Combined urinary and faecal incontinence.

Combined urinary and faecal (liquid or solid) incontinence (double incontinence) is the most severe and debilitating manifestation of pelvic floor dysfunction. The community prevalence is 9-19% (urinary) and 5-10% (faecal), increasing with age. Pathophysiological factors include childbirth-associated external anal sphincter injury and pudendal nerve damage, pelvic floor descent, menopause, collagen disorders and multiple sclerosis-like conditions. The presence of crossed reflexes between the bladder, urethra, anorectum and pelvic floor in animal studies may explain the comorbidity of urinary a

Outcomes after laparoscopic removal of retropubic midurethral slings for chronic pain.

INTRODUCTION AND HYPOTHESIS: Midurethral slings (MUS) are an established treatment for stress urinary incontinence (SUI), with good objective outcomes and low rates of complications. However, large population-based registry studies highlighted long-term complications from polypropylene slings including erosion, dyspareunia and chronic pain. With recent highly negative media coverage, many women are presenting with chronic pain attributed to the mesh to request complete removal. The available literature provides limited evidence on safety, symptom resolution and incontinence following MUS remov

Mini-slings can cause complications.

INTRODUCTION AND HYPOTHESIS: Single-incision mini-slings (SIMS) have been advocated to avoid the complications of transobturator and retropubic midurethral slings. We present a series of SIMS complications and their outcome after vaginal removal at a tertiary care center. METHODS: Following Institutional Review Board approval, a prospective database of consecutive women who underwent SIMS removal for complications and had a minimum follow-up of 6 months was reviewed. Patient-reported outcomes were assessed by main symptom category. In addition, an ideal outcome or cure was defined as continen

Inability to walk and persistent thigh pain after transobturator tape procedure for stress urinary incontinence: surgical management.

INTRODUCTION AND HYPOTHESIS: Groin pain after transobturator tape is often a self-limiting situation, but can occasionally persist and be associated with serious neurological sequelae. The video is aimed at presenting the surgical management of persistent groin pain and inability to walk after transobturator sling placement and subsequent partial removal. METHODS: The featured patient is a 31-year-old woman unable to walk after transobturator sling implantation 2 years before. She reported left thigh pain immediately after surgery that was not responsive to postoperative medication. Six month

Medium-term follow-up on use of freeze-dried, irradiated donor fascia for sacrocolpopexy and sling procedures.

The aim of this study was to document longer-term follow-up of patients in a previously reported series who underwent either sacrocolpopexy (SCP) or suburethral sling procedures utilizing freeze-dried, irradiated donor fascia. Subjects from the initial series of 67 SCPs and 35 slings were included in this retrospective chart review of postoperative follow-up where surgical follow-up longer than 3 months from the procedure was available. Subjects undergoing SCP were examined at the time of any clinical visit and their pelvic organ support evaluated utilizing the POP-Q system. The SCP procedure

[Multidisciplinary meetings in pelvic floor disease in women: A national survey by the CUROPF among French urologists].

INTRODUCTION: Multidisciplinary team meetings (MTMs) in the field of pelvic floor diseases in women tend to generalize, as they are required as mandatory before mid-urethral sling implantation or sacrocolpopexy by recent decrees published by the French health authorities. However, access to these meetings is variable in the French territory. The goal of the present study was to describe the existence and the settings of these kinds of meetings in France. MATERIEL AND METHODS: An on-line survey was conducted between June and July 2020 (stage 1) then between November 2021 and January 2022 (stag

Recent status of the treatment of stress urinary incontinence.

The therapeutic options for management of stress urinary incontinence can be conservative, pharmacological or surgical. The treatment of patients with stress urinary incontinence should be tailored to the individual to optimize care. A multitude of surgical techniques have emerged to treat this condition in recent years. The objective of the present review was to present an overview of current practice in the management of stress urinary incontinence while considering the evidence supporting the clinical effectiveness of these procedures.

Do nonautologous biological slings have a future?

PURPOSE OF REVIEW: The purpose of this paper is to review the literature published during 2005 on nonautologous biological slings for the treatment of stress urinary incontinence. RECENT FINDINGS: Little has been done by the way of research to assess adequately whether nonautologous biological slings perform better or worse than other types of slings. Three cohort studies and two comparative studies have been reported, but no randomized controlled trials have been published during the last year comparing biological slings with synthetic slings. Sporadic reports of unexpectedly high recurrent

Synthetic slings: which material, which approach.

PURPOSE OF REVIEW: The success in the use of the tension-free midurethral sling procedures (tension-free vaginal tape, suprapubic arch, transobturator tape) has dramatically altered the approach to the surgical management of stress urinary incontinence in women and marked a resurgence in the use of mesh. We will analyze the characteristics of synthetic meshes and explain the rationale for using monofilament materials with larger pore sizes. Thus, the purpose of this review is to introduce the surgeon to a variety of commercially available mesh materials and the characteristics that make mesh s

Is it possible to diagnose intrinsic sphincter deficiency in women?

PURPOSE OF REVIEW: The diagnosis of intrinsic sphincter deficiency became important about 20 years ago when it influenced the choice of operative procedure for women with stress urinary incontinence. However, it was ill-defined and diagnosed by a variety of techniques. The contemporary surgical treatment of urinary stress incontinence is by suburethral sling; so this review addresses three questions: is it still important to identify intrinsic sphincter deficiency prior to surgical intervention for stress incontinence? What techniques are available to do so? How robust are these measurements?

Retropubic hemorrhage following Midurethral sling surgery: Diagnosis, clinical challenges, and management.

Mid-urethral slings (MUS) such as tension-free vaginal tape (TVT) are the established gold standard surgical approach for treating stress urinary incontinence (SUI). While generally effective, complications such as bladder injury, urinary retention, and bleeding can occur. This article focuses on a rare but significant complication: postoperative hemorrhage into the retropubic space (Cavum Retzii) following MUS/TVT placement. Key challenges, diagnostic options, and management strategies for this condition are assessed. Additionally, a structured clinical guideline to support a systematic appro

Passive Versus Active Devices for Male Stress Urinary Incontinence: The Case for Passive Devices.

Passive continence devices provide continence outcomes with 30-80% improvement or cure rates while avoiding the cognitive and manual dexterity demands of most active devices. Their patient-independent function and outpatient adjustability align with preferences for simplicity and fewer future interventions.

Could infiltration anesthesia decrease anchor-fixation quality of midurethral slings in the obturator complex?

INTRODUCTION: Single-incision miduretral slings (SIMS) were withdrawn from the market in many countries due to lower efficacy. In some countries they are still in use, preferred primarily because it is possible to perform the procedure under local anesthesia. Based on our previous clinical experience we postulated that local anesthesia decreased primary anchor fixation in the obturator complex. The aim of the study is to assess how local infiltration anesthesia influences anchor fixation of the tape in porcine obturator complex. METHODS: The experiment was designed to determine the maximum fo
