Relationship Between Spleen Pathologic Changes and Spleen Stiffness in Portal Hypertension Rat Model.

OBJECTIVE: The aim of the study described here was to explore the influence of splenic pathology and hemodynamic parameters on spleen stiffness in portal hypertension (PH). METHODS: A Sprague‒Dawley rat model of PH (n = 34) induced by CCl4 was established, and 9 normal rats were used as controls. All animals underwent a routine ultrasound examination, spleen stiffness measurement (SSM), liver stiffness measurement (LSM), portal vein pressure (PVP) measurement and histopathologic assessment. The diagnostic performance of SSM and LSM in PH was evaluated. SSMs were compared among the groups at d…

The function of the spleen in adults after ligation of the splenic artery of the traumatized spleen in childhood.

BACKGROUND: Ligation of the splenic artery (LSA) has been successfully used as a spleen-saving procedure in rare cases of splenic trauma in children in which management with splenorrhaphy or partial splenectomy alone was not possible. There are no data regarding the long-term effects of the procedure on the functional status of the spleen. The purpose of this study is to present and discuss our clinical and laboratory findings in adults who underwent LSA in childhood. METHODS: Our first 2 patients in whom LSA was done at ages 4 and 2 years in 1977 underwent the following examinations in the y…

A model to estimate spleen volume from palpated spleen length in patients with myelofibrosis.

Spleen volume is commonly assessed in clinical trials involving patients with myelofibrosis (MF), but direct measurement requires radiologic imaging. In routine clinical settings, spleen length measured by palpation is frequently used as an alternative measure. As the relationship between spleen length and spleen volume is not strictly proportional, interpretation of palpation-based measurements may be variable. In this post hoc analysis, we examined the relationship between palpable spleen length and spleen volume in patients with MF using pooled data from two phase 3 clinical studies. Palpab…

Spleen stiffness determined by spleen-dedicated device accurately predicted esophageal varices in cirrhosis patients.

BACKGROUND: The advantages of spleen stiffness in prediction of high-risk varices (HRV) in cirrhosis patients have been confirmed. Recently, a new device utilizing a 100 Hz probe dedicated to spleen stiffness measurement (SSM) was developed. OBJECTIVES: To validate the clinical applicability of SSM@100 Hz in predicting HRV by comparing it with other non-invasive tests (NITs). DESIGN: A prospective cohort study. METHODS: A total of 171 cirrhosis patients who underwent esophagogastroduodenoscopy (EGD) examination were included in this study. SSM using a 100 Hz probe and liver stiffness measur…

Primitive isolated hydatid cyst of the spleen: total splenectomy versus spleen saving surgical modalities.

BACKGROUND: This study aims to describe the clinical features of the isolated primitive splenic hydatid cyst, discuss and compare the differ … ation were excluded. Four patients underwent total splenectomy and four of them underwent different spleen preserving surgical techniques including resection of the protruding dome, partial splenectomy and pericystectomy. RESULTS: The diagnosis was incidentally made in 50% of cases. The main other revealing complaints are pain in the left upper quadrant …

Laparoscopy-assisted spleen-preserving pancreatic resection for epidermoid cyst in an intrapancreatic accessory spleen.

A rare case of an epidermoid cyst originating in an intrapancreatic accessory spleen in a 50-year-old Japanese female is reported. A hypoechoic cystic tumor was detected incidentally by abdominal ultrasonography. It appeared to be a single cyst in the pancreatic tail with a contrasted mass lesion beside it. Laparoscopy-assisted spleen-preserving pancreatic tail resection was performed. Microscopic examination revealed that the cyst was surrounded by fibrous tissue and a thin layer of splenic tissue, adjacent to normal pancreatic parenchyma. The inner surface of the cyst was lined with non-kera…

Gut-Spleen Axis: Microbiota via Vascular and Immune Pathways Improve Busulfan-Induced Spleen Disruption.

Fecal microbiota transplantation (FMT) is an effective means of modulating gut microbiota for the treatment of many diseases, including Clostridioides difficile infections. The gut-spleen axis has been established, and this is involved in the development and function of the spleen. However, it is not understood whether gut microbiota can be used to improve spleen function, especially in spleens disrupted by a disease or an anti-cancer treatment. In the current investigation, we established that alginate oligosaccharide (AOS)-improved gut microbiota (A10-FMT) can rescue anticancer drug busulfan…

Delayed Spleen Rupture After Trauma.

The spleen is the most commonly injured solid organ in patients sustaining blunt abdominal trauma. The widespread use of computed tomography (CT) has significantly improved its diagnosis and grading. Delayed splenic rupture (DSR) refers to hemorrhage occurring more than 48 hours after blunt abdominal trauma in a patient who was initially hemodynamically stable. Although uncommon, it remains a recognized complication of splenic injury and can occur days to weeks after the initial trauma and carries significant mortality. We present a case of DSR presented one week after the injury, which was ma…

Spleen rupture due to brucellosis.

Brucellosis is a common zoonotic infection worldwide; it is caused by infection with the bacterial species Brucella and leads to severe diseases in humans and animals. In Turkey, this bacterial species has not been completely eradicated and is commonly found in animals (such as goats or sheep). Brucellosis can lead to various symptoms, affect multiple systems, and cause splenomegaly in the case of spleen involvement. In contrast to traumatic spleen ruptures, spontaneous spleen ruptures are rare and most commonly occur because of infectious causes. A 52-year-old man was treated at our infectiou…
