Experimental tendon grafting within intact tendon sheath.

A chicken toe experimental model was used to study the fate of flexor tendon grafts, introduced into intact tendon sheaths. After perfusion of the limb by India Ink, the vascular pattern of the graft and the synovial sheath could be identified. The grafts showed no or few adhesions to the sheath. With time, there was a conversion from a diffuse superficial vascular pattern towards a concentration of vessels on the dorsal aspect of the graft. In a control group, where the sheath had been resected, adhesions were more apparent, and could be observed still after 16 weeks. These results are in com

Biological effects of extracorporeal shockwave therapy in tendons: A systematic review.

Extracorporeal shockwave therapy was initially used for kidney stone disintegration and its application was then extended to calcific tendinitis. The therapeutic field expanded and included numerous types of tendinopathies, from shoulder to plantar fascia. The clinical benefits were documented in trials and the effects and mechanisms were studied on models including animal and human tendons. The present systematic review outlines a large spectrum of biological effects. First, an optimal dose is adapted for each species and each tendon; exceeding the optimal dose may lead to structural injury.

The Challenges in restoration of extensor tendons function at the hand.

INTRODUCTION: The authors discuss challenges in extensor function restoration at the finger level following distal posterior interosseous or tendon complex injuries, according to the typologies of lesions or the specific patient requirements. MATERIALS: The authors report on two cases describing challenging resolutions. One patient with EDC lag from zone 6B to 8 requiring FCU prolonged with cadaveric grafts.  One young patient with distal posterior interosseous lesion requiring selective tendon transfers to EPL and EIP to restore selective and autonomous index extension. RESULTS: In both cas

Irreducible Metacarpal Neck Fracture due to Interposition of the Extensor Tendons.

A 15-year-old male presented with multiple right-hand fractures, including a severely angulated small finger metacarpal neck fracture. Multiple closed attempts failed, and open reduction was performed. At surgery, the extensor tendon was found to be interposed within the fracture, thereby preventing closed reduction. The tendon was extricated from the fracture site, adequate reduction was obtained, and the fracture was stabilized using K-wires. The fracture united well, with good return of motion and strength. Extensor tendon interposition is a rare scenario associated with metacarpal neck fra

Gout tophus in transferred tendons after 33 years: A case report.

Surgical site gout is an extremely rare complication that is difficult to diagnose, particularly in patients without a history of gout. A 57-year-old male patient was admitted with no previous history of gout, complaining of surgical site gout located at the junction where flexor carpi ulnaris tendon was transferred to extensor digitorum communis tendon after 33 years of the initial surgery. The patient was presented with a progressive swelling over the last three months which was located on the dorsoulnar side of the right wrist joint. Magnetic resonance imaging revealed an iso/hypointense ma

How to visualize the innervation pattern in tendons: A methodical guide.

BACKGROUND: Tendon pathologies are common and several data suggests that the peripheral nervous system is involved in this disorder. Immunohistochemistry (IHC) is one of the pillars to characterize nervous structures and their implication in the pathogenesis of chronic tendon pain. Most commonly, formalin-fixed, paraffin-embedded (FFPE) tendons are used for immunohistochemical characterization of the innervation. However, FFPE specimens exhibit major disadvantages: First, antigens (proteins) are masked and antigen retrieval is necessary to restore antigenicity. Second, FFPE specimens involve i

Ultrasonic Percutaneous Tenotomy of Common Extensor Tendons for Recalcitrant Lateral Epicondylitis.

Tennis elbow is a common musculoskeletal condition affecting middle-aged patients with symptoms usually lasting from 6 months to 2 years. The vast majority of individuals will respond to conservative therapy; however, some will require surgical intervention. A new treatment system has been developed for use with ultrasound guidance in the ultrasonic microresection of tendinopathic tissue. This technology has been implemented in the TX1 Tissue Removal System and is used to treat various tendinopathies by debridement using targeted ultrasonic energy. We describe the surgical technique for the TX

[Injuries of major tendons : Review of current diagnostic and surgical standards].

Chronic degenerative tendon injuries as well as injuries due to acute high-energy trauma and incisional injuries are a significant aspect in the emergency surgery department. As a result of anatomical characteristics and exposure certain tendons are particularly vulnerable to injury. These include the biceps brachialis, quadriceps femoris, patella, achilles and tibialis anterior tendons. Besides the broad spectrum of non-surgical treatment, surgeons should have knowledge of the various surgical techniques including suturing, anchor refixation and reconstruction techniques. The indications for

Effects of gamma irradiation on the biomechanical properties of peroneus tendons.

PURPOSE: This study was designed to investigate the biomechanical properties of nonirradiated (NI) and irradiated (IR) peroneus tendons to determine if they would be suitable allografts, in regards to biomechanical properties, for anterior cruciate ligament reconstruction after a dose of 1.5-2.5 Mrad. METHODS: Seven pairs of peroneus longus (PL) and ten pairs of peroneus brevis (PB) tendons were procured from human cadavers. The diameter of each allograft was measured. The left side of each allograft was IR at 1.5-2.5 Mrad, whereas the right side was kept aseptic and NI. The allografts were t

Use of a Hunter Rod for Staged Reconstruction of Peroneal Tendons.

Peroneal tendon pathology is a commonly reported cause of lateral ankle pain. The causes include cavovarus foot type, overuse, chronic tendinosis, peroneal subluxation or dislocation, acute traumatic split tears, and traumatic rupture. The purpose of the present report is to describe an alternative approach for surgical reconstruction of the peroneal tendons in patients when repair might no longer be effective. The use of a Hunter rod was originally described by Hunter in 1971 for 2-stage reconstruction of tendons in the hand. We present a 2-stage surgical technique with the use of a Hunter ro

Forearm position's alteration of radial-head impingement on wrist-extensor tendons.

CONTEXT: Lateral epicondylalgia, pain at the lateral elbow, is commonly associated with extensor carpi radialis brevis tendinopathy. The radial head, which abuts the extensor tendons and is elliptical in shape, may affect the extensor tendons during pronation of the forearm. Cadaverous studies have shown that the radial head may act as a cam in pronation, to offer a mechanical advantage to the common extensor tendon and to mitigate load on the origin of the extensor carpi radialis brevis tendon. OBJECTIVE: To determine the effect of radial-head position on the wrist-extensor tendons in vivo.

Transection of the lateral and long digital extensor tendons in racehorses.

OBJECTIVE: To report on the outcome of wounds to the hindlimb of racehorses, and horses with the intended use of racing, where both the long digital extensor (LoDE) and lateral digital extensor (LaDE) tendons had been completely transected. DESIGN: Retrospective case series. METHODS: Records of all horses admitted with hindlimb lacerations between 2002 and 2009 were examined for cases where both the LoDE and LaDE tendons in the hindlimb had been severed, and specifically in horses intended to be used for racing. Outcome was assessed via retrieval of race records and via telephone questioning

Muscle, tendons, and bone: structural changes during denervation and FES treatment.

OBJECTIVES: This paper describes a novel approach to determine structural changes in bone, muscle, and tendons using medical imaging, finite element models, and processing techniques to evaluate and quantify: (1) progression of atrophy in permanently lower motor neuron (LMN) denervated human muscles, and tendons; (2) their recovery as induced by functional electrical stimulation (FES); and (3) changes in bone mineral density and bone strength as effect of FES treatment. METHODS: Briefly, we used three-dimensional reconstruction of muscle belly, tendons, and bone images to study the structural

Longitudinal tears of peroneus longus and brevis tendons: a gouty infiltration.

The natural history of recurrent uric acid deposition includes joint destruction, as well as tendon weakening and rupture. Simultaneous rupture of the peroneus longus and brevis tendons secondary to tophaceous gout is very rare. In the present report, we describe the case of a 37-year-old man who had a history of recurrent ankle pain and 4 previous episodes of acute gout localized to his ipsilateral first metatarsophalangeal joint. The physical examination revealed the ankle pain to actually be localized to the peroneal tendons immediately distal to the fibular malleolar groove. Magnetic reson

Tenotomy of the tensor tympani and stapedius tendons in Ménière's disease.

UNLABELLED: Tenotomy of the tensor tympani and stapedius tendons in Ménière's disease. OBJECTIVE: In Ménière's disease (MD), when patients have incapacitating vertigo that is resistant to drug treatment, an intratympanic gentamicin application (ITG) is often proposed. Recently, some authors suggested that tenotomy, sectioning of the tensor tympani and stapedius tendons, could be a promising treatment. We examined whether tenotomy (ST) has additional benefit, compared to ITG alone, with respect to tinnitus, vertigo, and quality of life. METHODOLOGY: We conducted a retrospective survey of the

Dynamic splinting for the repair of the first extensor compartment tendons.

As a hand therapist, we don't really treat a lacerated tendon. The physician is the one that handles the repair. We actually are treating all the secondary problems that occur as a result of the injury and the repair. To quote Ken Flowers (JHT past editor), " Scar Wars" can end up being one of our biggest challenges. This author has demonstrated how adhesions can affect motion after a repair of tendons in the first extensor compartment. He has also designed a splint to help with "battling" this dilemma.-Peggy Fillion, OTR, CHT, Practice Forum Editor.

Tibial press-fit fixation of the hamstring tendons for ACL-reconstruction.

Press-fit fixation of patellar tendon bone anterior cruciate ligament autografts is an interesting technique because no hardware is necessary to achieve fixation. Up till the present point, there is no biomechanical data available for the tibial press-fit fixation of the hamstring tendons. Hamstring tendons of 21 human cadavers (age: 41.9 +/- 13.1 years) were used. A press-fit fixation with looped semitendinosus and gracilis tendons secured by a tape (T) over a bone bridge, or by a baseball-stitched suture (S), was compared with degradable interference screw fixation (I) in 21 porcine tibiae.

[Management of affected flexor tendons in rheumatoid arthritis of the hand].

The affection to the flexor tendons of the patient with rheumatoid arthritis represents a substantial rheumatic change in the hand, which is characterized by restriction of movement, ulnar deviation in the level of the metacarpophalangeal joint and palmar incomplete dislocation. Early treatment by removing any possible constriction areas in synovial proliferations in the area of the flexor pullies or a complete tendosynovectomy makes an extensive restitution possible. In the case of only one ruptured flexor tendon, it can be treated by the transfer of the neighboring superficialis tendon or a

Secondary reconstruction of the extensor tendons and overlying soft tissue deficiency.

Reverse-flow pedicled radial forearm tendocutaneous flap transfers were performed to reconstruct simultaneous skin and tendon defects in the dorsum of the hand in six patients. All flaps survived and four of six patients obtained good functional results. To obtain satisfactory functional outcome, early surgical repair is critical after trauma. The only disadvantage of this operation is unacceptable donor site morbidity.
