Vaginal Mesh Exposure Presentation, Evaluation, and Management.

INTRODUCTION: According to FDA, in 2010, approximately 300,000 women underwent surgical procedures in the USA to repair pelvic organ prolapse and approximately 260,000 underwent surgical procedures to repair stress urinary incontinence. From 560,000 surgeries, synthetic mesh was used in one out of three, and three out of four were performed transvaginally. The incorporation of mesh into pelvic organ prolapse repair has improved the long-term surgical anatomical outcomes and lower recurrence rates. However, vaginal mesh placement is associated with risks such as vaginal mesh erosion, exposure,

Risk factors and management of vaginal mesh erosion after pelvic organ prolapse surgery.

OBJECTIVE: Mesh erosion is a serious and not uncommon complication in women undergoing vaginal mesh repair. We hypothesized that mesh erosion is associated with the patient's comorbidities, surgical procedures, and mesh material. The aims of this study were to identify the risk factors and optimal management for mesh erosion. MATERIALS AND METHODS: All women who underwent vaginal mesh repair from 2004 to 2014 were retrospectively reviewed. Data on patients' characteristics, presenting symptoms, treatment and outcomes were collected from their medical records. RESULTS: A total of 741 women un

Surgical management of bladder erosion and pelvic pain after laparoscopic lateral suspension for pelvic organ prolapse.

INTRODUCTION AND HYPOTHESIS: Mesh-augmented lateral suspension for prolapse repair seems to be associated with few complications. However, mesh-related complications can negatively affect the quality of life and may be challenging to manage. This video is aimed at presenting the surgical management of a case of severe pelvic pain and dyspareunia after lateral laparoscopic suspension associated with mesh erosion in the bladder. METHODS: A 46-year-old woman was referred to our Unit for severe pelvic pain and inability to have sexual intercourses since undergoing a uterus-sparing laparoscopic la

Caesarean Section Following Abdominoplasty With Mesh Repair: A Case Report.

Abdominoplasty with mesh reinforcement is an increasingly used surgical technique for abdominal wall restoration, but there is limited literature regarding pregnancy and delivery following such procedures. We report the case of a 38-year-old woman with a history of previous caesarean section and subsequent abdominoplasty with mesh repair who underwent an elective caesarean section at 39 weeks' gestation. Intraoperatively, the mesh was encountered beneath the rectus muscles and was sharply dissected to access the peritoneal cavity. The baby was delivered via a standard lower uterine segment inc

Prophylactic mesh to prevent parastomal hernia: a meta-analysis of randomized controlled studies.

The aim of the present meta-analysis was to determine whether prophylactic mesh decreases the odds of parastomal hernia formation. Randomized controlled trials referenced in MEDLINE or EMBASE between 1946 and 2016 comparing prophylactic mesh to standard stoma formation were included. The primary outcome was occurrence of parastomal hernia. Secondary outcomes were parastomal hernia requiring surgical intervention and complications. Odds ratios were calculated for the primary and secondary outcomes. Subgroup analyses were conducted based on mesh type, mesh location, laparoscopic versus open, and

Tissue engineered multifunctional chitosan-modified polypropylene hernia mesh loaded with bioactive phyto-extracts.

Surface modified tissue engineered polypropylene / PP hernia meshes were fabricated by incorporating Bacterial cellulose / BC and chitosan / CS and phytochemical extracts. Under current practice, hernia and other traumatic injuries to the abdominal organs are clinically treated with surgical meshes. Often the foreign body reaction and infections result in relapse in patients which dictates additional reparative surgical procedures and pain. To improve the outcome of clinical restorative procedures new biomaterials with improved characteristics are required. The functionalized meshes were physi…

Titanium mesh as an alternative to a membrane for ridge augmentation.

PURPOSE: The purpose of the present study was to determine the rate and extent of complications associated with using titanium mesh for bone regeneration and to identify which factors might be associated with postoperative complications, such as infection, soft tissue exposure, and graft failure. MATERIALS AND METHODS: A retrospective study design was used to analyze the treatment outcomes of 27 consecutive surgical sites in the maxilla and mandible that were treated with bone grafting in conjunction with fixation of titanium mesh. The primary study variable was whether the surgical sites tre

Mesh infection in ventral incisional hernia repair: incidence, contributing factors, and treatment.

BACKGROUND: Prosthetic mesh infection is a catastrophic complication of ventral incisional hernia (VIH) repair. METHODS: The current surgical literature was reviewed to determine the incidence, microbiology, risk factors, and treatment of mesh infections. RESULTS: Mesh infections tend to present late. Diagnosis depends on high clinical suspicion and relies on culture of the fluid surrounding the mesh or of the mesh itself. Risk factors may include a high body mass index (obesity); chronic obstructive pulmonary disease; abdominal aortic aneurysm repair; prior surgical site infection; use of l

Abdominal wall infections with in situ mesh.

BACKGROUND: Synthetic mesh is used commonly in the repair of abdominal wall hernias. Infection at the surgical site where mesh is present poses a formidable clinical problem. METHODS: The current surgical literature was reviewed to formulate accepted approaches to the management of hernia repairs with infected mesh. RESULTS: Prevention of mesh infection is best achieved by judicious use of systemic antibiotics. Topical antibiotics often are used without convincing evidence to support their value. Laparoscopic repairs have lower infection rates than open repairs. Evidence is lacking to suppor

[The devices used to correct the urinary incontinence by tension-free meshes. Are all them equals?].

Since 1996, when Ulmsten described the TVT procedure (Tension-free Vaginal Tape) for correction of women urinary stress incontinence, a large number of different devices have been manufactured with that purpose. Results depend not only of the surgical procedure but also of two principal factors: 1. Characteristics and properties of the mesh. 2. System and way for the implantation. Properties of the mesh used are probably as important as surgical technique. It's not possible to assume that results achieved with the TVT device will be similar with other meshes. Further randomized studies will be

Cystoscopic removal of intravesical synthetic mesh extrusion with the aid of Endoloop sutures and endoscopic scissors.

Synthetic mesh is commonly used in urogynecologic surgical procedures for correction of stress urinary incontinence and pelvic organ prolapse. Bladder mesh extrusion is a recognized complication and frequently requires extensive mesh resection. We present a case of intravesical polypropylene mesh extrusion following a combined mesh kit and midurethral sling placement. A novel transurethral approach, employing Endoloop sutures with endoscopic scissors, was used to resect the protruding mesh located in close proximity to a ureteral orifice. Medline was searched for other minimally invasive treat

Totally endoscopic sublay/extraperitoneal Sugarbaker mesh repair for parastomal hernia.

INTRODUCTION: Shared consensus is that a nonslit-mesh-based laparoscopic repair technique is the optimal treatment principle for parastomal hernia (PSH). MATERIALS AND SURGICAL TECHNIQUE: An 81-year-old female parastomal hernia patient who had a previous history of laparoscopic abdominoperineal resection presented for surgical treatment. We performed a novel totally endoscopic sublay/extraperitoneal Sugarbaker mesh repair (TES-Sugarbaker) for this disease. The mesh was deployed in the sublay/extraperitoneal plane in a Sugarbaker configuration. DISCUSSION: TES-Sugarbaker repair for parastomal

ENDOSCOPIC MANAGEMENT OF MESH MIGRATION FOLLOWING HIATAL HERNIA REPAIR.

BACKGROUND: The use of mesh in the repair of large hiatal hernias is still controversial. One of the most feared adverse events related to the use of mesh is erosion into the esophageal and gastric walls. AIMS: To record the endoscopic treatment of mesh that has migrated into the gastric lumen after surgical treatment of hiatal hernia. METHODS: The technical option was to wait for the progressive migration of the mesh into the gastric lumen, monitoring with upper digestive endoscopy, with removal by traction at the best time, with the aid of foreign body forceps. RESULTS: The mesh was compl

Time to reconsider the use of synthetic mesh in immediate prepectoral implant-based breast reconstruction: Impact of their use on short-term outcomes.

BACKGROUND: Breast reconstruction practices, predominantly implant-based, have evolved, with meshes aiding in overcoming traditional limitations. However, data comparing mesh-assisted prepectoral reconstruction with implants alone are lacking. This study aimed to assess whether synthetic meshes in prepectoral reconstruction impact postoperative complications. MATERIAL AND METHODS: We retrospectively studied 238 prepectoral immediate implant-based breast reconstructions (IBBR) in 211 patients from 2020 to 2022. Our primary endpoint was the 90-day revision surgery rate comparing mesh and non-me

Modified Implantation of Self-Adhesive Mesh in Laparoscopic Total Extraperitoneal Inguinal Hernia Repair.

Introduction: The self-adhesive mesh has become increasingly popular since its launch and has been proven to be reliable in laparoscopic hernia reparation. However, self-adhesive mesh may encounter unexpected adhesions during operation because one side of the mesh was covered with microgrips. Objective: Performing a modified technique of self-adhesive mesh implantation to avoid unexpected adhesions to the mesh itself and to the surrounding tissues in the operation space. Methods: We carried out a modified self-adhesive mesh implantation during May 2017 and March 2018. The modification was usin
