Stuttered disfluencies in Parkinson's disease: Longitudinal changes.

BACKGROUND: Acquired neurogenic stuttering occurs in approximately 1 in 5 people with Parkinson's disease. It is currently unknown how stuttering changes as the disease progresses. OBJECTIVE: Examine longitudinal changes in the frequency of stuttered disfluencies in individuals with Parkinson's disease. METHOD: We measured stuttered disfluencies in 63 people with Parkinson's disease in conversation, picture description, and reading samples. All participants completed at least two assessment sessions, a mean of 2.9 years (± 1.5) apart. Nineteen completed a third session, and two a fourth. Bay…

Analyzing the Attitudes of Teachers in Spain Toward Stuttering.

INTRODUCTION: Teachers play a crucial role in fostering supportive environments for all students. However, research indicates that their knowledge and attitudes toward stuttering are often similar to those of the general population. The primary aim of this study is to examine the attitudes of teachers at different educational levels in Spain toward stuttering and to explore how these attitudes relate to specific sociodemographic variables. METHOD: A cross-sectional, self-report design was employed with a sample of 250 teachers from various educational levels and teaching roles across Spain. P…

Stuttering and Neurodiversity Across the Lifespan: A Moveable Feast.

PURPOSE: This viewpoint explores the idea that a neurodiversity-affirming social model and a medical model of stuttering health care are not mutually exclusive. In all cases, they should be used in combination. For any client, the only way to attain an optimal clinical outcome is to apply a combined medical and social health care perspective. The two models have a changing contribution across the lifespan, according to the client's requirements, age, and presenting complaints and the viability of neuroplastic change of the speech mechanism. CONCLUSIONS: The benefits of neurodiversity applied …

Stuttering Priapism in a Teenage Boy: Lesson to be Learnt.

Untreated priapism can lead to ischemic damage of the penis and impotence. This case report describes a 14-year-old boy who presented with a history of priapism for 2 months, which was undiagnosed, ridiculed, and ignored even by medical practitioners. The underlying etiology was later identified to be chronic myeloid leukemia. Despite the usage of multimodal treatment, it took 7 days for control of priapism. The young boy is now left with an erectile dysfunction. The case highlights that priapism in children is a medical emergency needing aggressive evaluation and treatment.

The Pharmacologic Treatment of Stuttering and Its Neuropharmacologic Basis.

Stuttering is a DSM V psychiatric condition for which there are no FDA-approved medications for treatment. A growing body of evidence suggests that dopamine antagonist medications are effective in reducing the severity of stuttering symptoms. Stuttering shares many similarities to Tourette's Syndrome in that both begin in childhood, follow a similar male to female ratio of 4:1, respond to dopamine antagonists, and symptomatically worsen with dopamine agonists. In recent years, advances in the neurophysiology of stuttering have helped further guide pharmacological treatment. A newer medication …

Pediatrician Referral Practices for Children Who Stutter.

Purpose Given the marked increase in evidence-based information regarding the nature/treatment of stuttering, coupled with the fact that pediatricians tend to be one of the initial points of contact for parents who suspect their preschool-age child may stutter, this study explored pediatricians' (a) accuracy in identifying children who may stutter and (b) likelihood of referring children who present with a profile indicative of stuttering to speech-language pathologists. Method Pediatricians recruited nationally through professional organizations completed a 5- to 7-min online survey that prob…

The working alliance in stuttering treatment: a neglected variable?

BACKGROUND: Multiple factors can influence the working alliance and treatment outcome in speech and language therapy. The 'working alliance' is an important concept in treatment and can be described as the degree to which a treatment dyad is engaged in collaborative, purposive work. To date, relatively little attention has been paid to this concept within speech and language treatment in general, and within stuttering treatment research in particular. AIMS: To investigate the role of the working alliance within stuttering treatment, and to evaluate whether the quality of the working alliance …

Anxiety of children and adolescents who stutter: a review.

PURPOSE: Adults who stutter have heightened rates of anxiety disorders, particularly social anxiety disorder, compared with non-stuttering controls. However, the timing of anxiety onset and its development in relation to stuttering is poorly understood. Identifying the typical age of anxiety onset in stuttering has significant clinical implications and is crucial for the management of both disorders across the lifespan. The present review aims to determine the scope of the research pertaining to this topic, identify trends in findings, and delineate timing of anxiety onset in stuttering. METH…

Anxiety and stuttering: continuing to explore a complex relationship.

PURPOSE: The relationship between anxiety and stuttering has been widely studied. However, a review conducted more than 10 years ago (Menzies, Onslow, & Packman, 1999) identified 5 methodological issues thought to preclude consistent research findings regarding the nature of this relationship. The purpose of the present review was to determine whether methodological improvements have occurred since the Menzies et al. (1999) review. METHOD: Literature published since the Menzies et al. review was evaluated with regard to the 5 methodological issues identified in that review: (a) the construct …

Psychiatric disorders in a patient with persistent developmental stuttering.

Persistent developmental stuttering (PDS) is a common disorder of speech with no identifiable cause. Psychiatric disorders appear to be related and influence clinical manifestation of PDS. In this case report, we present the clinical evolution of 1 PDS patient submitted to pharmacological treatment with fluoxetine and speech therapy intervention. At the end of 12 weeks of treatment, she evolved from 28 at Beck Depression Inventory; 32 in the Hamilton Scale for Anxiety; 43 and 47, respectively, in the anxiety and avoidance components of the Liebowitz Social Anxiety Scale; and severe speech impa…

Stuttering treatment for adults: an update on contemporary approaches.

This article provides a brief overview of historical and current approaches to stuttering treatment for adults. Treatment is discussed in terms of stuttering management approaches, fluency-shaping approaches, and combined approaches. The evidence base for these various approaches is outlined. Fluency-shaping approaches have the most robust outcome evidence. Stuttering management approaches are based more on theoretical models of stuttering, and the evidence base tends to be inferred from work using the approaches of cognitive behavior therapy and desensitization with other disorders such as an…

Identification of cluttering and stuttering by the public in four countries.

The investigators sought to explore and compare the identification of cluttering vs stuttering in four different country samples. After reading lay definitions of the two fluency disorders in their own language, convenience samples of 60-90 adult respondents from Turkey, Bulgaria, Russia, and the US identified 51-119 children or adults who either cluttered, stuttered, or both. They also indicated whether or not they, themselves, cluttered or stuttered. The majority of respondents in all four samples identified at least one person who cluttered, stuttered, or cluttered and stuttered. The averag…

Social anxiety and the severity and typography of stuttering in adolescents.

UNLABELLED: The present study examined the relationship between anxiety, attitude toward daily communication, and stuttering symptomatology in adolescent stuttering. Adolescents who stuttered (n=19) showed significantly higher levels of trait, state and social anxiety than fluent speaking controls (n=18). Trait and state anxiety was significantly associated with difficulty with communication in daily situations for adolescents who stutter, but not for controls. No statistically significant associations were found between anxiety and measures of communication difficulty, and the severity or typ…
