infection.

Ureaplasma urealyticum is a urogenital commensal and often associated with localised infection. With the advent of monoclonal immunosuppressive therapy and improved diagnostic methods, reports of non-urogenital infections are accumulating. We report a rare case of U. urealyticum necrotizing soft tissue infection and left hip septic arthritis in a hypogammaglobulinaemic patient. Consideration of this organism as an etiological agent, and potential early use of nucleic-acid diagnostic investigation with empiric therapy including activity against Ureaplasma in this patient population may be warra

Extensive necrotizing soft tissue infection of the perineum.

Synergistic necrotizing fasciitis of the penis and scrotum was first described by Fournier, and remains a rare, but life-threatening disease. The reported mortality rates in different series range between 22 and 60%. We present a study of 20 patients treated over a 9 year period by early aggressive excision. Four of the treated patients (20%) died. A high index of suspicion, combined with early radical surgical intervention, is the key to effective treatment. Delayed diagnosis and treatment were the primary factors responsible for death.

Soft tissue infection in children in south Trinidad.

A review of 391 children in the first decade of life admitted to a surgical ward with soft tissue infections revealed predisposing factors in 38.4% of cases, mainly trauma and adjacent skin sepsis. Superficial abscess formation occurred in 90% of cases, muscle abscesses in 9.5%, and necrotizing fasciitis in less than 1%. The most frequent organism isolated was the staphylococcus aureus which was found in 52.8% of the available cultures. Coliform organisms were found in 14.6%, and haemolytic streptococci in just over 10%. A study of the culture and sensitivity profiles obtained made it possible…

The diabetic foot. Soft tissue and bone infection.

Diabetic patients, as a consequence of various neurologic, vascular, and metabolic perturbations, are at high risk for developing infections of the soft tissue and bones of the feet. The microbial etiology of soft tissue in infections is best determined by cultures of a tissue curetting or aspiration, rather than a swab. Aerobic gram-positive cocci are the major pathogens in diabetic foot infections; these may be the sole isolate(s) in acute uncomplicated infections, but they are usually accompanied by aerobic gram-negative bacilli or anaerobes in chronic or previously treated infections. Care

Soft-tissue infection in lower extremity trauma.

Soft-tissue infection following lower extremity trauma has not been studied in detail in light of recent data on biology of infection. This article examines specific problems in lower extremity trauma that allow the wound to become susceptible to wound infection. It also illustrates the various principles of wound management in lower extremity trauma that serve to prevent infection. Two case examples are used to illustrate principles of management. Other wound problems in lower extremity trauma are also discussed, such as rabies, necrotizing soft-tissue infection, tetanus, and diabetic foot in

Biochemical analysis of soft tissue infectious fluids and its diagnostic value in necrotizing soft tissue infections: a 5-year cohort study.

BACKGROUND: Necrotizing soft tissue infections (NSTI) are rapidly progressing and life-threatening conditions that require prompt diagnosis. However, differentiating NSTI from other non-necrotizing skin and soft tissue infections (SSTIs) remains challenging. We aimed to evaluate the diagnostic value of the biochemical analysis of soft tissue infectious fluid in distinguishing NSTIs from non-necrotizing SSTIs. METHODS: This cohort study prospectively enrolled adult patients between May 2023 and April 2024, and retrospectively included patients from April 2019 to April 2023. Patients with a cli…

Necrotizing soft tissue infection of the scalp and face.

INTRODUCTION: Necrotizing soft tissue infections (NSTIs) are characterized by rapidly spreading, life-threatening infections with widespread soft tissue necrosis that most commonly infect the extremities, torso, and perineum. Although rare, NSTIs of the head and neck are particularly dangerous given the complex anatomy surrounding the region and demand early diagnosis, antibiotic administration, and surgical debridement. CASE PRESENTATION: We report a case of a 64-year-old male who presented with a NSTI of the scalp and face after sustaining a laceration to the posterior scalp. The patient in…
