We report a 3-year-old girl with left ventricular outflow tract obstruction after repair of ostium primum defect. The girl had closure of an ostium primum defect performed at the age of 1 year. Two years later, she developed left ventricular hypertrophy with systolic ejection murmur. Echocardiography showed discrete stenosis of the left ventricular outflow tract. Cardiac catheterization showed a peak systolic gradient of 63 mmHg across the left ventricular outflow tract. At the reoperation, the fibrous tissue was excised and myectomy was done. After the operation the peak systolic gradient acr

Obstruction to pulmonary blood flow as a result of neoplasia in the right ventricular outflow tract is described in two dogs. Whereas one dog had exertional syncope and a systolic ejection murmur, the other had signs of congestive failure and hypoxia. In both animals the mass was detected in the right ventricle with two-dimensional echocardiography and confirmed angiographically. Although rare, primary right ventricular neoplasia represents a potentially treatable form of cardiac disease and should be considered as a cause of acquired outflow tract obstruction.

Left ventricular outflow tract obstruction (LVOTO) after mitral valve repair by Carpentier's technique has been recently reported in the literature. To assess the mechanisms of this phenomenon, we investigated 307 mitral valve repairs performed between July 1985 and December 1986. Incidence of LVOTO related to the mechanism of the mitral insufficiency and to the etiology demonstrates a direct relation to preoperative mitral valve prolapse (posterior leaflet +/- anterior leaflet) of degenerative origin. No LVOTO occurred after rheumatic mitral insufficiency repair regardless of size of the left

A case is reported of right ventricular outflow obstruction caused by hypertrophic cardiomyopathy. The pressure gradient between the inflow tract and the outflow tract of the right ventricle was 84 mm Hg. Resection of the hypertrophic muscle bundles practically eliminated the obstruction leaving only an insignificant subvalvular gradient of 2 mm Hg. Histologic examination revealed cardiac muscle cell hypertrophy and disorganization.

Arteriovenous fistulas (AVFs) are the preferred vascular access for hemodialysis due to their durability and lower risk of complications than alternative access methods. However, AVFs can significantly impact cardiac function, particularly in patients with preexisting cardiovascular conditions. We present a case of a 56-year-old female with a history of hypertension and end-stage renal disease who developed recurrent hypotension, dizziness, and dyspnea following AVF reopening. The echocardiographic evaluation revealed severe left ventricular hypertrophy, systolic anterior motion of the mitral

AIMS: Many factors cause left ventricular outflow tract obstruction (LVOTO) in hypertrophic cardiomyopathy (HCM). Previous studies reported that left ventricular basal muscle bundle (BMB) may be associated with LVOTO. We aimed to evaluate the role of BMB in LVOTO by echocardiography.
METHODS AND RESULTS: Two hundred fifty-six patients diagnosed with HCM were recruited. The morphologic characteristics of left ventricular outflow tract (LVOT) were analysed. BMB was detected in 178 (69.5%) patients by echocardiography. Patients were separated by a resting or provocative LVOT gradient ≥30 mmHg or

Hypertrophic cardiomyopathy (HC) has historically been characterized as a disease of substantial left ventricular hypertrophy, often associated with dynamic left ventricular outflow tract obstruction. However, we have recently encountered patients with subaortic obstruction and only minimal basal septal thickness, raising important management implications. Thereby, we sought to characterize the natural history and treatment strategies for this under-recognized subgroup of HC patients with dynamic obstruction. Of 1,591 consecutive patients with a HC diagnosis, 113 (7%) were identified with outf

Sinus of Valsalva aneurysms are rare cardiac anomalies that may be acquired or congenital. We describe the case of a patient who had an unruptured sinus of Valsalva aneurysm causing right ventricular outflow obstruction, with an associated subaortic ventricular septal defect. Sinus of Valsalva aneurysms rarely present until rupture occurs. However, the unruptured aneurysm of the right sinus of Valsalva caused significant right ventricular outflow tract obstruction, resulting in exertional dyspnea. Intraoperative multiplane transesophageal echocardiography and color Doppler helped in precise id…

On the basis of experience with older children, creation of a double-outlet ventricle by interposition of a valved conduit between the apex of the left ventricle and the thoracic aorta appeared to be an option for treating life-threatening left ventricular outflow tract obstruction even in the small heart of the infant. Over the recent 3 year period, nine infants ranging in age from 3 days to 13 months with various forms of left ventricular outflow tract obstruction underwent placement of an apical-aortic conduit to decompress the hypertensive left ventricle. There were two early and two late

BACKGROUND: Sinus of Valsalva aneurysm (SOVA) is an infrequent condition commonly affecting the right coronary sinus. It is typically asymptomatic unless it compresses or ruptures into nearby structures.
CASE SUMMARY: A 52-year-old man presented to the emergency department with clinical features of heart failure. Transthoracic echocardiography revealed a large SOVA with associated aortic regurgitation (AR), right ventricular outflow tract obstruction (RVOTo), and biventricular dysfunction. Given concern for impending rupture and hemodynamic compromise, the patient was transferred to a tertiar

BACKGROUND: Left bundle branch area pacing (LBBAP) is a physiological pacing strategy considered safe in non-obstructive hypertrophic cardiomyopathy (HCM). Its effect on left ventricular outflow tract (LVOT) obstruction in anatomically predisposed patients remains unclear.
CASE SUMMARY: A 76-year-old woman underwent LBBAP for atrioventricular nodal dysfunction. One week later, she developed chest pain and elevated troponin, despite unobstructed coronaries. Echocardiography revealed a resting LVOT gradient of 112.6 mmHg. Bipolar low-voltage pacing worsened the gradient, while unipolar high-vol

Liver transplant involves major hemodynamic changes due to hypovolemia, altered vascular resistance, and postreperfusion syndrome. Dynamic left ventricular outflow tract obstruction may occur during surgery and cause cardiovascular compromise. Early recognition is crucial to prevent adverse outcomes. A 55-year-old female patient with alcoholic liver disease underwent living donor liver transplant. Preoperative echo-cardiography showed a hyperdynamic left ventricle with preserved systolic function. During surgery, the patient developed hypotension, tachycardia, and desaturation. Transesophageal

BACKGROUND: Transcatheter mitral valve replacement can carry a left ventricular outflow tract (LVOT) obstruction in patients with small neo-LVOT.
CASE SUMMARY: A 74-year-old woman with a degenerated 27-mm surgical mitral bioprosthesis presented with severe symptomatic mitral stenosis and biventricular failure. She was deemed high risk for redo surgery. After heart team discussion, she underwent extracorporeal membrane oxygenation-supported valve-in-valve transcatheter mitral valve replacement with balloon-assisted translocation of the anterior mitral leaflet modification and intraleaflet depl

UNLABELLED: Transcatheter pulmonary valve implantation (TPVI) has emerged as a less invasive alternative to surgical conduit replacement in patients with recurrent right ventricular outflow tract (RVOT) obstruction, particularly after repair of tetralogy of Fallot (TOF). We report a case of successful TPVI using a balloon-expandable MyVal transcatheter heart valve in a 58-year-old male with a complex cardiac history, including multiple prior surgeries and interventions for TOF and RVOT re-stenosis. The patient presented with severe symptoms, and multimodality imaging revealed severe right vent

OBJECTIVE: The mechanism of left ventricular outflow tract obstruction (LVOTO) is complex in hypertrophic cardiomyopathy (HCM). We aimed to evaluate the impact of mitral valve geometry on LVOTO by echocardiography.
MATERIALS AND METHODS: The study population comprised 177 consecutive patients with HCM. Morphological findings of left ventricular hypertrophy and LVOTO-related abnormalities were assessed by comprehensive transthoracic echocardiography. Aortomitral angle, mitral leaflet length, and coaptation height were measured and analyzed at rest. Multivariable stepwise forward logistic regre

BACKGROUND: Patients with Takotsubo syndrome (TTS) who develop cardiogenic shock may present with left ventricular outflow tract obstruction (LVOTO). The prognosis and treatment of this population have not been defined in previous studies. The aim of this study is to describe the clinical presentation, management, evolution and prognosis of a subgroup of patients with TTS and cardiogenic shock according to whether they present with LVOTO or not.
METHODS: We analysed patients with TTS recruited from 2003 to 2022 in a multicentre registry. Patients were selected if they presented cardiogenic sh

Hypertrophic cardiomyopathy-both obstructive hypertrophic cardiomyopathy (oHCM) and nonobstructive hypertrophic cardiomyopathy (nHCM) subtypes-is the most common monogenic cardiomyopathy. Its structural hallmarks are abnormal thickening of the myocardium and hyperdynamic contractility, while its hemodynamic consequences are left ventricular outflow tract or intracavitary obstruction (in oHCM) and diastolic dysfunction (in both oHCM and nHCM). Several medical therapies are routinely used to improve these abnormalities with the goal to decrease symptom burden in patients with HCM. Current guidel
