The facts behind the figures: a reply to Enderby and Davies (1989).

In 1989, Enderby and Davies discussed the type and pattern of speech and language therapy provision which would be required to service the needs of the speech- and language-impaired population of the UK, on the basis of statistical information drawn from a wide range of sources. In this paper, some methodological issues arising from their study are addressed and an argument made that alternative models of service provision should be considered when planning for future needs.

Transcribing disordered speech: by target or by production?

The ability to transcribe disordered speech is a vital tool for speech-language pathologists, as accurate description of a client's speech output is needed for both diagnosis and effective intervention. Clients in the speech clinic often use sounds that are not part of the target sound system and which may, in some cases, be sounds not found in natural language at all. While the IPA provides a wide range of symbols that can be used in clinical transcription, the extended IPA (extIPA) may also be needed to transcribe atypical sounds never or rarely encountered in natural language. When using th

Cotreatment as a Vehicle for Interprofessional Collaborative Practice: Physical Therapists and Speech-Language Pathologists Collaborating in the Care of Children With Severe Disabilities.

PURPOSE: This article defines interprofessional collaborative practice and links its key features with accepted practice conceptualizations of physical therapy. Cotreatment with speech-language pathology is described as a vehicle for interprofessional collaborative practice for children with severe disabilities. METHOD: The article reviews the International Classification of Functioning, Disability, and Health (WHO, 2015) and the Hypothesis-Oriented Algorithm for Clinicians II (Rothstein, Echternach, & Riddle, 2003) as existing service-delivery frameworks in physical therapy and discusses how…

Accuracy of Automatic Processing of Speech-Language Pathologist and Child Talk During School-Based Therapy Sessions.

PURPOSE: This study examines the accuracy of Interaction Detection in Early Childhood Settings (IDEAS), a program that automatically transcribes audio files and estimates linguistic units relevant to speech-language therapy, including part-of-speech units that represent features of language complexity, such as adjectives and coordinating conjunctions. METHOD: Forty-five video-recorded speech-language therapy sessions involving 27 speech-language pathologists (SLPs) and 56 children were used. The F measure determines the accuracy of IDEAS diarization (i.e., speech segmentation and speaker clas

Technology and the evolution of clinical methods for stuttering.

UNLABELLED: The World Wide Web (WWW) was 20 years old last year. Enormous amounts of information about stuttering are now available to anyone who can access the Internet. Compared to 20 years ago, people who stutter and their families can now make more informed choices about speech-language interventions, from a distance. Blogs and chat rooms provide opportunities for people who stutter to share their experiences from a distance and to support one another. New technologies are also being adopted into speech-language pathology practice and service delivery. Telehealth is an exciting development…

Targeted speech sounds and their association to children's grade level and therapy dosage.

The purpose of this study was to understand which speech sounds are most commonly targeted in therapy, and how that target sound is associated with the child's grade and the dosage received within sessions. Dose is considered the number of times a speech sound is elicited in a single clinical session (e.g. 80 trials per session). We present data from 772 therapy sessions collected from 106 school-based speech-language pathologists, using the Experience Sampling Method (ESM). ESM is an approach to collecting data in situ during moments of specific interest. This investigation, specifically, exa

First-Line Health Care Providers' Reported Knowledge of and Referrals to Speech-Language Pathologists for Clients With Mild Traumatic Brain Injury.

Purpose People with mild traumatic brain injury (mTBI) may experience deficits in cognition or communication that go unnoticed by first-line health care providers (FHPs). Speech-language pathologists (SLPs) assess and treat these domains yet are often underrepresented on mTBI multidisciplinary teams. This study's aim was to evaluate FHPs' reported knowledge of and referral practices to SLPs for individuals across the life span with mTBI. Method Physicians, physician assistants, nurse practitioners, nurses, and athletic trainers (n = 126) completed an online survey, including two Likert scale q

A newly devised speech accumulator.

Voice therapy is often most effective for treating patients with vocal cord polyp, polypoid degeneration and singer's nodule. However, little is known about the total speaking times in 1 day, the ratio of speech per hour and the sound level during speech, in individual patients. If these parameters can be readily detected, it could be clarified as to how speaking times or patterns are related to a particular voice disorder and/or what instruction a doctor had given a patient. We devised a speech accumulator which records the vibration time of the vocal cords by a small contact microphone attac

Therapy for Childhood Apraxia of Speech Through a Dynamic Systems Lens: A Tutorial.

PURPOSE: Dynamic Systems Theory (DST) has been used as a foundational lens through which to observe and understand child development and disorders. Childhood apraxia of speech (CAS) is a motor planning speech disorder that can be difficult to treat. This tutorial outlines how a DST framework can be used to understand the therapy process for children with CAS, thus aiding clinicians to provide more individualized and focused therapy for these children. METHOD: This tutorial will focus on three concepts from DST that may have relevance for CAS related therapy. These are: (a) multicausality, (b)

A Tool for Differential Diagnosis of Childhood Apraxia of Speech and Dysarthria in Children: A Tutorial.

PURPOSE: While there has been mounting research centered on the diagnosis of childhood apraxia of speech (CAS), little has focused on differentiating CAS from pediatric dysarthria. Because CAS and dysarthria share overlapping speech symptoms and some children have both motor speech disorders, differential diagnosis can be challenging. There is a need for clinical tools that facilitate assessment of both CAS and dysarthria symptoms in children. The goals of this tutorial are to (a) determine confidence levels of clinicians in differentially diagnosing dysarthria and CAS and (b) provide a system

The influence of type of feedback during tablet-based delivery of intensive treatment for childhood apraxia of speech.

PURPOSE: One of the key principles of motor learning supports using knowledge of results feedback (KR, i.e., whether a response was correct / incorrect only) during high intensity motor practice, rather than knowledge of performance (KP, i.e., whether and how a response was correct/incorrect). In the future, mobile technology equipped with automatic speech recognition (ASR) could provide KR feedback, enabling this practice to move outside the clinic, supplementing speech pathology sessions and reducing burden on already stretched speech-language pathology resources. Here, we employ a randomize…
