INTRODUCTION: Arthrodesis is a reliable surgical procedure for treatment of thumb carpometacarpal (CMC) osteoarthritis that provides hand strength and pain relief. Locking plate fixation is a common technique that provides rigid fixation and a high rate of bone union; however, it requires extensive surgical exploration of the first metacarpal and trapezium. Here, we report the surgical outcome of minimally invasive arthroscopy-assisted thumb CMC arthrodesis that preserves soft tissue supplying the blood flow to the bones.
MATERIALS AND METHODS: Nine thumbs of nine patients who underwent arthr

Trapeziometacarpal (TMC) arthrodesis is a reliable procedure to treat individuals with trapeziometacarpal osteoarthritis (TMC OA) who have failed conservative treatment. Arthrodesis has classically been deemed as a suitable treatment for young, active males, particularly in the post-traumatic setting, who desire maximum strength. There are numerous arthrodesis techniques described in the literature, however, reported high nonunion rates may deter surgeons from treating TMC OA with arthrodesis. A technique for TMC arthrodesis using the Speed™ Continuous Compression Nitinol Fixation Syst

BACKGROUND: Type II lunate has an extra facet with the hamate. This difference alters the biomechanics of the midcarpal joint, influencing the development of scapho-trapezio-trapezoid (STT) arthritis. We aimed to investigate whether, in patients with trapeziometacarpal (TMC) osteoarthritis (OA), involvement of the STT joint is associated with the lunate type.
METHODS: A retrospective review was performed on 121 patients (mean age of 69 years) diagnosed with TMC OA. The lunate type was classified based on the presence of the facet detected on computed tomography images. The incidence of STT OA

The aim of this study was to better quantify the role of delayed MRI scans in acute wrist injuries, and to assess the prevalence and distribution of multiple occult injuries of the wrist. A retrospective study was made of all patients who had been referred to the orthopaedic trauma clinic for a possible scaphoid fracture and with normal radiographs over a two year period. There were 110 patients. Patients were initially treated conservatively with a scaphoid cast or a futura splint for two weeks. This was then removed and the wrist examined and further radiographs taken. The patients with norm

Generally, skeletal peripheral metastases below the elbow and the knee are rare. Skeletal metastases to the hand or foot are very rare; but when they do it may be a revealing clinical finding. Purely lytic lesions are commonly seen in metastases from lung, renal, and thyroid tumors, but they are also known to occur in primary myeloma, brown tumor and lymphomas. A 70-year-old man was brought to the emergency department with acute painful swelling involving his right hand and the right knee. Due to significant accompanying soft tissue swellings cellulitis, acute osteomyelitis and gouty arthropat

The best way of immobilization as well as effectiveness of rehabilitation for trapeziometacarpal total joint arthroplasty is unknown. We aimed to identify and describe the available evidence, practice variation and knowledge gaps. The literature was searched without restrictions. 123 studies were included, reporting 21 types of prosthesis. Reported immobilization types were cast (23%), splint (18%), compression bandage (10%), or combinations (26%). In 19%, immobilization time and type was not reported. Supervised rehabilitation (22%), self-rehabilitation (11%), functional use (11%), or customi

The aim of this article is to provide an updated systematic review on the 8 most commonly used surgical procedures to treat trapeziometacarpal osteoarthritis. A thorough literature search was performed using predetermined criteria. A total of 35 articles fulfilled the inclusion criteria. Nine of these 35 articles were not included in previous systematic reviews. Systematic evaluation demonstrated the following: (1) there is no evidence that trapeziectomy or trapeziectomy with tendon interposition is superior to any of the other techniques. However, when interposition is performed, autologous t

Trapeziometacarpal (TMC) arthroplasty is a widely accepted treatment for osteoarthritis of the TMC joint. However, one potential complication is De Quervain's tenosynovitis and its cause post-arthroplasty remains unclear. The aim of this study was to evaluate whether thumb lengthening, as measured by the Scaphometacarpal Index (SMI), is associated with the incidence of De Quervain's tenosynovitis following TMC arthroplasty. A retrospective analysis was conducted of 114 TMC arthroplasties performed between 2015 and 2022. Thumb lengthening was quantified using pre- and postoperative radiographs.

PURPOSE: The extensor pollicis longus (EPL) and brevis (EPB) and abductor pollicis longus (APL) are muscles located in the posterior compartment of the forearm. These muscles allow the thumb to move independently from the other four fingers by attaching to the thumb separately. This study's goal was to investigate the tendon variations and insertions of these muscles.
METHODS: Our study was conducted on 43 fetal cadavers (86 upper extremities) aged between 17 and 40 weeks of gestation. This study investigated the tendon numbers and insertions of the EPL, EPB, and APL.
RESULTS: The tendon num

PURPOSE: Various surgical options exist to treat thumb carpometacarpal (CMC) osteoarthritis, including trapeziectomy-based procedures and CMC denervation. Presently, there is limited data on the relative longevity of these 2 procedures, with conflicting data on the rates of secondary conversion after CMC denervation. We sought to investigate the rates of secondary surgery in patients undergoing trapeziectomy or denervation for CMC osteoarthritis.
METHODS: The PearlDiver Mariner Database was retrospectively queried for patients with CMC arthritis undergoing trapeziectomy-based procedures or CM

In this retrospective study we reviewed the outcomes of 46 trapeziometacarpal joint replacements with the Moovis® prosthesis in 46 patients with painful osteoarthritis. The dual mobility design of this prosthesis aims to reduce prosthetic dislocation and subsidence. At follow-up 2 to 6 years (mean 5 years) after operation, pain was reduced from 6 to 0 on a visual analogue scale from 0 to 10. The scores of the Quick Disabilities of the Arm, Shoulder and Hand questionnaire improved significantly. Thumb motion and grip and pinch strength did not differ significantly from the contralateral hand. T

BACKGROUND: Trapeziometacarpal (TM) arthroscopy should be viewed as a useful minimally invasive adjunctive technique rather than the operation itself since it allows one to visualize the joint surface under high-power magnification with minimal disruption of the important ligamentous complex. Relatively few articles describe the arthroscopic treatment of TM osteoarthritis (OA) and the arthroscopic anatomy of the TM joint. There is lingering confusion as to whether soft tissue interposition and K-wire fixation of the joint are needed and whether the outcomes of arthroscopic procedures compare t

In carpal tunnel syndrome, the abductor pollicis brevis, which is the primary muscle for the palmar abduction of the thumb, is almost inevitably impaired. The active palmar abduction of the thumb may be a better indicator of thumb disability. The authors aimed to establish a simple and accurate method to measure the angle of active palmar abduction of the thumb and to determine the maximum angle values in healthy women. Twenty-five women 20 to 21 years old with no disorder of the hand participated in this study voluntarily. Three measurement methods were tested. The first method was designed a

BACKGROUND: Wrist injuries limit function and can lead to long-term pain and disability. Current interventions aim to decrease pain and restore healthy function; however, healthy function during dynamic motion has not been fully characterized. Four-dimensional computed tomography (4DCT) can capture dynamic bony motion providing new insight to the dynamic wrist joint.
MATERIALS AND METHODS: Ten young healthy participants were scanned using a 4DCT scanner and three-dimensional (3D) models were made of the radius and carpal bones. Using helical axes and local coordinate systems for the radius, r

INTRODUCTION: Malignant diseases with infiltration of bony structures in the area of the phalanges or metacarpals require either amputation or complex reconstruction. The decision for reconstruction means to restore length, mobility, sensibility, stability as well as aesthetics.
METHODS: We present a case of complex first ray reconstruction of the left hand using a free osteocutaneous lateral arm flap from the ipsilateral side. The reconstruction was performed after radical resection of an exulcerated squamous cell carcinoma, including the first metacarpal bone, trapezium, partial trapezoid a

With the increase in the number of trapeziometacarpal arthroplasties being done, the indications for revision due to implant wear will also increase significantly in the coming years. Isolated linear exchange in the context of polyethylene wear without osteolysis or loosening of the implants is already well known in the case of hip arthroplasty. This revision option offers the advantages of less morbidity, faster recovery, and bone preservation. We retrospectively reviewed five patients who underwent trapeziometacarpal revision with isolated polyethylene liner exchange. Revision surgery was pe

The present report is based on assessment of 48 patients who underwent carpal Silastic H.P. implant arthroplasty (trapezium, condylar, scaphoid, lunate and scapholunate implants). Mean follow-up period was 29 months (range 6-82). Recurrent pain and/or evidence of wrist synovitis and lytic lesions made early subsequent surgery necessary in 10 patients. In the remaining 38 patients the post-operative course was followed for an average period of 33 months (range 8-82). Severe giant-cell silicone synovitis combined with isolated or disseminated osteolytic lesions were found in 17/30 (56%) patients

INTRODUCTION: Osteoarthritis of the trapeziometacarpal joint, also known as the thumb carpometacarpal joint, is one of the most common anatomic sites of arthritis in the human body. Many surgical techniques to address this problem have been developed; including, trapeziectomy with or without ligament reconstruction tendon interposition, implant arthroplasty, and arthrodesis. No methods have yet proven superior to the others, and each has associated limitations and complications. The primary complication found in the literature after arthrodesis of the trapeziometacarpal joint is nonunion; with
