School phobia.

Pediatricians and other primary care providers are in an ideal position to prevent, to diagnose and to treat children with school refusal. Detection requires recognition of high risk situations, and delineation of possible reality factors or environmental hazards. The physician will find school refusal associated with perceived or actual physical illness, debility, or vulnerability, family stresses including illnesses and marital problems, over-protective mother-child interaction patterns, and previous difficulties in achieving mother-child independence and separation. Confirming the diagnosis

A Rare Case of Sacral Decubitus Injury Causing Methicillin-Resistant Staphylococcus aureus (MRSA) Bacteremia With a Spinal Abscess.

Sacral ulcers can be precipitated by poor circulation, damaged skin and tissue from immobility or direct trauma, poor nutrition, diabetes, and incontinence. If not treated promptly, the sacral ulcer can serve as a portal of entry for bacteria, such as methicillin-resistant Staphylococcus aureus (MRSA), into the bloodstream, leading to complications including bacteremia, abscesses, sepsis, and possible infectious endocarditis, as occurred in this case. During the patient's hospital stay, he was diagnosed with possible schizoaffective disorder with full mental capacity, but had multiple episodes

Implementing the living will and other advance directives: guidelines for health care providers.

Living wills and other advance directives for the refusal of medical treatment by patients no longer able to make their own medical decisions have gained increasing acceptance in the last decade. In addition to constitutional and common law rights to refuse treatment, living will statutes have been enacted in many states to permit advance directives under specified circumstances. This article explores both statutory and non-statutory advance directives and offers guidelines for hospitals and medical staffs when decisions regarding life-sustaining treatment must be made for adults with terminal

[Novel resources utilization technique for rural domestic refuse].

In order to speed up rural domestic refuse resources utilization, intermittent aeration and continuous aeration were applied to treat rural domestic refuse after anaerobic fermentation. Three kinds of refuse were selected on base of fermentative age, i.e. three months, five months and seven months. Results showed that aeration could remove water and organic materials of the refuse effectively. Points of view on aeration, continuous aeration was better than intermittent aeration, and on the other side, water removal rate increased with ventilation and decreased with fermentative age in the cond

Treating against a stated refusal: The ethics of substitute decision-making when a patient cannot capably refuse.

Decision-making capacity in health care refers to a person's ability to understand information relevant to a decision, appreciate how the decision affects them, make a choice, and communicate that choice. Capacity is essential in ethics and professional nursing practice because it forms the basis of informed consent. Nurses encounter various medical conditions, such as dementia, that can contribute to changes in cognitive abilities and result in diminished capacity. However, having a cognitive condition such as dementia does not necessarily mean a person lacks the capacity to make a specific d

Revive and Refuse: Capacity, Autonomy, and Refusal of Care After Opioid Overdose.

Physicians generally recommend that patients resuscitated with naloxone after opioid overdose stay in the emergency department for a period of observation in order to prevent harm from delayed sequelae of opioid toxicity. Patients frequently refuse this period of observation despiteenefit to risk. Healthcare providers are thus confronted with the challenge of how best to protect the patient's interests while also respecting autonomy, including assessing whether the patient is making an autonomous choice to refuse care. Previous studies have shown that physicians have widely divergent approache

Frequency and impact of nonadherence to immunosuppressants after renal transplantation: a systematic review.

Nonadherence to immunosuppressants is recognized to occur after renal transplantation, but the size of its impact on transplant survival is not known. A systematic literature search identified 325 studies (in 324 articles) published from 1980 to 2001 reporting the frequency and impact of nonadherence in adult renal transplant recipients. Thirty-six studies meeting the inclusion criteria for further review were grouped into cross-sectional and cohort studies and case series. Meta-analysis was used to estimate the size of the impact of nonadherence on graft failure. Only two studies measured adh

Autoamputation of the foot caused by untreated gas gangrene: a case report.

Gas-producing diabetic foot infections are limb-threatening emergencies commonly encountered by foot and ankle surgeons. Appropriate treatment includes aggressive surgical debridement, parenteral antibiotics, and postoperative wound care. The authors present a unique case of a patient who refused treatment for a deep-space gas-producing infection, resulting in autoamputation of the foot. The authors also discuss the confusion surrounding the definition of gas gangrene.

A non-compliant patient is defined as one who has taken less than 80% of prescribed antituberculosis medication. Spontaneous auto-amputation of the foot in a case of diabetes, atherosclerosis and gangrene.

Auto-amputation is the spontaneous separation of non-viable tissue from viable tissue, and is usually associated with dry gangrene, occurring in the distal portions of the lower extremities. A 68-year-old female presented with dry gangrene of both feet and subsequent loss of the right foot and digits on the left foot over a period of one and half years. After about six months disarticulation of the right foot from the ankle joint resulted. She gradually lost the digits of the left foot two months later. She had pallor and showed absence of popliteal and distal pulses on both sides. The left fo
