Anterior fixation of thoracolumbar traumatic spinal injuries.

BACKGROUND: Traumatic spinal fractures require surgical treatment to restore the anatomical curvatures of the spine and release the nervous structures. However, uniform management guidelines have not been established and thus the literature includes papers focussing only on the posterior techniques as well as those assessing the anterior or anteroposterior approaches. This paper presents the outcomes of the treatment of spinal fractures with anterior and anteroposterior fixation. MATERIAL AND METHODS: The study enrolled 48 patients. The age of the patients at the time of injury was 36.8 years

Congenital spinal cord anomalies: a pictorial review.

Development of spinal canal and its contents occurs in a much regulated fashion. Aberration at any stage of development namely gastrulation, primary neurulation, secondary neurulation, and retrogressive differentiation can result in a specific abnormality. Spinal cord anomalies or spinal dysraphism is a heterogeneous group containing some entities that are obvious at birth and many that are discovered only after imaging for neurological symptoms or signs. Congenital spinal tumors are closely related and present either as an external mass or imaging abnormalities. Radiological imaging plays a c

Neurofibromatosis in children.

Neurofibromatosis occurs once in every 3,000 live births. Café au lait spots are the most common presenting lesion. Five spots with a diameter of at least 0.5 cm. should be considered diagnostic in children. Spinal deformity is the most common bony lesion. Scoliosis varies from mild nonprogressive forms to hairpin curvatures. Four types of pseudarthrosis of the tibia can be distinguished, with progressively more serious prognoses. The incidence of neoplasia greatly exceeds that of the general population.

Evaluation of Chêneau brace in the treatment of thoracic and lumbar adolescent idiopathic scoliosis with apical vertebral rotation.

BACKGROUND: Adolescent idiopathic scoliosis (AIS) is a common structural disorder of the spine in adolescents, often associated with structural deformities in both coronal and axial positions. Apical vertex rotation (AVR) is one of the main indicators of axial deformity in patients with scoliosis. Currently, there are few studies on the impact of AVR in the treatment of AIS. OBJECTIVE: This study examined the influence of different AVR on AIS after brace treatment. METHODS: Data were collected from 106 AIS participants aged 11-16 years from the orthopedic outpatient clinic of the Second Hosp

[Musculoskeletal disorders in patients with Parkinson's disease].

Parkinson's disease (PD) is a progressive neurodegenerative disorder that causes posture alterations with trunk anteflexion and the flexion of the hip and knee joints, leading to changes in the musculoskeletal system (MS). MS disorders in Parkinson's Disease (PD) are linked to two primary mechanisms: they can be a direct result of the motor symptoms of PD and may also serve as early indicators of the disease. On one hand, clinically significant manifestations such as camptocormia, Pisa syndrome, anterocollis, and hand dystonia can lead to postural alterations and deformities of the fingers as

Combined anterior-posterior versus posterior only approach for surgical management of adult spinal deformity: a systematic review and meta-analysis of comparative studies.

PURPOSE: The purpose of this systematic review and meta-analysis was to examine the clinical outcomes and complication rates for fusion procedures of adult spinal deformity (ASD) performed via an anterior-posterior approach as compared to a posterior-only approach to guide surgical decision-making. Numerous surgical techniques exist for operative management of ASD; however, no systematic review and meta-analysis exists comparing combined anterior-posterior approaches to posterior-only approach, despite significant interest in the current literature. METHODS: Four databases were used to collec

The predictive power of the Roussouly classification on mechanical complications after surgery for adult spinal deformity: systematic review and meta-analysis.

BACKGROUND: With the increasing prevalence of adult spinal deformity (ASD) in the aging population, the need for corrective surgery has surged, highlighting the importance of preventing mechanical complications (MC) such as junctional kyphosis/failure and rod breakage. The Roussouly classification, which categorizes natural variations in spinal posture, may hold predictive value in assessing the risk of these complications, as it guides the restoration of sagittal alignment based on a patient's preoperative spinal shape. METHODS: PubMed, Cochrane, and Google Scholar (pages 1-20) were searched

Measurement Issue in Antecollis.

BACKGROUND: Antecollis is defined as an involuntary forward flexion of the neck. Previous reports have measured the neck flexion angles based on the line perpendicular to the ground. This led to an inflation of the neck flexion angles in patients who had combined forward truncal flexions, especially upper camptocormia. METHODS: We examined the neck flexion angles and the upper camptocormia angle in the published photographs of antecollis. MEDLINE search was conducted using the following search terms: antecollis OR anterocollis. Lateral-view photographs of patients diagnosed with antecollis we

Pseudarthrosis in adult and pediatric spinal deformity surgery: a systematic review of the literature and meta-analysis of incidence, characteristics, and risk factors.

We conducted a systematic review with meta-analysis and qualitative synthesis. This study aims to characterize pseudarthrosis after long-segment fusion in spinal deformity by identifying incidence rates by etiology, risk factors for its development, and common features. Pseudarthrosis can be a painful and debilitating complication of spinal fusion that may require reoperation. It is poorly characterized in the setting of spinal deformity. The MEDLINE, EMBASE, and Cochrane databases were searched for clinical research including spinal deformity patients treated with long-segment fusions reporti

Treatment of camptocormia with botulinum toxin.

Camptocormia is defined as an involuntary axial postural distortion of >45° flexion which occurs in the upright position, increases whilst walking and resolves when supine (Ashour and Jankovic, 2006). Unlike orthopaedic or age related kyphosis it is not a fixed structural deformity and produces kyphosis at predominantly lumbar and thoracic rather than cervical regions. Camptocormia has been reported due to a wide range of neurologic, psychiatric, muscular and orthopaedic conditions as well as rare reports of its emergence following the initiation of a number of medications (Finsterer and Strob

Coronal realignment and reduction techniques and complication avoidance.

Scoliosis is a broad term encompassing multiple pathologies with different etiologies. Patients may range from the infant with congenital deformity, to the adolescent with idiopathic scoliosis, to the elderly patient with severe degenerative scoliosis. Treatment must be tailored to individual circumstances and the pathoanatomy of each deformity. Various coronal reduction techniques have been described and will be discussed within this article. While scoliosis is generally considered a deformity in the coronal plane, often deformity is present in the sagittal and axial planes also. Treatment of

Long-term impact of atlantoaxial arthrodesis on the pediatric cervical spine.

BACKGROUND: This study was conducted to elucidate radiographically the long-term impact of atlantoaxial arthrodesis on the pediatric cervical spine. METHODS: The records of eight children who underwent atlantoaxial arthrodesis for the treatment of upper cervical spinal disorders and were followed up to their adulthood were retrospectively reviewed. Changes in the curvature of the whole cervical spine, and anteroposterior diameters of the spinal canal at C1 and C2 levels were investigated on lateral radiographs before surgery, at 6 months after surgery, and at the final follow-up. RESULTS: Th
