A critical appraisal of issues and problems in monitoring urinary phencyclidine (PCP) is presented. Problems may be related to impurities of ingested material and/or metabolites, and methods that are not sensitive enough to detect PCP in the nanogram/ml amounts which may be present in blood or urine. BACKGROUND: Water-loss dehydration (hypertonic, hyperosmotic, or intracellular dehydration) is due to insufficient fluid intake and is distinct from hypovolemia due to excess fluid losses. Water-loss dehydration is associated with poor health outcomes such as disability and mortality in older people. BACKGROUND: Differentiating Weber B/SER4a from SER4b fractures relies on weightbearing radiographs and gravity stress tests, assumed to reflect deep posterior tibiotalar ligament (dPTTL) integrity. This study assessed their diagnostic accuracy for dPTTL injury, with arthroscopy as the reference standard.
METHODS: We prospectively assessed 20 Weber B/SER4a-b fractures with a medial clear space ≥ 5.0 mm. Arthroscopy under local anesthesia evaluated dPTTL integrity, defined as tensioning and medial joint space closure during dorsiflexion.

Autonomic dysreflexia is the most important specific complication of high level spinal cord injury both in tetraplegic and in paraplegic patients above the midthoracic neural segment. It is a life threatening emergency that may lead to apoplexy. The use of tegretol in a 24-hour dose of 200-600 mg in 12 patients with diabetes incipidus led to the improvement of their general condition, lesser thirst, reduction of the 24-hour diuresis and increase of specific gravity of the urine. A positive effect of prescribing tegretol together with chlorpropamide was noted in a patient with diabetes mellitus and incipidus.
