Neuroradiological features of vertigo.

The diagnostic pathway in a patient with vertigo starts with the accurate evaluation of medical history followed by a general physical and neurological examination. This step can often lead to the identification of the correct cause of the disease or, at least, to a distinction between peripheral and central vertigo. Neuroradiological investigations have to be considered as elective diagnostic procedures and include: computed tomography (CT), magnetic resonance (MR), MR angiography (MRA), angiography. For the diagnosis of peripheral vertigo, benign paroxysmal positional vertigo, labyrinthitis,…

[Drug treatment for vertigo].

The approach to drug treatment of vertigo is almost exclusively symptomatic. There are 3 major goals for drug treatment of vertigo: to eliminate the hallucination of motion, to reduce the accompanying neurovegetative and psychoaffective signs (nausea, vomiting, anxiety), and to enhance the process of "vestibular compensation" to allow the brain to find a new sensory equilibrium in spite of the vestibular lesion. Three different types of vertigo drug treatment must be distinguished: the treatment of acute vertigo attacks, the treatment of chronic vertigo, and the treatment of patients with Meni…

[Cardiovascular aspects of vertigo].

True vertigo described as a sensation of body or surrounding objects rotation is seldom of cardiovascular origin. The main causes are caused by ear pathology, neurologic pathology or are of toxic origin. Patients talk frequently of vertigo to describe symptoms in relation with a fall of the brain blood output. These dizziness feelings are frequently of cardiovascular origin and may be associated with syncope which represents a more important fall in the brain blood pressure. Medical history, physical examination, electrocardiogram represents the cornerstones of the cardiological workout allowi…

[Vertigo as a symptom of migraine].

EPIDEMIOLOGY: Since both migraine and vertigo are common complaints in clinical practice they may coincide in an individual patient just by chance. There are, however, numerous patients with vestibular symptoms caused by migraine, accounting for 6-8% of diagnoses in specialized dizziness clinics. CLINICAL MANIFESTATION: Migraine-associated vertigo is a vestibular disorder which manifests itself with spontaneous or positional rotational vertigo or dizziness induced by head motion. The vertigo may occur without accompanying headache and may last from seconds to several weeks. DIAGNOSIS: Migrain…

The effectiveness of piracetam in vertigo.

Vertigo is a sensation of altered orientation in space and may be defined as an illusion of movement. It is a subjective symptom and therefore difficult to assess. Examination and diagnosis remain difficult. Although treatment should be directed at the underlying cause or disorder, the origin of vertigo is frequently unknown or untreatable. Pharmacotherapy is required for symptomatic treatment. Piracetam has been shown to be effective in vertigo of both central and peripheral origin. It is thought to act on vestibular and oculomotor nuclei in the brain stem and thus on the central control of b…

The visual vertigo syndrome.

Neuro-otological and posturography findings in 15 patients with visually induced vertiginous symptoms (visual vertigo, VV) are reported. Most patients were considered to have peripheral vestibular disorders but one had cerebellar degeneration and another a brainstem stroke. Five patients showed abnormally large body sway induced by full-field visual motion stimulation; including the 2 patients with CNS disease. Four of these 5 patients had additional strabismic symptoms (diplopia, squint surgery and ocular muscle weakness). VV may occur if patients with balance disorders show high visual field…

[Reeducation of patients with vertigo].

The use of physical therapy in the treatment of vertigo is based on the plasticity and the restoration and adaptive qualities of the vestibular apparatus. Indications for physical therapy and the choice of techniques are based on the location of the lesions: exercises aimed at habituation in positional vertigo, with use of the rocking manoeuvre in the case of cupulolithiasis, and stimulation of spinal, labyrinth, visual or extraocular muscle proprioceptors in the case of unilateral lesions of the first neuron or in ototoxic destruction. These techniques give lasting effects and avoid prolonged…

[Surgical treatment of vertigo].

Some types of incapacitating otologic vertigo can be successfully treated by surgery. Episodes of vertigo in Ménière's disease can be precluded either by so-called "draining" operations such as shunt of the saccus endolymphaticus, sacculotomy or cochleosacculotomy, or by ablations such as selective vestibular nerve neurectomy or labyrinthectomy. Persistent positional vertigo can be cured either by selective neurectomy of the posterior canal nerve, or by occlusion of the posterior semicircular canal. Vertigo can be caused by fistulas of the inner ear, which can be plugged with bone wax or greas…

Vertigo: assessment and management.

Elucidation of the cause of vertigo must consist of an adequate history to see if a clinical entity such as BPPV or Meniere's Disease can be recognised, and a general physical examination with emphasis on: The ear--discharge, perforation, fistula sign and hearing. Vestibular system--nystagmus and Romberg test with past pointing and positional testing. CNS--cranial nerves, cerebellar signs and gait. CVS--BP and bruits in neck and cardiac irregularities. Audiogram, ENG and CT or MRI. The specific treatment must be supplemented by sympathetic, symptomatic therapy and reassurance that the conditio…

[Therapy of vestibular vertigo].

The non-surgical treatment of vestibular disorders must be based on current knowledge of vestibular pathophysiology. It is generally accepted that after vestibular lesions a self-repair mechanism exists that allows a more or less complete recovery. In cases of persisting vestibular complaints the physician's duty consists in stimulation of these pre-existing mechanisms. This can be done by physical exercises, as has been recommended since the work of Cawthorne and Cooksey in 1946. This concept is meanwhile supported by modern neurophysiological research. This article describes a short training

Dyscalculia in patients with vertigo.

This paper reports a hitherto undescribed relationship between vertigo of central origin and dyscalculia. Subjects with vertigo skipped and displaced decades when counting backwards by two. The error is not recognized when presented visually. The subjects also display decrements in ability to do mental arithmetic and in central auditory processing. The results are discussed in light of the relationship between the central vestibular/auditory system and structures involved in higher cognitive function. The relationship between balance disorders and children with learning disabilities is also ex…

Vertigo in the hyperviscosity syndrome.

Diseases that cause hyperviscosity of the blood can result in otologic symptoms--especially vertigo. These symptoms are predominantly produced by peripheral vestibular involvement. The pathophysiology probably involves vascular obstruction in the venules. Specific changes in the microvasculature are presumed to be comparable to those that occur with hyperviscosity disorders in the retina. Maintenance of a normal blood viscosity will prevent damage to the ear, as well as other organs. Additionally, reduction of the blood viscosity can greatly improve otologic symptoms. Case studies and a review…

Vertigo and imbalance in the elderly.

The diagnosis of vestibular disorders causing dizziness and imbalance in the elderly can present a difficult problem. Knowledgeable diagnosis depends on an understanding of the role of the vestibular end-organs as motion sensors and sensors of gravity as well as an understanding of how the vestibular system influences body balance and how visual mechanisms interact with the vestibular system. Based on a knowledge of these physiological principles, a number of interesting aspects of age-related vestibular disorders are discussed in more detail.

Surgical treatment of vertigo.

Endolymphatic saccotomy, vestibular neurectomy via middle fossa, labrinthectomy and translabyrinthine vestibular neurectomy have been used by the authors in 54 patients with episodic vertigo, chiefly from Ménière's disease. Over a mean follow-up period of 15 months hearing retention and vertigo control were of the same general order as in the reports of other surgeons. Our current ideas on indications for the different surgical procedures are discussed.

Vertigo: a physiological approach.

An understanding of vestibular function and a careful history will quickly separate patients with disease of the vestibular system from others. Further history and physical examination, often coupled with audiometric and vestibular function tests, will aid in localizing the lesion to the central or peripheral portions of the system and in determining the affected side. Medical management, which is often nonspecific and involves considerable psychological support, usually suffices.

Clinical evaluation of vertigo.

In attempting to outline the clinical evaluation of these patients with vertigo we have discussed examples which range in order from benign self-limited disease to those requiring emergency surgery or extensive diagnostic evaluation. We sought also to illustrate how the logic of hypothesis testing is generally employed by clinicians in approaching this or other diagnostic problems. The examples were chosen to illustrate the indications for, as well as the limitations of, the various diagnostic modalities--caloric testing, electronystagmography, audiometric testing, roentgenographic and nuclear…
