Severe splenic rupture after colorectal endoscopic submucosal dissection.

Splenic rupture (SR) after colonoscopy is a very rare but potentially serious complication. Delayed diagnosis is common, and may increase morbidity and mortality associated. There is no clear relation between SR and difficult diagnostic or therapeutic procedures, but it has been suggested that loop formation and excessive torquing might be risk factors. This is a case of a 65-year-old woman who underwent endoscopic submucosal dissection (ESD) for lateral spreading tumor in the descending colon, and 36 h afterwards presented symptoms and signs of severe hypotension due to SR. Standard splenecto…

Atraumatic splenic rupture secondary to chronic HIV infection.

As patients infected with HIV live longer due to effective anti-retroviral therapy, new disease manifestations are becoming apparent. We describe the case of a 59-year-old patient who presented to our unit with atraumatic splenic rupture secondary to chronic HIV infection. Given the high mortality associated with atraumatic splenic rupture, we believe it should be included in the differential diagnosis of HIV-positive patients presenting with acute abdominal pain.

Delayed and occult splenic rupture: a diagnostic puzzle.

In developing nations like India, chronic otitis media (COM) is a common middle ear ailment that has serious ramifications for both hearing and quality of life. Long-term inflammation of middle ear cavity and tympanic membrane are the hallmarks of COM, which can result in consequences like facial paralysis, labyrinthitis, hearing loss, and potentially fatal cerebral abscesses. The effect of COM on vestibular function is still unknown. Studies reveal a positive correlation between the severity of COM and vestibular impairment, which may point to a cause-and-effect link. Using air caloric testin

[Lethal atraumatic splenic rupture due to infectious mononucleosis].

The authors report on the sudden and unexpected death of a 20-year-old man from atraumatic rupture of the enlarged spleen due to infectious mononucleosis. The case exemplifies the forensic relevance of infectious mononucleosis, atraumatic splenic rupture and post-mortem serological tests.

Spontaneous splenic rupture caused by Babesia microti infection.

Babesiosis has not been previously associated with spontaneous splenic rupture. We describe an otherwise healthy 61-year-old man with symptomatic babesiosis whose spleen ruptured during hospitalization. Although this complication is rare, practitioners who commonly treat patients with babesiosis should be aware of its potential occurrence.

Spontaneous splenic vein aneurysm rupture in the puerperium.

A case of intra- and retroperitoneal haemorrhage after a splenic vein aneurysm rupture is described. A 27-year-old woman complained of chest pain on her first postpartum day. Symptoms were initially suggestive of pulmonary embolism, but within 3 h she developed severe acute abdominal pain, abdominal distension and haemodynamic instability. Ultrasound demonstrated a non-echogenic mass in the abdomen suggestive of a fluid collection and a computed tomography scan confirmed the presence of a lesion at the pancreatic tail. Urgent laparotomy revealed splenic vein rupture near the pancreatic tail. P

Splenic rupture after laparoscopic surgery: a case report.

Splenic rupture is a rare and severe complication of laparoscopic surgery. We report a case of delayed splenic rupture as a severe complication of laparoscopy, occurring 5 days after surgery performed on a 52-year-old woman with history of abdominal surgery and with acute pelvic infection. Subcapsular hematoma, resulting in splenic rupture, may have been a result of overlooked puncture by the Veress needle or by tension on splenic adhesions during lysis. The diagnosis should be brought up during postoperative care in case of severe pain with hemodynamic failure, hemoglobin concentration decrea…

[Spontaneous splenic and renal rupture in polyarteritis nodosa].

Classic polyarteritis nodosa (PAN) is a rare necrotising vasculitis and can result in intrarenal or perirenal bleeding because of aneurysm formation. This report describes the case of a patient with severe manifestation of PAN who developed a subcapsular hematoma from a spontaneously ruptured aneurysm in the spleen and a retroperitoneal hematoma from a renal cortex aneurysm. For the first time angiographic controls have been documented over years in this disease. The value of angiographic procedures for diagnosis and for evaluation of the disease course of PAN is discussed.

Spontaneous splenic rupture in infectious mononucleosis: a review.

Spontaneous rupture of the spleen is a rare complication of infectious mononucleosis (IM) occurring in 0.1-0.5 percent of patients with prov … ection. We report the case of a 36 year old man with infectious mononucleosis who had a spontaneous splenic rupture and who was successfully managed by splenectomy. Based on review of the literature, an approach to management of a spontaneously ruptured spleen secondary to IM is suggested.

Choriocarcinoma complicated by splenic rupture: an unusual presentation.

A patient with choriocarcinoma presenting with acute abdominal pain due to splenic rupture from secondary deposits is reported. This rare case exhibited several other unusual features including coexisting bilateral ovarian teratomas and the absence of any evidence of a primary lesion. Several hemangiomata in the liver initially confused the clinic picture.

[Late splenic rupture: a risk of conservative treatment].

We report a 33 year old male admitted after a traffic accident with a painful abdomen and an open ankle fracture. An abdominal CAT scan showed a splenic laceration and free ascites. A conservative treatment was decided considering that the patient was hemodynamically stabilized. Seven days later, the patient appeared hypotense and with severe pain and was operated. During surgical intervention, a massive hemoperitoneum due to splenic bleeding from the spleen was found and a splenectomy was performed. Postoperative outcome was uneventful. The therapeutic approach to splenic traumatism ranges fr

Spontaneous splenic rupture. The role of nonoperative management.

Spontaneous splenic rupture associated with infectious mononucleosis is an infrequent occurrence. Splenectomy has been advocated as the appropriate treatment for these patients. Recently, three patients with spontaneous splenic rupture were successfully treated at our institution without surgery. Management of spontaneous splenic rupture in 37 other patients in the literature was reviewed. Nine of these patients also were treated nonoperatively. Although it has been suggested that splenectomy is the treatment of choice for patients with spontaneous splenic rupture, we believe selective nonoper…

Radionuclide diagnosis of splenic rupture in infectious mononucleosis.

Spontaneous splenic rupture is a rare but serious complication of infectious mononucleosis. Although radionuclide spleen imaging is a well accepted method for diagnosis of traumatic rupture, interpretation can be difficult in the setting of mononucleosis, as tears may be ill-defined and diagnosis hampered by inhomogeneous splenic uptake. Four proven cases of spontaneous rupture are presented, three of which illustrate these diagnostic problems.
