The suspicion of a diagnosis of Rett syndrome (RTT) is based on clinical criteria that are often not present in the first two stages of the disease, as many of its symptoms will appear at a later age. Myoclonic epilepsy is being increasingly recognized as a late-onset complication in middle-aged or elderly patients with Down syndrome, in association with cognitive decline. We report on polysomnographic findings in a 9-year-old boy affected by rhythmic movement disorder. The subject's rhythmic movements were found to be intimately linked to unstable nonrapid eye movement N-REM sleep, as shown by their close association with the A phases of the cyclic alternating pattern. We examine the complex interactions between arousal mechanisms and rhythmic movements occurring during sleep.

BACKGROUND: Rett syndrome is a neurodevelopmental disorder that almost exclusively affects females. In addition to neurodevelopmental regression and loss of hand skills, apraxia, deceleration of head growth, and increasing spasticity and scoliosis, a number of behavioural features are also seen, including stereotypic hand movements, hyperventilation and breath holding. The aim of the study was to investigate the extent to which analogue environmental conditions affected the frequency of repetitive hand behaviour in eight girls and young women with Rett syndrome.

Gamma knife radiosurgery is generally considered a safer alternative to traditional pallidotomy or thalamotomy. We report the case of a 59-year-old patient with essential tremor who developed a complex, disabling movement disorder following gamma knife thalamotomy. This case illustrates the need for long-term follow-up to fully evaluate the potential for complications following radiosurgery.

The results of an experimental study of the motivational consequences of short-term prevention of crib-biting are reported here. Eight test horses wore a cribbing collar for 24 h. This was effective in preventing crib-biting in 6 subjects. Studies of the non-human temporal lobe, particularly the hippocampus, confirm its significant role in learning and memory, particularly allocentric spatial mapping of the environment. The role of the human temporal lobes in topographical orientation was investigated by examining the formation of representations of a large-scale real-world environment after unilateral left and right temporal lobe surgery. Patients and normal control subjects viewed videotape presentations of overlapping routes through a novel urban area. Topographical orientation was then assessed across a range of parameters.

Thirteen male patients consecutively admitted to an inpatient drug and alcohol rehabilitation hospital with the primary diagnosis of crack cocaine dependence were shown a 30-minute videotape that included salient environmental cues intended to elicit cocaine craving. The use of paramedical personnel to perform sigmoidoscopy as a screening test for colorectal cancer has been advocated as a means of increasing the availability of this test to the population at risk. A model system has been developed utilizing flexible videosigmoidoscopy performed by nurse practitioners with videotape review by physician endoscopists. In order to clarify contradictory reports concerning ciliary structure and function, follicular epithelium from macroscopically normal portions of 37 surgical specimens of human thyroid were processed for video-microscopy and/or transmission electron microscopy.
