[Lasik after corneal ulcer].

Our case report presents successful treatment of corneal ulcer caused by the most common pathogen of infectious keratitis--Pseudomonas aeruginosa. Treatment was followed by LASIK as a final refractive error solution. 20 year old patient (contact lens wearer) was admitted for acute infectious corneal ulcer in her left eye. Her visual acuity was 0.5 partial with full correction. Intensive local treatment with Tobramycin, Ofloxacin and Scopolamin was started. After healing of infectious focuses, local steroids (Fluorometholonin acetas) were used to clear residual stromal haze. Refractive error wa…

Arteriolosclerotic ulcer of Martorell.

In 1945, Martorell described ischaemic leg ulcers in patients with hypertension. He suggested that the ischaemic necrosis was secondary to a hypertensive arteriolar disease and referred to them as 'hypertensive ischaemic ulcers'. In recent years, the specific entity of these ulcers has been questioned. Others claim they have a much higher incidence, but presume the diagnosis is frequently missed. Almost 900 cases of Martorell's ulcers have been reported in literature since the first description. A systematic review and comprehensive search of literature (evidence-based) was needed to character…

Hypercalcemia complicating Marjolin's ulcer.

A 54-year-old male with chronic osteomyelitis presented with new-onset hypercalcemia. Extensive initial evaluation did not reveal the underlying cause of his hypercalcemia. Subsequent biopsies revealed squamous cell carcinoma transformation consistent with Marjolin's ulcer. Timely surgery had been shown to be effective in achieving remission in patients with Marjolin's ulcers, even in the presence of hypercalcemia. Therefore, we recommend that patients with chronic wounds and new-onset or otherwise unexplained hypercalcemia should be evaluated for development of Marjolin's ulcers.

Understanding non ulcer dyspepsia.

Non ulcer dyspepsia is one of the most common problems encountered in primary care practice. [Ulcer surgery - what remains?].

Ulcer surgery today concentrates on the complications of chronic ulcer disease, especially ulcer perforation and endoscopically uncontrollable ulcer bleeding. In this case the laparoscopic or open closure of the gastroduodenal defect or local hemostasis of the bleeding ulcer by laparotomy are the main aims of surgery. Elective operations due to recurrent gastric or duodenal ulcers have become rare. An indication for gastric ulcer resistant to conservative therapy could be persisting suspicion of malignancy whereas in duodenal ulcer gastric outlet obstruction represents a reason for surgery. If…
