Swallowing dysfunction in nephropathic cystinosis.

BACKGROUND: Nephropathic cystinosis causes renal failure in most patients at approximately 10 years of age. This can be prevented or retarded by cystine-depleting therapy with oral cysteamine. Many patients who do not receive adequate cysteamine therapy undergo renal transplantation, but the accumulation of cystine continues in other organs, resulting in various clinical abnormalities. We report age-related swallowing dysfunction in patients with nephropathic cystinosis. METHODS: We studied 43 patients with cystinosis (24 who had received a renal transplant and 19 who had not), 3 to 31 years

Surgical management of swallowing disorders.

Many patients with severe swallowing disorders will require surgical intervention at some point in the course of their illness. Surgery should be designed to address specific dysfunctions identified by a thorough evaluation. Defining a patient's problem is usually a multidisciplinary effort, requiring the skills of several specialists.

Swallowing-induced supraventricular tachycardia.

A rare form of paroxysmal supraventricular tachycardia (SVT), associated with swallowing solid food, is described. Repeated triggering of the arrhythmia during esophagoscopy was produced by mechanical stimulation of the anterior esophageal wall, 30 cm from the incisors. Treatment with verapamil abolished this tachycardia. Esophagoscopic examination, the first of its kind in this setting, aided the investigation.

Act of swallowing in the fixed larynx.

In a 16-year-old female, complete stenosis of the larynx and hypopharynx developed as a consequence of the ingestion of lye cristals. Supraglotic laryngectomy, plastic surgery of the hypopharynx and antethoracic oesophagus were performed and the upper aero-digestive tract reconstructed. The dilated larynx remained fixed by the surrounding fibrous tissue, but the sphincteric action of the mobile vocal cords was partly salvaged. In that condition, the act of swallowing could be reestablished. The pathophysiology of this new mechanism of swallowing is discussed.

The Impact of Videofluoroscopic Pulse Rate on Measures of Swallow Safety in Infants and Adults with Feeding and Swallowing Disorders.

This investigation assessed the impact of videofluoroscopic resolution on measures of swallow safety. Thirty adult and twenty infant swallow … red when lower pulse rates were employed. All 20 infant participants were classified as having safe swallows at 5 pps, despite 11 demonstrating unsafe swallows (PAS > 2) at 30 pps. This pattern was also observed in adult participants to a lesser extent. Reducing pulse rates below 30 pps significantly compromises the accuracy of swallow safety assessment fo …

Effect of bilateral cerebellar rTMS on swallowing function and swallowing phase in patients with dysphagia after stroke.

OBJECTIVE: To observe the effect of bilateral cerebellar high-frequency rTMS on swallowing function and swallowing phase in patients with PSD. METHODS: 42 patients with PSD were randomly divided into bilateral cerebellar stimulation group and sham group (stimulating coil and scalp at a 90° angle). The rTMS stimulation parameters are 10 Hz,120% rMT, with 9 s intervals and 1 s stimulus, totaling 500 pulses. Before and after 3-week treatment, the MEPs amplitude of the suprahyoid muscles of the bilateral cerebral cortex was recorded as an index to quantify the excitability of the cerebral swallow

Investigation of the relationship between expanded disability status and swallowing difficulty with swallowing anxiety in multiple sclerosis.

BACKGROUND: Swallowing difficulty (SD) is defined as any disruption in the bolus transport from the mouth to the stomach. SD is very common in Multiple Sclerosis (MS). It is thought that individuals experiencing SD develop fears such as choking and inability to swallow, along with the development of Swallowing Anxiety (SA), and SA could be a symptom that may appear before the signs of SD. OBJECTIVES: To investigate the relationship between disability levels and SD with SA in MS. STUDY DESIGN: A cross-sectional correlational study. METHODS: The study included 57 MS patients with a mean age o

Comparison of swallowing dynamics between jelly and thickened liquid commonly used for swallowing training in Japan.

This study investigated the swallowing dynamics of jelly, thickened liquid, and thin liquid in selected stroke patients who exhibited near-normal swallowing function with screening tests. Videofluoroscopic examination compared the pharyngeal transit time (PTT), pharyngeal delay time (PDT), and laryngeal elevation delay time (LEDT). Of 175 patients (104 men, 71 women; mean age: 68.6 ± 12.0 years) evaluated, 24 (13.7%) experienced aspiration, significantly prolonging LEDT in swallowing thin liquid. PTT did not differ in swallowing jelly, thickened liquid, or thin liquid among the patients who di

Risk of Aspiration Increased by Post-swallow Residue in Infant Fiberoptic Endoscopic Evaluation of Swallowing (FEES).

OBJECTIVE: To assess the risk of aspiration associated with post-swallow residue subsites in Fiberoptic Endoscopic Evaluation of Swallowing (FEES) in bottle-fed infants <1 year of age. METHODS: This is a retrospective matched-pairs cohort study at an academic tertiary children's hospital. FEES and Videofluoroscopic Swallowing Study (VFSS) trials performed within the same infant <5 days apart were paired by matching bolus consistency and bottle flow rate. Positive aspiration was defined by the "or rule" in which aspiration is positive when either FEES or VFSS within a matched pair is positive.

Cervical auscultation as an adjunct to the clinical swallow examination: a comparison with fibre-optic endoscopic evaluation of swallowing.

This prospective, single-blinded study investigated the validity and reliability of cervical auscultation (CA) under two conditions; (1) CA-only, using isolated swallow-sound clips, and (2) CSE + CA, using extra clinical swallow examination (CSE) information such as patient case history, oromotor assessment, and the same swallow-sound clips as condition one. The two CA conditions were compared against a fibre-optic endoscopic evaluation of swallowing (FEES) reference test. Each CA condition consisted of 18 swallows samples compiled from 12 adult patients consecutively referred to the FEES clin…
