Bacterial resistance to the beta-lactam drugs is extremely widespread, as a result of extensive drug use. Loss of susceptibility is primarily attributable to hydrolysis by inactivating enzymes, namely the beta-lactamases. While the number of characterised beta-lactamases may exceed 100, only a few are a problem in the treatment of community-acquired infections (TEM-1, TEM-2, SHV-1, BRO-1). Chromosomally mediated and extended-spectrum beta-lactamases are usually dominant in nosocomial pathogens where oral antibiotic therapy is seldom used. Therefore, the threat posed by beta-lactamases must be …

BACKGROUND: The problem of resistance to beta-lactam antibiotics, which is caused by ESBL and AmpC β-lactamases, is getting worse globally. Infections caused by bacterial isolates harboring these enzymes are difficult to treat with carbapenems being the sole effective treatment option for such infections. The objective of this study was to determine the frequency of ESBLs and AmpC-producing Gram-negative bacilli isolated from clinical specimens and to evaluate the sensitivity of cefepime-tazobactam combination against them.
METHODS: This is an observational cross-sectional study carried out o

This study is concerned with the level of antibiotic resistance of extended-spectrum beta-lactamase (ESBL)-producing Klebsiella pneumoniae, isolated in Shantou, China, and its mechanism. Seventy-four non-repetitive clinical isolates of K. pneumoniae producing ESBLs were isolated over a period of 2 years. Antibiotic susceptibility, carried out by Epsilometer test, showed that most of the isolates were multiresistant. Polymerase chain reaction showed that, among the several types of beta-lactamases, SHV was the most prevalent, TEM was the second most prevalent, and CTX-M was the least prevalent.…

Among the Enterobacteriaceae, Proteus vulgaris is exceptional in the inducible production of a 29-kDa beta-lactamase (cefuroximase) with an unusually high activity towards the beta-lactamase-stable oximino-cephalosporins (e.g. cefuroxime and cefotaxime). Sequencing of the corresponding gene, cumA, showed that the derived CumA beta-lactamase belonged to the molecular class A. The structural gene was under the direct control of gene cumR, which was transcribed backwards and whose initiation codon was 165 bp away from that of the beta-lactamase gene. This resembled the arrangement of structural a

Beta-lactam resistance poses a global issue and a considerable challenge to effective antimicrobial therapy. The study aimed to ascertain the phenotypic and genotype traits of carbapenemase, extended-spectrum beta (β)-lactamases (ESBL), and AmpC β-lactamase-producing isolates collected from hospitals. A range of clinical samples consisted of 63 Escherichia coli (E. coli) and 30 Klebsiella pneumoniae (K. pneumoniae) isolates. Phenotypic characterization was carried out utilizing the MASTDISCS® Combi ESBL, AmpC, and carbapenemase detection set-D72C (Mast Group Ltd, Bootle, United Kingdom). Molec

A systematic analysis of beta-lactamases based on comparative proteomics has not been performed thus far. In this report, we searched for the presence of beta-lactam-related proteins in 591 bacterial proteomes belonging to 52 species that are pathogenic to humans. The amino acid sequences for 19 different types of beta-lactamases (ACT, CARB, CifA, CMY, CTX, FOX, GES, GOB, IMP, IND, KPC, LEN, OKP, OXA, OXY, SHV, TEM, NDM, and VIM) were obtained from the ARG-ANNOT database and were used to construct 19 HMM profiles, which were used to identify potential beta-lactamases in the completely sequence…

Mycobacteria produce beta-lactamases and are intrinsically resistant to beta-lactam antibiotics. In addition to the beta-lactamases, cell envelope permeability and variations in certain peptidoglycan biosynthetic enzymes are believed to contribute to beta-lactam resistance in these organisms. To allow the study of these additional mechanisms, mutants of the major beta-lactamases, BlaC and BlaS, were generated in the pathogenic Mycobacterium tuberculosis strain H37Rv and the model organism Mycobacterium smegmatis strain PM274. The mutants M. tuberculosis PM638 (DeltablaC1) and M. smegmatis PM75…

Alteration of PBPs has proved an effective way for gram-positive bacteria to become resistant to beta-lactams. The gram-positive species have not been able to use decreased permeability or synthesis of novel beta-lactamases to overcome the advances by medicinal chemists. Nonetheless, altered PBPs have proved to be a formidable form of resistance for staphylococci, enterococci, and even some S. pneumoniae. Although isolated examples of resistance of gram-negative species to beta-lactams have been seen for E. coli, Serratia, or P. aeruginosa, in general this form of resistance has not been used …

PURPOSE: A dramatic increase in OXA-48 β-lactamase was observed recently not only in large hospital centres, but also in smaller suburban hospital centres in geographic areas bordering Croatia. The aim of the study was to analyse the epidemiology, the mechanisms of antibiotic resistance and the routes of spread of OXA-48 carbapenemase in Croatia.
METHODS: Carbapenemase and other β-lactamase and fluoroquinolone resistance genes were detected by PCR and sequencing. Whole-genome sequencing (WGS) was performed on five representative isolates. The isolates were genotyped by PFGE.
RESULTS: Forty-e

Carbapenem-resistant Enterobacteriaceae are amongst the most feared pathogens due to severely limited treatment options. In response to this threat, three novel β-lactamase inhibitors have been developed in an attempt to reinvigorate and sustain our current antimicrobial therapies. Avibactam, vaborbactam, and relebactam are inhibitor agents with high affinity to Ambler class A and C β-lactamases and favorable outcomes in current clinical trials. However, although they do possess key similarities, these agents have unique differences which may have important clinical implications. The microbiol

The discovery that the mechanism of beta-lactam hydrolysis catalyzed by the class A (active site serine-dependent) beta-lactamases proceeds via an acyl-enzyme intermediate was made thirty years ago. Since this discovery, the active site circumstance that enables acylation of the active site serine and further enables hydrolytic deacylation of the acyl-serine intermediate, has received extraordinary scrutiny. The justification for this scrutiny is the direct relevance of the beta-lactamases to the manifestation of bacterial resistance to the beta-lactam antibiotics, and the subsequent (to the d…

Members of the beta-lactam class of antibiotics, which inhibit the bacterial d,d-transpeptidases involved in cell wall biosynthesis, have never been used systematically in the treatment of Mycobacterium tuberculosis infections because of this organism's resistance to beta-lactams. The critical resistance factor is the constitutive production of a chromosomally encoded, Ambler class A beta-lactamase, BlaC in M. tuberculosis. We show that BlaC is an extended spectrum beta-lactamase (ESBL) with high levels of penicillinase and cephalosporinase activity as well as measurable activity with carbapen

Beta-lactamases confer bacterial resistance to beta-lactam antibiotics, such as penicillins. The characteristic class C beta-lactamase AmpC catalyzes the reaction with several key residues including Ser64, Tyr150, and Lys67. Here, we describe a 1.07 A X-ray crystallographic structure of AmpC beta-lactamase in complex with a boronic acid deacylation transition-state analogue. The high quality of the electron density map allows the determination of many proton positions. The proton on the Tyr150 hydroxyl group is clearly visible and is donated to the boronic oxygen mimicking the deacylation wate…

A review of antibiotic options for the treatment of infections caused by extended-spectrum beta-lactamase-producing isolates is presented. The use of the third-generation cephalosporin, cefotaxime, for infections caused by isolates producing ceftazidimase-type extended-spectrum beta-lactamases is controversial, despite in vitro susceptibility to the antibiotic in many instances. The fourth-generation cephalosporin, cefipime, although active against most extended-spectrum beta-lactamases, is reported to show a marked inoculum effect. The cephamycins, such as cefoxitin. are generally effective a…

Extended-spectrum beta-lactamases (ESBLs) are associated particularly with Klebsiella spp. These enzymes have arisen by mutation of the genes coding for clavulanate-sensitive, plasmid-mediated beta-lactamases such as TEM-1, TEM-2 and SHV-1. Amino acid changes in ESBLs confer enhanced hydrolysis of oxyimino-aminothiazolyl beta-lactams and aztreonam. Enzyme hyperproduction and loss of porins contribute to hydrolytic efficiency. ESBLs are highly susceptible to inhibition by clavulanate, and their presence can be detected by the disc-approximation test, using amoxycillin/clavulanate and an ESBL-su…

A multi-resistant strain of Escherichia coli isolated from a patient from Pakistan was shown to possess a new plasmid-mediated beta-lactamase. This enzyme, designated BIL-1, conferred resistance to extended-spectrum cephalosporins such as cefotaxime and ceftazidime. However, unlike other plasmid-mediated beta-lactamases capable of conferring resistance to these drugs, the BIL-1 was not a member of the TEM or SHV group of plasmid-mediated beta-lactamases and it also conferred resistance to beta-lactam drugs in combination with beta-lactamase inhibitors (i.e. clavulanic acid). The biochemical pr…

The presence of beta-lactamase producing bacteria (beta LPB) was investigated in specimens obtained from 1469 children who presented with infections of the skin and soft tissue (648), upper respiratory tract (514), pulmonary sites (137), surgical sites (113), and other (57). Of 4989 bacterial isolates recovered, 910 (18%) were beta LPB, 492 (54%) aerobes, and 418 (46%) anaerobes. The beta LPB were recovered in 751 (51%) of the children. The most frequently recovered beta LPB was Staphylococcus aureus, which was recovered in 356 (47%) patients. Most isolates were recovered from patients with sk

Imipenem has excellent antimicrobial activity owing in part to beta-lactamase stability. We found that only 2 of over 350 Bacteroides fragilis group clinical isolates were resistant to imipenem, with an MIC of more than 16 micrograms/ml. These two isolates from the Tufts Anaerobe Laboratory (TAL) were resistant to all other beta-lactam agents tested. The organisms were able to inactivate imipenem in broth cultures and contained similar beta-lactamases that were able to hydrolyze carbapenems, cephamycins, cephalosporins, and penicillins. The molecular sizes of the beta-lactamases in TAL2480 and…

The beta-lactamase activity with ampicillin (A) and cephalothin (Ce) as substrate was examined in 28 strains of Ent. cloacae and 24 strains of C. freundii by a micro-iodometric-(MIA) and an ultraviolet assay (UVA). Carbenicillin resistance (Ca-R) was present in 32 strains (16 of each species). All Ca-resistant (Ca-r) strains had elevated amounts of beta-lactamase. The conditions were very similar in the two species. The Ca-r strains could be divided into two subgroups. One group had very high ampicillinase activity and lower cephalothinase activity in MIA. In UVA the cephalothinase activity wa
