Deep brain stimulation in craniofacial pain: seven years' experience.

Cluster headache (CH) is a primary headache with excruciatingly painful attacks that are strictly unilateral. About 10% of cases experience no significant remission, and about 15% of these do not respond to medication, so surgery is considered. Neuroimaging studies show that the posterior inferior hypothalamus is activated during CH attacks and is plausibly the CH generator. We report on 16 chronic CH patients, with headaches refractory to all medication, who received long-term hypothalamic stimulation following electrode implant to the posterior inferior hypothalamus. After a mean follow-up o

Central connections of the nucleus mesencephalicus nervi trigemini in the mallard (Anas platyrhynchos L.).

BACKGROUND: In the mallard duck, functionally distinct groups of jaw muscles are each innervated by a different subnucleus of the main trigeminal (mV) or facial (mVII) motor nucleus. The other subnuclei of mV and mVII innervate several head muscles, including lingual muscles. The reticular premotor cells of the trigeminal and facial jaw motor subnuclei occupy different areas in the parvocellular reticular formation (RPc). The cell bodies of jaw muscle spindle afferents are situated in the mesencephalic nucleus (MesV). In the present study, the central connections of MesV with jaw motor subnucl

Immunocytochemical localization of the AMPA receptor subunits in the mesencephalic trigeminal nucleus of the rat.

The mesencephalic trigeminal nucleus is composed of large (35-50 microns) pseudo-unipolar neurons. Closely associated with them are small (< 20 microns) multipolar neurons. An unique peculiarity of the pseudo-unipolar perikarya is that they receive synaptic input from various sources, which sets them apart from the dorsal root and cranial nerves sensory ganglia neurons. Whereas glutamate is the best neurotransmitter candidate in pseudo-unipolar neurons, glutamatergic input into them has not yet been reported. AMPA glutamate receptors are implicated in fast excitatory glutamatergic synaptic tra

Morphological features of identified trigeminocerebellar projection neurons in the border zone of rat trigeminal nucleus oralis.

The retrograde transport of horseradish peroxidase (HRP) was used to assess the overall morphology of neurons in the dorsal half of the border zone (BZ) of rat trigeminal nucleus oralis (Vo) that project to ipsilateral orofacial portions of four major tactile areas (crura I and II, the paramedian lobule, and the uvula) of the cerebellar cortex. We wished to answer two important questions: Does this group of cells consist of one or more morphologically distinct types, each projecting to different cerebellar tactile areas? Does the overall morphology or morphologies of these BZ neurons resemble

Greater occipital nerve block modulates nociceptive signals within the trigeminocervical complex.

INTRODUCTION: The pharmacological block of the greater occipital nerve has been proven effective in numerous headache and facial pain syndromes. This clinical effect supports the hypothesis of a strong functional interaction between the occipital and trigeminal nerves which has been proposed in neurophysiological in vivo experiments in rodents. Although it is likely that the interaction has to occur in the central nervous system, the exact site and the mechanisms of the interaction remain largely unknown. METHODS: Focusing on these questions we investigated in a double-blind, placebo-controll

Comatose state maintained during eight years following a vascular ponto-mesencephalic lesion.

A 24-year-old man survived for 8 years after a vascular lesion of the pons and midbrain. During these years a subresponsive comatose state with neurological signs extending from the oculomotor nuclei to the trigeminal and facial nuclei levels was present. A decorticated type of rigidity, with bilateral paralysis of the limbs accompanied by pyramidal signs, was also present. The EEG during the first months showed slow activity which afterwards gradually changed towards fast and alpha activities, maintained in the course of the years. The pathological diagnosis was a fibrous endarteritis, mainly

Effects of neonatal attenuation of axoplasmic flow or transection of the rat's infraorbital nerve on the morphology of individual trigeminal primary afferent terminals in the brainstem.

Attenuation of axoplasmic transport in the infraorbital nerve (ION), or transection of this trigeminal (V) branch at birth, results in degradation of the central cellular aggregates related to the mystacial vibrissae. However, blockade of axoplasmic transport does not result in the nearly 90% loss of ION ganglion cells that follows neonatal transection of this nerve. The present study was undertaken to further characterize the response of individual ION axons to attenuation of axoplasmic transport and to compare these effects to the changes observed following nerve transection. Neurobiotin inj

Neuroparalytic keratitis in Goldenhar-Gorlin syndrome.

A 2-year-old boy had bilateral corneal ulceration with the Goldenhar-Gorlin syndrome. Initially, the patient received subconjunctival and topical antibiotics with minimal effect. With subsequent treatment of atropine, sterile ophthalmic petrolatum ointment, and humidification of the patient's crib, ulcers in both eyes healed in two weeks without further problems. He had a neuroparalytic keratitis with corneal anesthesia and decreased tear production bilaterally. Our evidence suggested that aplasia or hypoplasia of the trigeminal nuclei was the probable cause.

Coexpression of VGLUT1 and VGLUT2 in trigeminothalamic projection neurons in the principal sensory trigeminal nucleus of the rat.

VGLUT1 and VGLUT2 have been reported to show complementary distributions in most brain regions and have been assumed to define distinct functional elements. In the present study, we first investigated the expression of VGLUT1 and VGLUT2 in the trigeminal sensory nuclear complex of the rat by dual-fluorescence in situ hybridization. Although VGLUT1 and/or VGLUT2 mRNA signals were detected in all the nuclei, colocalization was found only in the principal sensory trigeminal nucleus (Vp). About 64% of glutamatergic Vp neurons coexpressed VGLUT1 and VGLUT2, and the others expressed either VGLUT1 or

Immunohistochemical mapping of neurophysins and calcitonin gene-related peptide in the human brainstem and cervical spinal cord.

Our study investigates the distribution of neurophysins (Nph), proteins that are part of the precursors for vasopressin and oxytocin, and calcitonin gene-related peptide (CGRP) in the human brainstem by immunohistochemistry. Both peptides were found in discrete regions of the human hindbrain. Nph could be demonstrated exclusively in fibers and punctate perineural varicosities that were travelling within the mesencephalic central gray, substantia nigra, as well as locus coeruleus, medial longitudinal fascicle, raphe, nucleus of the solitary tract, lateral reticular nucleus and area postrema. Amyotrophic lateral sclerosis with temporal lobe atrophy.

Clinical and neuropathological findings are reported on a 48-year-old man who developed progressive bulbar palsy, muscle atrophy of four extremities, and hyperreflexia. Duration of the illness was about 19 months. Moderate personality changes were observed during his hospitalization. Neuropathological examination revealed the presence of an ALS: severe loss of the large motoneurons in the spinal anterior horns, and degeneration of the corticospinal tract, more prominent on the left side. The hypoglossal, the facial, and the motor trigeminal nuclei were also involved, but the oculomotor, the tr…

When light hurts: Comparative Morphometry of Human Brainstem in Traumatic Photalgia.

Traumatic brain injury is an increasingly common affliction, although many of its serious repercussions are still underappreciated. A frequent consequence is the development of light-induced pain, or 'photalgia', which can often lead to prolonged debilitation. The mechanism underlying the sensitivity to light, however, remains unresolved. Since tissue oedema (swelling) is a common feature of traumatic brain injury, we propose that the brainstem oedema, in particular, might sensitize the brainstem trigeminal complex to signals from ocular mechanisms activated in bright light. To assess this hyp

NADPH-diaphorase-positive neurons and pathways in the brain of the frog Rana esculenta.

We described the NADPH-diaphorase-containing neurons and fibres in the brain of the frog Rana esculenta. In the telencephalon stained cells occurred in the olfactory bulb, all subdivisions of the pallium, the diagonal band, the medial septum and the striatum. The olfactory glomeruli showed the most intense enzyme reaction. The neuropil of the accessory olfactory bulb was also heavily stained and this staining extended to the rostral diencephalon through the ventral lateral pallium. Fibre staining was less intense in the medial pallium and the medial septum. In the diencephalon, NADPH-diaphoras

[Influence of manual therapy of cervical spine on typical trigeminal neuralgia: a case report].

This article presents the case of a 43-year-old female patient with pain in the cervical spine area and a typical trigeminal neuralgia (TN; French name "tic douloureux") in the receptive field of the second and the third branches of the left trigeminal nerve. The patient came to our medical practice for a manual therapy of the cervical spine as the application of the standard therapy had not given her any pain reduction in the cervical spine area. As the result of the manual therapy of the cervical spine (nonspecific traction mobilization, specific or segmental mobilization, manipulation), not

Topography and organization of cranial nerve nuclei in the sand lizard, Lacerta agilis.

Cobaltic-lysine complex compound was used to label cranial nerves of the ventrolateral (branchiomotor) and dorsomedial (somatomotor) nuclear columns in the sand lizard, Lacerta agilis. The dendritic arborizations and axonal trajectories of neurons of the respective nuclei were reconstructed from serial sections. A fairly uniform neuronal morphology was found in the nuclei of the ventrolateral column: a spindle-shaped perikaryon gave rise to dorsomedial and ventrolateral dendritic trees, the latter arborizing in a characteristic broomlike manner within a narrow region in the lateral white matte

Pain. Part 2a: Trigeminal Anatomy Related to Pain.

In order to understand the underlying principles of orofacial pain it is important to understand the corresponding anatomy and mechanisms. Paper 1 of this series explains the central nervous and peripheral nervous systems relating to pain. The trigeminal nerve is the 'great protector' of the most important region of our body. It is the largest sensory nerve of the body and over half of the sensory cortex is responsive to any stimulation within this system. This nerve is the main sensory system of the branchial arches and underpins the protection of the brain, sight, smell, airway, hearing and
