The use of arteriovenous fistula for cancer chemotherapy administration in hemodialysis patients.

Vascular access preservation is fundamental in hemodialysis patients to ensure dialysis adequacy, patient survival, and quality of life. Chemotherapy administration typically requires central venous access devices, which are associated with significant complications and potential compromise of future vascular access options. We describe two chronic hemodialysis-dependent patients in whom intravenous chemotherapy was administered via a pre-existing mature autogenous arteriovenous fistula (AVF). In both cases, chemotherapy was delivered through AVF puncture using a peripheral catheter on non-dia

Safety and Efficacy of Transbrachial Access for Endovascular Procedures: A Single-Center Retrospective Analysis.

BACKGROUND: Endovascular procedures nowadays are generally performed via the femoral and radial artery. Although not routinely used by many, there is still an essential role for vascular interventions via brachial access. The technological advancement of endovascular devices leads to a significant increase of the variety of percutaneously treatable vascular pathology. The brachial artery approach nowadays is becoming crucial for complex procedures either as single access or in the context of mandatory double vascular approach for many complex interventions. OBJECTIVE: To evaluate the safety a

Substantial harm associated with failure of chronic paediatric central venous access devices.

Central venous access devices (CVADs) form an important component of modern paediatric healthcare, especially for children with chronic health conditions such as cancer or gastrointestinal disorders. However device failure and complications rates are high.Over 2½ years, a child requiring parenteral nutrition and associated vascular access dependency due to 'short gut syndrome' (intestinal failure secondary to gastroschisis and resultant significant bowel resection) had ten CVADs inserted, with ninesubsequently failing. This resulted in multiple anaesthetics, invasive procedures, injuries, vasc

A retrospective, comparative, clinical study of occlusion rate of peripherally inserted central catheters fabricated of poly(vinyl alcohol)-based hydrogel composite.

Thrombotic accumulation is associated with surface interactions between blood proteins and vascular access devices. Catheter occlusion results from this process, and is a costly, common, occurrence with peripherally inserted central catheters (PICCs). Hydrophilic catheter materials exhibit antithrombotic properties. This retrospective study evaluates the occurrence of catheter occlusion of PICCs constructed of a poly(vinyl alcohol)-based hydrogel composite known as hydrophilic biomaterial (HBM), compared to thermoplastic polyurethane (TPU) control devices. A total of 121 PICCs, 60 HBM and 61 T…

An innovative technology for safe needle disconnection after hemodialysis.

PURPOSE: Vascular access care is a key topic for hemodialysis patients. The most cost-effective and lasting vascular access for chronic hemodialysis is the native arteriovenous fistula (AVF); however, bleeding after dialysis session from puncture site is a relevant problem. Achieving hemostasis is necessary and requires hand compression by the nurse or the patient if he or she is capable and cooperative. METHODS: We assessed a new vascular closure device, VITACLIP® (Serumwerk Bernburg Vertriebs GmbH) that is an adhesive silicone seal device, which can be set onto the skin and punctured by dia

Totally Percutaneous Transfemoral Transcatheter Aortic Valve Replacement Despite Failure to Deploy a Vascular Closure Device: A Single-Centre Case Series.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) is preferably performed as a completely percutaneous procedure via transfemoral access. Suture-mediated vascular closure devices are deployed prior to sheath insertion (pre-closure). Inability to perform pre-closure may necessitate surgical vascular repair of the femoral artery. Patients at increased risk of vascular surgery complications may benefit from a percutaneous method for achieving access site haemostasis. Stent graft implantation is commonly used for treating access site injury following TAVR. This study assessed the feasibili

Routine use of percutaneous femoral cannulation in minimally invasive cardiac surgery.

OBJECTIVES: Large series of percutaneous femoral access for extracorporeal circulation in minimally invasive cardiac surgery (MICS) are scarcely reported. METHODS: This is a single-centre study describing the use of percutaneous femoral access in patients undergoing MICS via minithoracotomy. Femoral artery closure was performed with a plug-based closure device. To reduce the risk for vascular complications, intraoperative ultrasound assessment of correct deployment of the arterial closure device was done during the later period of the study. RESULTS: During a 5-year period, 650 patients unde

Brachial artery access for endovascular procedures: how safe is it?

PURPOSE: While femoral and radial artery access are routine for endovascular procedures, brachial artery access remains an important alternative in selected cases. This study aimed to evaluate the complication rate of percutaneous brachial artery access at a tertiary centre and identify potential risk factors for access-site complications. MATERIALS AND METHODS: A retrospective review was performed of all procedures utilising percutaneous brachial artery access over a 12.5-year period. Patient demographics, sheath size, closure technique and complications were recorded. Complications were cla

Vascular complications of intravenous vancomycin: Safety of peripheral, midline and central catheters. A scoping review.

INTRODUCTION: Vancomycin treats severe infections by resistant bacteria, but its acidity and local toxicity demand careful intravenous administration. Central venous catheters reduce these risks but pose complications. Midline catheters offer a less invasive alternative, though evidence remains limited. OBJECTIVES: To map and describe the available clinical evidence on the safety and vascular complications associated with peripheral intravenous administration of vancomycin, compared with other vascular access devices, focusing on the type of complications reported, their diagnostic methods, a…

I-DECIDED®-a decision tool for assessment and management of invasive devices in the hospital setting.

Indwelling medical devices, including vascular access and urinary catheters, pose a risk for infection, and therefore daily assessment and consideration of their continued need is a patient safety priority. The I-DECIDED® device assessment and decision tool is an evidence-based checklist, designed to improve the assessment, care and timely removal of invasive devices in acute hospitalized patients. This paper explains each step of the tool, with rationale for inclusion.

Effectiveness of Sternal Intraosseous Device in Patients Presenting with Circulatory Shock: A Retrospective Observational Study.

BACKGROUND: Hemorrhagic shock requires timely administration of blood products and resuscitative adjuncts through multiple access sites. Intraosseous (IO) devices offer an alternative to intravenous (IV) access as recommended by the massive hemorrhage, A-airway, R-respiratory, C-circulation, and H-hypothermia (MARCH) algorithm of Tactical Combat Casualty Care (TCCC). However, venous injuries proximal to the site of IO access may complicate resuscitative attempts. Sternal IO access represents an alternative pioneered by military personnel. However, its effectiveness in patients with shock is su

Cutting balloon for femoral arterial and venus obstructions due to suture-based closure devices: Case series.

Suture-based vascular closure devices have been shown to be effective in hemostasis for procedures with vascular access. However, iatrogenic vascular occlusion may occur. The cutting balloon (CB) is a noncompliant balloon wrapped with 3-4 microsurgical blades that are intended to modify vascular lesions, but it may also be utilized to cut and release endovascular sutures. We report two cases in which the CB was employed as a bailout strategy to alleviate suture-related vascular occlusion after transcatheter aortic valve replacement. The CB can be effectively utilized to resolve suture-related

Hemodialysis vascular access flow surveillance: Current evidence and future directions.

A well-functioning vascular access is essential for delivering adequate hemodialysis in patients with end-stage renal disease. However, vascular access dysfunction, particularly stenosis and thrombosis, remains a leading cause of morbidity, repeated interventions, and hospitalization in this patient population. Vascular access monitoring and surveillance are designed to detect hemodynamically significant stenosis early, thereby reducing the risk of thrombosis and maintaining access patency. Evidence from meta-analyses and randomized controlled trials suggests that access blood flow (Qa)-based

Infraumbilical surgical retrieval of transected umbilical artery catheter in an 840 g newborn.

An 840 g female baby born at 25 weeks gestation suffered a rare complication of umbilical arterial catheter (UAC) insertion, involving an accidental transection resulting in 11 cm of retained catheter inside the baby. Investigations revealed migration of the proximal tip of the catheter within 10 hours into the left subclavian artery. This complication is rare and has not frequently been described in the literature. Given the size and gestation of the patient, the risks and benefits of both conservative and non-conservative management were discussed in detail prior to any treatment decision bei
