Does Intestinal Peristalsis Cause Suture Failure After Instrument Suture?

BACKGROUND/AIM: Gastrectomy with lymphadenectomy is a standard treatment for gastric cancer. Anastomotic leakage remains a potentially fatal complication of gastrectomy. Forceful stapler extraction may cause anastomotic complications. We focused on the duodenal peristalsis, as we hypothesized that it might cause forceful stapler extraction. We then retrospectively investigated duodenal peristalsis and reviewed videos of Da Vinci system cases to clarify the relationship between peristalsis and anastomotic complications. PATIENTS AND METHODS: Forty-nine cases with stored videos of laparoscopic

A biomechanical comparison of a mesh suture to a polyblend suture in a porcine tendon model.

BACKGROUND: The suture-tendon interface turned out to be the weak point of a repaired rotator cuff. A double rip-stop (DRS) technique was developed to enhance the strength of the suture-tendon interface. The first aim of this study was to compare the suture-tendon interface strength between mesh suture and the No. 2 FiberWire (FW), which is commonly used in the clinic. The second aim was to compare the biomechanical properties of rotator cuff repair between mesh suture and No. 2 FiberWire using a typical suture-bridge (SB) and DRS techniques. METHODS: Eighteen porcine subscapularis tendon (SS

The suction-assisted endoscopic suture technique: A simple method for endotracheal suturing.

BACKGROUND: Silicone stents are commonly employed to treat subglottic stenosis. A frequent complication is the tendency of stents to migrate. As such, various techniques to secure stents have been developed over the years, none of which have gained large popularity. We present a novel, low-cost, and easy-to-perform technique herein. OBJECTIVES: To describe a novel surgical technique to secure silicone stents and prevent migration for management of subglottic stenosis. MATERIALS AND METHODS: After standard excision and dilation of stenotic portions in the subglottic or trachea. A silicone ste

Lens aberrations and their relationship with lens sutures for species with Y-suture branches.

Work remains to be done to understand the origins of ocular aberrations. We analyze lens aberrations of several species with Y-suture branch … hree species. Moreover, we observe a high degree of correlation between the orientation of the lens sutures and the axis of nonrotationally symmetric aberrations. Our results point to lens sutures as the histological origin of the most significant lens aberrations: astigmatism, coma, and trefoil, but we are unable to find a major suture governing all the …

Outcome of conjunctival autograft sutured with polyamide black sutures in pterygium surgery.

INTRODUCTION: Pterygium excision with conjunctival autografting is the most effective way of treating pterygium with the least recurrence rate. The sutures which can be used to suture the conjunctival autograft are absorbable polyglactin 10-0 sutures or nonabsorbable polyamide 10-0 sutures. The polyamide sutures are inert, elicit minimal acute inflammatory reaction, do not support infection and allow easy removal without tissue adherence. They are very cost-effective compared to polyglactin sutures. OBJECTIVE: This study was aimed to assess the efficacy of the cost-effective polyamide sutures…

Normal sutural fusion and the etiology of single sutural craniosynostosis: the microspicule hypothesis.

Single sutural craniosynostosis is a disorder wherein a calvarial suture fuses prematurely, resulting in an abnormally shaped head. Children afflicted are normal neurologically, and the bone bridging the sutures is normal histologically. The mechanism of normal or pathologic sutural fusion is unknown. These facts prompted the authors to reexamine normal sutural anatomy and the concepts of skull growth in an animal model. Histochemical staining to identify osteoblasts and osteoclasts and tetracycline labeling were performed on neonatal rabbit calvarial sutures. Osteoblasts are found on all bony…

[Surgical treatment of carcinoma of the esophagus: automatic suture versus manual suture].

The authors have reviewed the anastomosis and sutures utilized for the surgical treatment of 38 patients with carcinoma of the esophagus and cardia. They have compared results following the stapled anastomosis and sutures with those following hand sutured procedures (30 anastomosis and 13 sutures) performed by the same group of surgeons. The stapled anastomosis were performed utilizing a new circular stapling device and gastric and jejunal stapled sutures, utilizing Nakayama's clamp. When anastomosis and sutures were performed by hand suturing, standard techniques were utilized. Patients were …

Bacterial adherence to surgical sutures. A possible factor in suture induced infection.

Surgical sutures are known to potentiate the development of wound infection. The purpose of this study was to investigate whether the capability of bacteria to adhere to various types of sutures has a significant effect on their ability to cause infections. Bacterial adherence to sutures was quantitatively measured using radiolabeled bacteria. In vitro adherence assays revealed remarkable variations in the affinity of bacteria to the various sutures: nylon bound the least bacteria while bacterial adherence to braided sutures (silk, Ti-cron, Dexon) was five to eight folds higher. The degree of …

Poor knot security when closing running sutures with three threads.

BACKGROUND: Knot security varies with half-knot and half-hitch sequences, rotation sense, and the type and diameter of sutures. OBJECTIVES: To evaluate the security of knots with three threads, as used for tying off running sutures. METHODS: Three-way randomised experiments, with a factorial design and 10 knots/cell. The first experiment evaluated the knot security of a surgical knot (H2H1sH1s) for single stitches (2 threads) or running sutures (3 threads), using polyglactin 910 or polydioxanone, of 2-0 or 1 diameter. The second experiment evaluated the knot security of a running suture with…

Mechanical performance of sutures exposed to natural extract-based mouthwashes.

This study examined the effects of natural herbal mouthwashes, dipotassium glycyrrhizinate, propolis extract, and a 1:1 combination of both, on the tensile strength of sutures commonly used in oral surgical procedures. Two types of absorbable sutures, polyglactin (PGA) and poliglecaprone 25 (PGCL), in sizes 3/0 and 4/0, were evaluated. Tensile strength was measured before and after immersion in five media (artificial saliva, chlorhexidine, and three herbal solutions) for 24 h, 3 days, 1 week, and 2 weeks. A universal testing machine applied a 50 N load at a constant rate of 2 mm/min. Statistic…

Lariat knot: simple tying technique for robotic and endoscopic sutures.

Minimally invasive cardiac surgeries, including robot-assisted and other minimally invasive cardiac surgeries (MICS), have advanced, allowing procedures like mitral valve repair under robotic or video-assisted guidance. Ensuring complete closure of atrial orifices to prevent postoperative bleeding is crucial. This paper introduces the "Lariat knot," a simple and effective technique for secure initial ligation. The Lariat Knot is created by double wrapping the suture to form a loose knot, and then passing the free end through this knot to create an additional loop. The initial loose knot is tig

Ultrasonographic assessment of early leakage in intestinal sutures in dogs.

Intestinal suture dehiscence is one of the most feared complications following gastrointestinal surgery in both human and veterinary medicine, increasing the morbidity and mortality of these patients. Clinical and laboratory early signs of septic peritonitis are not always easily identifiable while prompt treatment should help decrease postoperative morbidity and mortality. The aim of this study is to describe the ultrasonographic (US) features of confirmed leakage of intestinal sutures (LIS) and to evaluate if this imaging technique can be useful as noninvasive tool for the early diagnosis of…

A Review of Barbed Sutures-Evolution, Applications and Clinical Significance.

Surgical ligatures are a critical component of any surgical procedure since they are the device that provides immediate post-surgical tissue apposition. There have been several studies to improve the design and use of these wound closure devices for different surgical procedures. Yet, there is no standardized technique or device that can be used for any specific application. Over the last two decades, there has been an increased focus on the innovative surgical sutures known as knotless or barbed sutures, along with studies focusing on their advantages and disadvantages in clinical environment…

Unusually isolated Staphylococcus arlettae in intra-oral sutures - Case series.

INTRODUCTION: The human oral cavity comprises various niches such as teeth, gingiva, tongue, soft and hard palate, and various dental prostheses, all inhabited by different bacterial species. Although more than 600 taxa belong to the oral cavity, identifying Staphylococcus arlettae , an incompletely understood bacterium, has been rare. METHODS: Three patients who underwent periodontal flap surgeries were reported with the incidental finding of S. arlettae associated with the intra-oral sutures placed. Environmental sampling was performed, to establish the exact source of this bacterium. RESU…
