Pertussis toxin: the cause of the harmful effects and prolonged immunity of whooping cough. A hypothesis.

The nature of the pathogenesis and of the prolonged immunity of whooping cough has not been clearly defined. The literature of Bordetella pertussis indicated that only the antigen that induces histamine sensitization, lymphocytosis, and other biological reactions in mice is the cause of the harmful effects and prolonged immunity of whooping cough. This antigen has the general characteristics of bacterial protein exotoxins that cause the harmful effects of infectious diseases such as diphtheria and tetanus. It is proposed that this antigen, which is histamine-sensitizing, lymphocyte-leukocyte-p

[The relationships between antitoxic titers determined in a group of young people before and after booster ADT by the in vivo neutralization test. Comparative NT-PHAR evaluations].

The geometrical mean of antitoxic titers determined in a lot of immunized young men who received, 5-7 years before, a booster with diphtheria-tetanus bivaccine, was of 1.54 (x/divided by 10.54) I.U./ml. At one month after another booster, performed with Tetanus toxoid (adsorbed), an increase of 34 times was recorded, the geometrical mean reaching the value of 52.67 (x/divided by 10.42 I.U./ml). The covering coefficients of the minimal protective antitoxin level have increased from 154 to 5267, in the estimation performed at the level of geometrical mean (calculating the ratio between the value

Thirteen cases of tetanus in dogs.

The records of 13 dogs with tetanus were reviewed, 12 of the dogs survived and were discharged, but the other died as a result of the acute onset of hyperthermia. Long-term follow up was available for 10 of the survivors of which nine were reported to be normal. One case was euthanased after a cervical spine fracture four months after it was discharged. Complications encountered during management included aspiration pneumonia in three cases, urinary tract infection in two cases, and upper respiratory tract obstruction, hiatal hernia, coxofemoral luxation, seizures and respiratory arrest in one

Bronchoscopy-induced fatal asystole in tetanus: the result of combined carotid-body chemoreceptor and vasovagal reflexes.

A 44-year-old man with tetanus had bradycardia with hypotension and then died suddenly when asystole developed during bronchoscopy. Although signs and symptoms of sympathetic hyperactivity are characteristic of tetanus, vagal stimulation via vasovagal and carotid-body chemoreceptor reflexes may produce sudden, unexpected, and catastrophic effects.

Naturally occurring eastern equine encephalitis in a Hampshire wether.

Eastern equine encephalitis (EEE) was diagnosed (postmortem) in a sheep with clinical signs attributable to a central nervous system disease … nt included hydrotherapy for lowering body temperature, intravenous fluids, thiamine hydrochloride, tetanus antitoxin, antibiotics, and corticosteroids. The only gross lesion at the time of necropsy was a wet glistening surface of the brain (leptomeninges). Microscopically, there was severe nonsuppurative meningoencephalitis, poliomyelitis, and polyradiculoneuritis …

[Effect of tetanic anatoxin and carbonic acid level in blood on biosynthesis of antibodies in rats].

The paper deals with dynamics of antibody formation in rats. Tetanic anatoxin independently of doses and methods of administration manifests weak immunogenic properties. A complete Freind adjuvant stimulates considerably the biosynthesis of antitetanic antibodies in the primary and secondary immune responses. Electrophoresis of tetanic anatoxin polyacrylamide gel permits isolating two protein fractions, one of them migrates in the immunoglobulin G zone and its mass constitutes 73%, the other-more low-molecular and contains 27% of protein. Dynamics of the acid-base blood state indices and of an

Control testing of combined vaccines: a consideration of potential problems and approaches.

Vaccines comprising combinations of diphtheria, tetanus and pertussis (DTP) components with Haemophilus influenzae b polysaccharide--protein conjugates (DTP-Hib) are now available. Combinations of DTP-Hib with additional components such as inactivated poliomyelitis vaccine, hepatitis B vaccine, meningococcal and pneumococcal polysaccharide-protein conjugates are under development. Other combinations, such as Hib vaccine with meningococcal A, B and C components and possibly pneumococcal conjugates, or non-capsulated Haemophilus components combined with pneumococcal conjugates, developed against

Intravenous magnesium sulphate infusion as first-line therapy in the control of spasms and muscular rigidity in childhood tetanus.

Background: Reports suggest that, in tetanus, magnesium sulphate (MgSO4) alone may control muscle spasms, thereby avoiding sedation and mechanical ventilation. Aim: To study the efficacy and safety of intravenous MgSO4 in controlling spasms and rigidity in children with tetanus. Methods: All children with tetanus consecutively admitted over a 2-year period in a tertiary-care teaching hospital were recruited. In addition to human tetanus immunoglobulin and parenteral metronidazole, patients received MgSO4 100 mg/kg intravenously followed by infusion at 40 mg/kg/hr. The infusion rate was increas

Intradermal antitetanic-antityphoid booster by jet injection.

The immunogenicity and the reactogenicity of an unadsorbed Tetanus vaccine, intradermally administered in man as a booster immunization associated or not with lyophilized Typhoid vaccine, were comparatively studied. There were no differences in postvaccinal reactions between the Tetanus vaccine administered alone or associated with Typhoid vaccine as well as between the unadsorbed and adsorbed Tetanus vaccine. The booster inoculation carried out with Tetanus vaccine by i.d. route with doses of 10 Lf/0.1 ml proved to be effective, inducing to all the subjects a definite protective titre, mainta

Tetanus after anorectal abscess.

As illustrated by this case report, tetanus can occur as a complication of anorectal surgical procedures or abscesses just as it can in other wounds. A synergistic infection of the perineum occurred in a 62-year-old man 8 days after drainage of an anorectal abscess. He was treated with vigorous debridement and antibiotics and was given tetanus prophylaxis. The next day, tetanus developed, presumably from the original abscess. The patient recovered after aggressive therapy, including muscle relaxants and ventilatory support.

Tetanus prophylaxis following ocular injuries.

The administration of prophylaxis against tetanus following a corneal abrasion is routinely performed in many acute care facilities, despite a lack of support in the literature for its necessity. The risk of developing clinical tetanus from three different types of injuries to the eye was evaluated in an animal model. Clinical tetanus was induced in unimmunized mice by injecting Clostridium tetani organisms or toxin into the anterior chamber. Immunized mice injected intracamerally did not develop signs of tetanus. Tetanus was not induced by topical inoculation of either live organisms or toxin

Active Immunization-Some Present-Day Problems: (Section of Therapeutics and Pharmacology).

Diphtheria.-Immunization is safe and effective. Compulsory measures are indicated, especially for the younger age-groups. The Schick test may be reserved for selected groups and to control modified methods. Proper spacing of doses of prophylactics and periodic reinoculation will ensure a high level of immunity. It is important to beware of "one-shot" methods. Indiscriminate swabbing is to be discouraged; high carrier rates are an indication for widespread diphtheria prophylaxis.Enteric fever.-Mass immunization is desirable in many areas, although there is no justification for compulsion except

Experience with tetanus in a tertiary hospital in south east Nigeria.

BACKGROUND: Tetanus has for long been an avoidable source of morbidity and mortality particularly in developing countries. The aim of this study was to review the experience with managing tetanus in a regional tertiary hospital in Nigeria. METHODS: A retrospective study of patients who were admitted with a clinical diagnosis of tetanus between January 1999 and December 2003 was done. Relevant data were extracted from the patients'case records. RESULTS: A total of 12 patients were identified within the time frame with a diagnosis of tetanus. There were more male than female patients (ratio 1.
