General anesthesia in dental treatments: where and how?

BACKGROUND: Patients with physical and/or mental disabilities exhibit higher caries activity and more treatment needs compared with the general population. Thus, the number of patients awaiting dental treatment under general anesthesia (GA) is far from negligible. Factors such as disabilities, presence of comorbidities, underlying medical conditions, and potential drug interactions render these patients a high-risk group for GA. Consequently, meticulous planning is essential when scheduling dental treatment under GA. The objective of the present research was to gather the opinions of anesthesi

Clinical Performance of Two Resin Composite Materials in Class II Cavities in Primary Molars: An Observational Study.

BACKGROUND: Composite resins have seen significant development. In pediatric dentistry, direct composite resin restorations are becoming a crucial component of conservative care. Since their initial introduction, these materials have undergone fast evolution, leading to more esthetically pleasing and long-lasting repairs. As a result, amalgam in dentistry underwent a paradigm change in favor of more tooth-structure-preserving composites. AIM: To evaluate and compare the clinical performance of a flowable composite resin and a packable composite resin in class II direct restorations in primary

Kinematic analysis of work-related musculoskeletal loading of trunk among dentists in Germany.

BACKGROUND: In Germany, about 86.7 % of the dentists have stated to suffer from pain in the neck and shoulder region. These findings are predominantly based on surveys. Therefore the objective of this study is to conduct a kinematic analysis of occupational posture in dentistry. METHODS: Twenty one dentists (11 f/10 m; age: 40.1 ± 10.4 years) have participated in this examination. The CUELA-System was used to collect kinematic data of the activities on an average dental workday. A detailed, computer-based task analysis took place parallel to the kinematic examination. Through the synchronizat

[Dental care using silver amalgam].

Dental amalgam is the most widely used filling material in dentistry. In our country there are an estimated 40 million amalgam fillings in place. The mercury present in these fillings has caused health concerns over the last 160 years that amalgam has been used in decayed teeth. The fears have always proven to be unjustified and no harmful effects have ever been demonstrated in dental patients. Mercury can be found in several forms. In dentistry, only the metallic form is used, while inorganic and organic compounds are also present in the environment. The metallic form is absorbed in the human

Trends in death associated with pediatric dental sedation and general anesthesia.

BACKGROUND: Inadequate access to oral health care places children at risk of caries. Disease severity and inability to cooperate often result in treatment with general anesthesia (GA). Sedation is increasingly popular and viewed as lower risk than GA in community settings. Currently, few data are available to quantify pediatric morbidity and mortality related to dental anesthesia. OBJECTIVE: Summarize dental anesthesia-related pediatric deaths described in media reports. METHODS: Review of media reports in the Lexis-Nexis Academic database and a private foundation website. SETTINGS: Dental

Visit-level quality measurement reveals missed opportunities and overuse in preventive oral health care.

BACKGROUND: Traditional annual dental quality measures fail to capture real-time care delivery and can mask critical gaps in patient-specific treatment. The authors developed and implemented visit-level adaptations of established caries quality measures to enable actionable, point-of-care assessment. METHODS: The authors conducted a retrospective analysis of 1.09 million dental visits across 400,000 patients (2021-2023) at 4 US institutions using axiUm (Henry Schein Corp) or Epic (Epic Systems) electronic health records. The authors transformed 2 annual measures involving caries risk assessme

Periodontal diseases of children and adolescents.

Children and adolescents are subject to several periodontal diseases. Although there is a much lower prevalence of destructive periodontal diseases in children than in adults, children can develop severe forms of periodontitis. In some cases, this destructive disease is a manifestation of a known underlying systemic disease. In other young patients, the underlying cause for increased susceptibility and early onset of disease is unknown. These diseases are often familial, suggesting a genetic predisposition for aggressive disease. Current modalities for managing periodontal diseases of children

On the Future Design of Bio-Inspired Polyetheretherketone Dental Implants.

Polyetheretherketone (PEEK) is a promising implant material because of its excellent mechanical characteristics. Although this polymer is a standard material in spinal applications, PEEK is not in use in the manufacturing of dental implants, where titanium is still the most-used material. This may be caused by its relative bio-inertness. By the use of various surface modification techniques, efforts have been made to enhance its osseointegrative characteristics to enable the polymer to be used in dentistry. In this feature paper, the state-of-the-art for dental implants is given and different

Methoxyflurane: a review with emphasis on its role in dental practice.

Methoxyflurane was developed as an anaesthetic agent and introduced into clinical practice in 1960. It soon became evident that it possessed analgesic properties that other drugs did not. Due to toxicity concerns, it lost favour in general anaesthesia and had been largely abandoned by the late 1970s. The manufacturer withdrew it in 1999, and the Food and Drug Administration in the United States did not renew its licence in 2005. It has also been withdrawn by the European Union. However, it continues to be used in Australasia, primarily as an inhaled self-administered analgesic by emergency ser

US/ROS-responsive ALN-modified polymer nanoparticles enhance the efficacy of infiltration anesthesia through improved bone targeting and permeation.

Complex maxillofacial bone anatomy compromises the efficacy of local anesthetics. This study develops an ultrasound (US)/reactive oxygen species (ROS) cascade-responsive polylactic-coglycolic acid (PLGA) nanoparticle system for enhanced dental infiltration anesthesia and perioperative pain management. The nanoparticles, which were surface-modified with alendronate (ALN) via an ROS-cleavable thioketal (TK) linker and coloaded with bupivacaine (BUPI) and perfluorohexane (PFH), enabled enhanced bone targeting and penetration. Under US stimulation, PFH undergoes a liquid-gas phase transition, gene

Hydroxyapatite Dental Inserts for Tooth Restoration: Stress and Displacement Analysis.

Hydroxyapatite (HAP) inserts minimize restoration contraction by constituting a major part of the restoration; however, their effect on the relaxation of tooth tissues has not been previously tested. Finite element analysis was employed to estimate stress and displacement when HAP inserts with a thickness of 1.7 mm or 4.7 mm and a diameter of 4.7 mm were used to substitute for dentin. The volumetric contraction of the composite during polymerization, simulated through steady-state heat transfer analysis, yielded a contraction rate of 3.7%. Descriptive statistics revealed that the incorporation

Quantitative Real-Time RT-PCR Verifying Gene Expression Profile of Cavitations Within Human Jaw Bone.

Background/Objectives: Immune cells are integral to bone homeostasis, including the repair and remodeling of bone tissue. Chronic dysregulation within this osteoimmune network can lead to bone marrow defects of the jaw (BMDJ), particularly fatty degenerative osteonecrosis of the jaw (FDOJ). These localized pathologies are implicated in systemic immune dysfunctions. Methods: This study is designed to determine whether BMDJ/FDOJ samples are indicative of medullary bone pathology by evaluating FDOJ gene expression patterns using quantitative real-time PCR. Results: Comparative analyses between pa
