Reliability and Interobserver Variability of Evans' Index and Disproportionately Enlarged Subarachnoid Space Hydrocephalus as Diagnostic Criteria for Idiopathic Normal Pressure Hydrocephalus.

BACKGROUND: The image diagnosis of idiopathic normal-pressure hydrocephalus (iNPH) is based on the ventriculomegaly, whose criterion is an Evans' Index (EI) >0.3. Recently, disproportionately enlarged subarachnoid space hydrocephalus (DESH) has been proposed as a morphological characteristic to iNPH. Several studies cast doubt on the reliability of these criteria in the diagnosis of iNPH. Furthermore, interobserver differences of these criteria have not yet been investigated. The objective of this study was to assess the diagnostic reliability and interobserver variability of EI and DESH. MAT…

Abnormal hyperintensity within the subarachnoid space evaluated by fluid-attenuated inversion-recovery MR imaging: a spectrum of central nervous system diseases.

A variety of central nervous system (CNS) diseases are associated with abnormal hyperintensity within the subarachnoid space (SAS) by fluid-attenuated inversion-recovery (FLAIR) MR imaging. Careful attention to the SAS can provide additional useful information that may not be available with conventional MR sequences. The purpose of this article is to provide a pictorial essay about CNS diseases and FLAIR images with abnormal hyperintensity within the SAS. We present several CNS diseases including subarachnoid hemorrhage, meningitis, leptomeningeal metastases, acute infarction, and severe arter…

Nimodipine and enoxaparin improve neurological outcomes and reduce cerebral edema by modulating glymphatic perivascular spaces in a rabbit model of subarachnoid hemorrhage.

BACKGROUND: Glymphatic system dysfunction is a mechanism potentially linked to secondary brain injury after subarachnoid hemorrhage (SAH). While nimodipine and enoxaparin are commonly used in clinical practice, their influence on perivascular clearance mechanisms remains unexplored. This exploratory study aimed to provide preliminary insights into how these agents might modulate the glymphatic pathway and influence early functional recovery in an experimental SAH model, addressing a gap in understanding the mechanistic effects of these standard treatments. METHODS: Twenty male New Zealand whi

Verminous myelopathy secondary to aberrant Dirofilaria immitis migrans was detected in the cervical subarachnoid space of four dogs using MRI and CT.

Aberrant Dirofilaria immitis migrans is a rare cause of neurologic signs in dogs, however, published studies describing the computed tomographic (CT) and magnetic resonance imaging (MRI) characteristics of this problem are currently lacking. The objective of this retrospective case series study was to describe the clinical and imaging findings for four adult dogs with verminous myelopathy due to aberrant Dirofilaria immitis migrans within the cervical subarachnoid space. All dogs were toy breeds, were heartworm antigen positive, had neurologic signs (ranging from cervical hyperesthesia to tetr…

Value of a single-shot turbo spin-echo pulse sequence for assessing the architecture of the subarachnoid space and the constitutive nature of cerebrospinal fluid.

Three case history reports are presented to illustrate the value of the single-shot turbo spin-echo pulse sequence for assessment of the subarachnoid space. The use of the single-shot turbo spin-echo pulse sequence, which is a heavily T2-weighted sequence, allows for a rapid, noninvasive evaluation of the subarachnoid space by using the high signal from cerebrospinal fluid. This sequence can be completed in seconds rather than the several minutes required for a T2-fast spin-echo sequence. Unlike the standard T2-fast spin-echo sequence, a single-shot turbo spin-echo pulse sequence also provides…

Utility of MRI-based disproportionately enlarged subarachnoid space hydrocephalus scoring for predicting prognosis after surgery for idiopathic normal pressure hydrocephalus: clinical research.

OBJECTIVE The presence of disproportionately enlarged subarachnoid space hydrocephalus (DESH) on brain imaging is a recognized finding of idiopathic normal pressure hydrocephalus (iNPH), but the features of DESH can vary across patients. The aim of this study was to evaluate the utility of MRI-based DESH scoring for predicting prognosis after surgery. METHODS In this single-center, retrospective cohort study, the DESH score was determined by consensus between a group of neurosurgeons, neurologists, and a neuroradiologist based on the preoperative MRI findings of the patients with suspected iNP…

A new quantitative method to assess disproportionately enlarged subarachnoid space (DESH) in patients with possible idiopathic normal pressure hydrocephalus: The SILVER index.

OBJECTIVES: Preoperative diagnosis of idiopathic normal-pressure hydrocephalus (iNPH) remains challenging. Recently, the presence of disproportionally enlarged subarachnoid spaces and hydrocephalus (DESH) on diagnostic images has been linked to clinical improvement after ventriculoperitoneal (VP) shunt placement. In this study we describe a new quantitative method to assess DESH on CT scans and to evaluate its prognostic value. PATIENTS AND METHODS: A multiplanar reconstruction software was used to retrospectively evaluate prospectively collected radiological data (CT scans) of 26 controls an…

Development of calcitonin gene-related peptide slow-release tablet implanted in CSF space for prevention of cerebral vasospasm after experimental subarachnoid haemorrhage.

The calcitonin gene-related peptide (CGRP), a known potent intrinsic cerebral vasodilator, is contained in the sensory nerves from trigeminal ganglia that inervate the cerebral arteries. We previously reported that human alpha CGRP (hCGRP) dilates spastic cerebral arteries after experimental subarachnoid haemorrhage (SAH) in rabbits. In the present study, we investigated the prophylactic potential of a sustained higher cerebrospinal fluid level of hCGRP against experimental cerebral vasospasm. An hCGRP slow-release tablet (hCGRP s-r tablet) was developed for cisternal implantation. Experimenta

Ischemia triggered by red blood cell products in the subarachnoid space is inhibited by nimodipine administration or moderate volume expansion/hemodilution in rats.

OBJECTIVE: It has been proposed that delayed ischemic neurological deficits are induced by red blood cell (RBC) products after subarachnoid hemorrhage. Prophylactic treatment with the Ca2+ antagonist nimodipine or prevention of systemic volume contraction reduces the occurrence of delayed ischemic neurological deficits. To gain insight into the underlying mechanism, we studied the effects of nimodipine or volume expansion on ischemic events induced by RBC products in rats. METHODS: A cranial window was implanted in 52 rats. At the window, cerebral blood flow (measured with laser Doppler flowm

Diagnostic importance of disproportionately enlarged subarachnoid space hydrocephalus and Callosal Angle measurements in the context of normal pressure hydrocephalus: A useful tool for clinicians.

Hydrocephalus is a clinical and neurologic condition characterized by an abnormal accumulation of cerebrospinal fluid (CSF). Currently, idiopathic Normal Pressure Hydrocephalus (iNPH) is recognized as a late-onset neurologic disorder defined by a clinical triad of gait disturbance, urinary incontinence, and dementia. Imaging studies often reveal a disproportionate increase in subarachnoid space and ventricular size relative to brain volume, with normal CSF pressure during lumbar puncture. Recent developments in neuroimaging have led to the establishment of criteria for diagnosing and managing …

Lumbar elastance and resistance to CSF outflow correlated to patency of the cranial subarachnoid space and clinical outcome of endoscopic third ventriculostomy in obstructive hydrocephalus.

OBJECT: The purpose of the present study was to elucidate the value of the lumbar and intraventricular infusion tests in the selection of patients with obstructive hydrocephalus (OH) for endoscopic third ventriculostomy (ETV), and whether the presence of a diminished cranial subarachnoid space was a source of error in the interpretation of the results. METHODS: In 32 consecutive adult patients (15 M, 17 F, mean age: 46 years) with possible treatment-requiring OH, the resistance to cerebrospinal fluid (CSF) outflow (Rout) and elastance was measured with a lumbar infusion test. Eleven of the pa…

[Transplantation of basal forebrain cells of fetal rats into the subarachnoid space: improvement of disturbance of passive avoidance memory due to injury of basal forebrain].

The memory disturbance of senile dementia of Alzheimer type has been thought to associate with marked degeneration and loss of cholinergic neurons of the basal forebrain (nucleus basalis of Meynert, NBM). Electrical or chemical destruction of the NBM causes memory deficits in rats. After unilateral lesioning of the NBM in adult rats with excitotoxic amino acid, kainic acid, basal forebrain cells of fetal rats were transplanted through a microsyringe needle, the tip of which was transcortically inserted to the subarachnoid space. Eight weeks after the transplantation, passive avoidance response…

MRI-based evaluation of cerebrospinal fluid dynamics and optic nerve subarachnoid space alterations in idiopathic intracranial hypertension and normal tension glaucoma: a systematic review.

BACKGROUND: Idiopathic intracranial hypertension (IIH) and normal tension glaucoma (NTG) are optic nerve-related disorders linked to cerebrospinal fluid (CSF) abnormalities but driven by distinct mechanisms. Continuity between the intracranial and optic nerve subarachnoid spaces allows CSF pressure and flow alterations to affect the optic nerve. This study aims to evaluate optic nerve subarachnoid space structure and CSF dynamics. METHODS: The Protocol was registered in the PROSPERO database (CRD420251267099). A systematic literature search of PubMed, Springer Nature, ScienceDirect, Scopus, a…
