Trends in Youth Mental Health Among Patients in Family Medicine Practices: 2017-2023.

OBJECTIVE: The burden of poor mental health among young people in the United States was rising before the onset of COVID-19. Prior research documents short-term increases in anxiety and depression in the wake of the pandemic, especially among older females. We study how the pandemic altered pre-existing trends in adolescent mental health and examine differential effects across demographic groups and time. METHODS: We used electronic health record data from a national primary care practice registry to identify primary care visits with diagnoses for anxiety, affective mood disorders, attention-

Can Mindfulness-Based Interventions Be Used to Treat PTSD-derived GI Disruptions in Black Women? A Review of Current Literature.

BACKGROUND: This review explores unique racial and trauma-related constructs associated with gastrointestinal conditions (GCs) in Black women, offering culturally relevant contemplative solutions for healing and holistic well-being. Additionally, we investigate the potential of mindfulness-based interventions (MBIs) in mitigating stress-induced GCs within this demographic. Guided by two theoretical frameworks, the Superwoman Schema (SWS) and critical feminist theory, the study unravels how cumulative stress, or allostatic load, intersects with socio-contextual factors, contributing to debilita

Longitudinal assessment of anger trajectories following childhood trauma and combat exposure: Associations with PTSD and alcohol use disorder symptoms.

For veterans, early life stressors, such as childhood trauma and experience of combat-related events, are associated with reporting greater levels of anger. Anger, if maintained over time, can have detrimental effects on mental health such as posttraumatic stress disorder (PTSD) and alcohol use disorder (AUD). Some modifiable factors, such as self-compassion and posttraumatic growth have been shown to mitigate long-term symptomology. Veterans (n = 1,230) completed online surveys at baseline and 9-, 12-, 18-, and 24-month follow-up. We used a latent transition model to understand how emergent c

Treating complicated grief.

IMPORTANCE: The death of a loved one is one of life's greatest, universal stressors to which most bereaved individuals successfully adapt without clinical intervention. For a minority of bereaved individuals, grief is complicated by superimposed problems and healing does not occur. The resulting syndrome of complicated grief causes substantial distress and functional impairment even years after a loss, yet knowing when and how to intervene can be a challenge. OBJECTIVE: To discuss the differential diagnosis, risk factors for and management of complicated grief based on available evidence and

Effects of juvenile exposure to predator odor on adolescent and adult anxiety and pain nociception.

Clinical researchers have tracked patients with early life trauma and noted generalized anxiety disorder, unipolar depression, and risk-taking behaviors developing in late adolescence and into early adulthood. Animal models provide an opportunity to investigate the neural and developmental processes that underlie the relationship between early stress and later abnormal behavior. The present model used repeated exposure to 2,3,5-trimethyl-3-thiazoline (TMT), a component of fox feces, as an unconditioned fear-eliciting stimulus in order to induce stress in juvenile rats aged postnatal day (PND)

Comparing Rates Mental Health Diagnosis in Adolescents Evaluated at a Community Clinic versus Detention-based Clinic: Is Traumatic Stress Still Most Salient?

BACKGROUND: Adolescents involved in the juvenile legal system (JLS) have higher rates of mental health treatment needs compared to their non-justice-involved peers, and they experience disproportionate rates of trauma exposure and trauma-related psychosocial concerns. Most research comparing these adolescent groups draws data from separate studies, making it more challenging to understand meaningful differences between the two groups. Research documenting making such comparisons can guide prevention and intervention strategies for communities and their juvenile detention centers. The current s

Effectiveness of evidence-based psychotherapy for posttraumatic distress within a jail diversion program.

Adults with severe mental illness (SMI) who come into contact with the criminal justice system experience high rates of trauma exposure and posttraumatic stress disorder (PTSD). For individuals with SMI, symptoms of PTSD are associated with exacerbated psychiatric symptoms, higher risk of homelessness and-for those with a history of arrest-increased risk of recidivism. The goal of the present study was to assess the effectiveness of evidence-based psychotherapies (EBPs) for trauma-related distress in treating individuals with SMI who have been diverted from jail to receive community-based trea

Association between ambient temperature and emergency psychiatric consultations: A case-crossover study in a South American emergency setting (2021-2023).

BACKGROUND: Although environmental factors have been studied for their impact on mental health, the link between ambient temperature and mental disorders remains insufficiently explored. AIM: This study aimed to explore the relationship between ambient temperature and the frequency of emergency psychiatric consultations at the sole university hospital in Paraguay from 2021 to 2023. METHODS: This investigation adopted a quantitative, observational, cross-sectional design, using a case-crossover approach. Data collection entailed reviewing patient records and meteorological data, focusing on t

Autism and attachment disorder symptoms in the general population: Prevalence, overlap, and burden.

BACKGROUND: Co-occurring trauma-related and neurodevelopmental problems are common in maltreated children. In population research and clinical practice, these tend to be considered separately. Overlapping health problems, that is, "multi-morbidity," in adulthood is associated with increased service burden and costs, but this has not been investigated in childhood. METHODS: Using well-validated parent-report questionnaires, we examined the overlap between symptoms of the neurodevelopmental disorder autism (autism spectrum disorder, ASD) and symptoms of the trauma- and stressor-related disorder

Editorial: Improving Evidence-Based Mental Health Care for Refugee and Internally Displaced Children: What Works and Why?

There has been a rapid increase in the number of displaced children over the past decades due to armed conflicts, violence, and natural disasters. In 2024 alone, the United Nations Children's Fund (UNICEF) estimated that 53.2 million children were forced to flee their homes, which is about triple the estimate of 2010.1 These children experience a multitude of stressors both before and during their displacement, including poor living conditions and serious risk of emotional, sexual, and physical abuse. Unsurprisingly, sustained exposure to such stressors places these children at heightened risk

Psychiatric complications of family reunion after four decades of separation.

OBJECTIVE: After four decades of separation due to civil war, many people who had migrated to Taiwan from mainland China had the unexpected opportunity to briefly visit their families in mainland China. A study was conducted to examine psychiatric complications associated with these family reunions following long involuntary separation. METHOD: Eighty subjects who made the journey were given semistructured interviews investigating their psychological experiences relating to the family reunion. The variables investigated were sociodemographic characteristics, the subject's previsit emotional c

Psychological predictors of negative treatment outcome with Erenumab in chronic migraine: data from an open label long-term prospective study.

BACKGROUND: Monoclonal antibodies (mABs) targeting the calcitonin gene-related peptide (CGRP) pathway represent the first disease-specific preventive migraine therapy. Growing evidence suggests that they are effective in the preventive treatment of difficult-to-treat patients. In this study, we evaluated the psychological predictors of the outcome of treatment with the anti-CGRP monoclonal antibody erenumab in patients with chronic migraine (CM). METHODS: Seventy-five patients with CM who had already failed at least 3 preventive therapies received erenumab every 28 days for a period of 12 mon

Recovery from post-traumatic stress disorder after a flood in China: a 13-year follow-up and its prediction by degree of collective action.

BACKGROUND: Victims exposed to serious traumatic experiences may develop post-traumatic stress disorder (PTSD) and suffer this mental health problem for a long time. Different types of trauma displayed a chronicity rate of PTSD within the range of 6.3-68.9 %. As one of the most common and severe natural disasters, the natural progression of flood related PTSD has not been revealed. The aim of this study was to estimate the chronicity rate and identify the prognostic factors of PTSD in flood victims. METHODS: Flood victims, who were over the age of 16 and diagnosed with PTSD in 2000 in Huarong

Chronic psychological and functional sequelae after emergent hand surgery.

PURPOSE: Several studies have shown that upper extremity trauma has serious, acute psychological effects after injury. This study's goal was to assess the psychological outcomes, including symptoms of major depression, posttraumatic stress disorder (PTSD), and other psychosocial variables, as well as the Quick Disabilities of the Arm, Shoulder, and Hand (QuickDASH) results, after severe hand trauma. We hypothesized that hand trauma would have persistent psychological sequelae long after the physical injury. METHODS: We performed a cross-sectional survey of 34 patients who had emergency hand s

Identification of DNA Methylation Changes That Predict Onset of Post-traumatic Stress Disorder and Depression Following Physical Trauma.

Post-traumatic stress disorder (PTSD) and major depressive disorder (MDD) are commonly experienced after exposure to highly stressful events, including physical trauma, yet, biological predictors remain elusive. Methylation of DNA may provide key insights, as it likely is reflective of factors that may increase the risk in trauma patients, as DNA methylation is altered by previous stressors. Here, we compared DNA methylation patterns using bisulfite sequencing in patients with a physical trauma that required more than a 24-h hospitalization (n = 33). We then compared DNA methylation in patient

Exploring Diagnostic Classification Prevalence in German and Dutch Child and Adolescent Psychiatry: An International Cross-Sectional Comparative Study.

Classification systems like the DSM-5 and ICD-10 facilitate international comparative research on mental disorders. However, few studies have compared classification distributions in child and adolescent psychiatric settings across countries. This study explored similarities and differences in classification prevalence between German and Dutch children and adolescents referred to psychiatric facilities. Data were retrospectively collected from clinical samples of inpatients and outpatients who underwent diagnostic assessments in Germany (n = 7,089) and the Netherlands (n = 2,574), aged 0-18 ye

A Psychiatry Service for Parents of Children With Medical Complexity.

BACKGROUND AND OBJECTIVES: Children with medical complexity are caregiver dependent because of complex, chronic health conditions; functional limitations; technology dependence; and high health care use. Their parental caregivers have suboptimal mental health, yet little is known about the psychiatric profiles of this group. We aimed to describe the clinical and demographic characteristics of parents of children with medical complexity seen by an integrated psychiatry service. METHODS: This is a descriptive case series using retrospective medical record review of electronic medical records fr

Are youths with disruptive mood dysregulation disorder different from youths with major depressive disorder or persistent depressive disorder?

BACKGROUND: Although the disruptive mood dysregulation disorder (DMDD) was included in the depressive disorders (DD) section of the DSM-5, common and distinctive features between DMDD and the pre-existing DD (i.e., major depressive disorder, MDD, and persistent depressive disorder, PDD) received little scrutiny. METHODS: Youths consecutively assessed as outpatients at two Canadian mood clinics over four years were included in the study (n = 163; mean age:13.4 ± 0.3; range:7-17). After controlling for inter-rater agreement, data were extracted from medical charts, using previously validated ch

Prior differences in previous trauma exposure primarily drive the observed racial/ethnic differences in posttrauma depression and anxiety following a recent trauma.

BACKGROUND: Racial and ethnic groups in the USA differ in the prevalence of posttraumatic stress disorder (PTSD). Recent research however has not observed consistent racial/ethnic differences in posttraumatic stress in the early aftermath of trauma, suggesting that such differences in chronic PTSD rates may be related to differences in recovery over time. METHODS: As part of the multisite, longitudinal AURORA study, we investigated racial/ethnic differences in PTSD and related outcomes within 3 months after trauma. Participants (n = 930) were recruited from emergency departments across the US

Finalizing PTSD in DSM-5: getting here from there and where to go next.

The process that resulted in the diagnostic criteria for posttraumatic stress disorder (PTSD) in the Diagnostic and Statistical Manual of Mental Disorders (5th ed.; DSM-5; American Psychiatric Association; ) was empirically based and rigorous. There was a high threshold for any changes in any DSM-IV diagnostic criterion. The process is described in this article. The rationale is presented that led to the creation of the new chapter, "Trauma- and Stressor-Related Disorders," within the DSM-5 metastructure. Specific issues discussed about the DSM-5 PTSD criteria themselves include a broad versus
