Palliative care in Kerala, India: a model for resource-poor settings.

BACKGROUND: The need for palliative care is increasing throughout the world. Developing countries are progressively faced with an ageing population, whose changing health needs require urgent action. Insufficient access to care for both cancer and AIDS patients make palliative care the only humane and feasible response in many contexts. MATERIAL AND METHODS: In order to describe the need for palliative care in Kerala and the development of the new palliative care policy, we have collected and reviewed information from government officials, health professionals involved in cancer care, patient

Place of death for people with dementia: A scoping review.

BACKGROUND: Dementia is a global health concern that contributes significantly to severe health-related suffering and poses high demands on healthcare systems, patients, families, and caregivers. Caring for people with dementia should include all the stages of illness, including end-of-life care. AIM: Given that the place of death is a crucial aspect of end-of-life care, we aimed to analyze the scientific literature on the place of death of individuals with dementia, the sociodemographic and contextual variables associated with it, and evidence-based suggestions to improve end-of-life care fo

Biological Effects of Music Therapy in End-of-Life Care: A Narrative Review.

Background and Objectives: Music therapy has a long tradition in palliative care, and recent studies have investigated its Neuro-Psycho-Endocrine-Immunological (NPEI) effects in terminally ill patients. Despite numerous published articles, there is a lack of a compendium connecting the physiological basis of music therapy with the specific musical elements most effective in end-of-life settings. This narrative review aims to synthesize current evidence on the physiological mechanisms underlying responses to music, with a focus on terminal patients and implications for nursing practice. Symptomatic Control in End-of-Life Patients.

End-of-life patients present a variety of symptoms that cause suffering for them and their respective families. Health professionals throughout their university, internship and medical careers are ill-prepared to manage and improve the quality of life of these patients. This article aims to provide basic skills in the symptomatic management of end-of-life patients, focusing in particular on the control of pain, dyspnoea, fatigue, nausea, vomiting and anorexia. It also aims to draw attention to basic concepts of control concerning refractory symptoms and palliative sedation.

The use of chlorpromazine for symptom control in dying cancer patients.

Terminal restlessness is a significant problem in deaths from advanced cancer, it is difficult not only for patients, but for family and health-care providers also. Chlorpromazine is an antipsychotic phenothiazine, safe at high doses and by many routes. It has also been used as an adjunct to morphine in advanced cancer. We have conducted a study using chlorpromazine in 20 patients, in which the drug was administered intravenously (i.v.) or rectally (PR) in inpatients and outpatients for terminal restlessness and dyspnea. The median PR dose was 25 mg every 4-12 hr. The median IV dose was 12.5 m

Care at the end of life: What really matters to people?

BACKGROUND: Providing high quality end of life care requires all health professionals, including general practitioners (GPs), to possess an appreciation of what issues are likely to be important to people in their final stages of life. This includes timely conversations to discuss and document people's expectations for their care. OBJECTIVE: The objective of this paper is to summarise the major issues people have defined as important contributors to high quality end of life. This includes a summary of the issues and ways to consider assessment and management strategies. DISCUSSION: As death

Drugs, delirium, and ethics at the end of life.

For older persons with delirium at the end of life, treatment involves complex trade-offs and highly value-sensitive decisions. The principles of beneficence, nonmaleficence, respect for autonomy, and justice establish important parameters but lack the structure necessary to guide clinicians in the optimal management of these patients. We propose a set of ethical rules to guide therapeutics-the canons of therapy-as a toolset to help clinicians deliberate about the competing concerns involved in the management of older patients with delirium at the end of life. These canons are standards of jud

Caring for the dying in a developing country, how prepared are we?

End of life care is framework to allow for a peaceful, comfortable and dignified death while considering the patients' personal and religious values, bioethics and knowledge of the disease process. A well planned end of life pathway should allow for the flexibility to shift from an active (or aggressive) treatment approach to one of comfort and care when initial interventions have failed. The need for this pathway is most apparent in the intensive care setting. Implementation of a pathway will face various challenges due to religious and cultural beliefs, education of healthcare providers to c

The management of constipation in end-stage disease.

A rectal examination may be considered an invasive intervention in a frail patient, but it is preferable to a blanket approach to treatment, which may be ineffective or do more harm than good, for example, giving antidiarroeals when the diarrhoea is spurious secondary to impaction, or continuing to soften soft faeces when in fact a stimulant laxative is required. The rectal examination can be modified to a gentle inspection. It should not need to be performed repeatedly if the regular regimen is altered according to the findings. This approach prevents the need for routine suppositories, enema

Healthcare interventions improving and reducing quality of life in children at the end of life: a systematic review.

BACKGROUND: Children with serious illness suffer from symptoms at the end of life that often fail to be relieved. An overview is required of healthcare interventions improving and decreasing quality of life (QOL) for children with serious illness at the end of life. METHODS: A systematic review was performed in five databases, January 2000 to July 2018 without language limit. Reviewers selected quantitative studies with a healthcare intervention, for example, medication or treatment, and QOL outcomes or QOL-related measures, for example, symptoms, for children aged 1-17 years with serious ill

Treatment of terminal restlessness: a review of the evidence.

Terminal restlessness is an important issue in the management of symptoms at the end of life with between 25% and 88% of dying patients exhibiting this condition. (1-5) The purpose of this review was to examine the empiric evidence about the pharmacological treatment for terminal restlessness. All available literature, in all languages, from 1966-2002 including randomized trials, concurrent observations, retrospective chart reviews, single case studies, clinical practice guidelines, expert consensus, single expert opinion articles, and editorials were reviewed as "best available evidence." Levomepromazine (methotrimeprazine) and the last 48 hours.

Levomepromazine (previously known as methotrimeprazine) has a broad range of beneficial effects in the terminal phase of many illnesses, resulting from its combined antipsychotic, anxiolytic and sedative actions. Levomepromazine can safely be administered in a continuous subcutaneous infusion with most other commonly used drugs in palliative care.

Management of malignant bowel obstruction in patients with advanced cancer at the end of life.

BACKGROUND: Malignant bowel obstruction in patients with advanced cancer at the end of life is common. Patients may have substantial symptoms (pain, nausea and vomiting) and experience aggressive care at the end of life. Due to the lack of robust evidence, the care algorithm of treatment for patients at the end of life is not standardized. Patient's prognosis, clinical condition and patient's preferences related to goals of care discussion must be considered when opting between comfort-focused care (conservative/pharmacological treatment), palliative (nonsurgical) procedures and palliative sur

Christian perspectives on palliative sedation: a literature study.

BACKGROUND: Christianity has always been concerned about the fate of the sick and the dying. Charity is one of the seven Christian virtues and is seen as an act of love for one's neighbour. At the same time, Christianity is a strong defender of the dignity of human life, especially the most vulnerable. In the context of advanced disease and end of life, all major Christian traditions (Anglican, Catholic, Eastern Orthodox and Protestant) advocate appropriate care to protect the dignity of those suffering from illness or facing the prospect of death. This translates into a strong opposition to a

[Palliative psychiatry; .an exploration].

BACKGROUND: Many psychiatric disorders have a chronic and/or recurrent course. The suffering of these patients and their relatives is usuall … s in chronic mental health care. To avoid association of such ‘alleviating’ policy with terminal care, the term ‘oyster’ care is preferable. Sometimes this can be an alternative to a self-chosen end of life.

Methodological reflections to support good practice in using nominal group techniques: Insights from applications in palliative care studies.

High-quality recommendations require rigorous methods based on strong evidence to improve clinical practice. In palliative and end-of-life care, expert consensus is sometimes achieved through nominal group techniques. However, its practical challenges are often underestimated, potentially compromising the rigour and the quality of the recommendations.Methodological reflections:The methodological reflections on developing recommendations using the nominal group technique are discussed in this paper. These reflections are drawn from its theoretical foundations and applications in palliative care

The Burden of having to Wonder: Hospice Caregiving Experiences of LGBTQ+ Cancer Family Caregivers.

BACKGROUND AND OBJECTIVES: LGBTQ+ people are more likely to be caregivers for family and friends with life-limiting illnesses than non-LGBTQ+ people. LGBTQ+ caregivers may also experience stigma, bias, and discrimination, in addition to caregiving stress. Yet few studies have elicited LGBTQ+ family caregivers' perspectives on their end-of-life (EOL) experiences of home hospice. RESEARCH DESIGN AND METHODS: We conducted semi-structured interviews with LGBTQ+ family caregivers of home hospice patients (N = 20). Following an interpretive descriptive approach, interview data were audio recorded,
