Alpha-lipoic acid: High risk, little reward. A case of severe intoxication.

BACKGROUND: Alpha-lipoic acid (ALA) is a naturally occurring antioxidant and mitochondrial cofactor, widely marketed as an over-the-counter dietary supplement. In clinical studies, ALA has been safe and well-tolerated at doses around 600 mg daily. However, several case reports worldwide have documented that overdose can result in rapid and severe toxicity. CASE REPORT: We describe an elderly woman who accidently ingested 6 g of ALA, mistaking it for her usual fiber supplement. Three hours later, she was in a confused state, with nausea, vomiting, and diaphoresis. On admission, she displayed a

Preparation of a pH-Responsive Janus Composite Fibrous Membrane: an Integrated Performance Study on the Visualized Detoxification of a Mustard Gas Simulant and Treatment of Mustard Gas-Induced Skin Injury.

Mustard gas (HD) causes ulcerative skin wounds; however, owing to its multiple mechanisms of action, no officially approved specific antidote has been developed to date. Moreover, current therapeutic materials for HD-induced skin injuries generally overlook the potential retention of residual HD at the wound site, which may adversely affect the healing process. Herein, a Janus membrane was rationally designed and successfully fabricated by electrospinning a blend of curcumin (denoted Cur)-loaded γ-cyclodextrin metal-organic framework (Cur@γ-CD-MOF) and polyacrylonitrile (PAN) onto a chitosan n

Nature's Poison Pill: A Case of Digoxin-Like Toxicity From the Seed of the Ornamental Plant Thevetia peruviana.

Thevetia peruviana (yellow oleander) is an ornamental shrub found in tropical and subtropical regions, including tropical America. All parts of the plant are toxic due to cardiac glycosides, which can cause severe cardiotoxicity and arrhythmias. Poisoning is associated with significant mortality, often due to deliberate self-harm. This report describes a case of accidental ingestion of Thevetia peruviana seeds by a patient who believed that they could treat benign prostatic hyperplasia (BPH). The patient presented to the emergency department within 30 minutes of ingesting yellow oleander seeds

A Synthetic, Small Molecule, Allosteric Inhibitor of Human Factor XIa for Thrombosis.

Only a handful of anticoagulants have reached the clinic to treat and prevent thrombotic episodes in the millions of patients suffering from cardiovascular, cancer, viral (COVID-19) and metabolic diseases. In this work, we report the discovery, chemical synthesis, mechanistic biochemistry, pharmacology, and toxicology of SB-01, a novel, allosteric, human factor XIa (FXIa) inhibitor that displays potent antithrombotic activity in preclinical models. SB-01 is a synthetic, highly sulfated, aromatic agent that binds in both anion-binding exosites of hFXIa to induce inhibition of factor IX activati

A fully human antibody A140 neutralizes abrin toxin via lysosomal intracellular mechanisms, confers potent protection in vitro and in vivo, and reverses transcriptomic dysregulation with a Favorable safety profile.

Abrin is a type II ribosome-inactivating protein extracted from the seeds of Abrus precatorius. Due to its extreme toxicity and ease of acquisition, it is classified as a Category B bioterrorism agent, and no effective antidote is currently available. In this study, a fully human antibody against abrin, designated A140, was generated via screening of a fully human phage-display antibody library and subsequent expression. The A140 antibody demonstrates high affinity while maintaining specificity for its target antigen. It exhibited potent neutralizing efficacy against abrin toxin in both in vit

Poison Apple-like Fruit Ingestion: A Case Report and Review of Manchineel Fruit Toxicity.

Manchineel dermatitis, caused by the sap of the manchineel tree native to the coastlands of the Caribbean, West Africa, and Central America, has received limited attention in the literature despite significant morbidity. The toxic sap causes a burning, blistering rash and conjunctivitis, while ingestion of the fruit may lead to oropharyngeal pain, hemodynamic instability, and gastrointestinal distress. Tourists unfamiliar with the manchineel tree may be exposed through exposure to the tree's sap, contaminated rainwater, or ingestion of the apple-like fruit. We herein present a case of a patien

Assessing the efficacy and safety of calcium gluconate soaking as a treatment modality for hydrofluoric acid burns.

INTRODUCTION: Hydrofluoric acid is extremely toxic and can corrode tissues while reacting with calcium and magnesium, leading to severe tissue damage. The antidote, calcium gluconate, is effective for treating mild to moderate burns. However, our hospital lacks registered calcium gluconate gel, making treatment challenging. We investigated alternative soaking methods due to scant literature on this approach. METHODS: This retrospective case series study reviews hydrofluoric acid burn cases at China Medical University Hospital from 2004 and 2020, focusing on the effectiveness of calcium glucon

Targeting the Enzymatic Site of Botulinum Neurotoxin Type E With 8-Hydroxyquinolinol-Based Inhibitors: In Silico, In Vitro, and In Vivo Evaluation.

Botulinum neurotoxins are the most potent toxins responsible for causing flaccid paralysis of muscles by blocking the release of acetylcholine at the neuromuscular junction. There are no postexposure therapeutics and effective active/passive prophylaxis available for the treatment. Therefore, it is highly desirable to develop a potential antidote to counter botulinum neurotoxicity. In this study, ~800 molecules were mined by a structure similarity search from open databases and docked into the pocket of the catalytic domain of botulinum toxin type E using AutoDock 4.2. Twenty-four small molecu

A new role for glucagon: from secondary hormone to key player.

BACKGROUND AND AIMS: In the years immediately after its discovery, glucagon was considered only as an "antidote" for the treatment of insulin-induced hypoglycemia. The advent of specific radioimmunoassay (RIA) techniques has enabled a better understanding of glucagon's role, revealing its close connection with the pathogenesis of diabetes mellitus. This review briefly summarizes the studies regarding the pathophysiology of glucagon and its different therapeutic implications in metabolic disorders. DATA SYNTHESIS: A comprehensive literature search was conducted utilizing PubMed and Scopus, emp

Ferroptosis in Human Diseases: Fundamental Roles and Emerging Therapeutic Perspectives.

Ferroptosis is a novel iron-sensitive subtype of regulated cell death (RCD), persisting under extreme lipid peroxidation and iron/redox imbalances. Unlike apoptosis, necroptosis, and pyroptosis, ferroptosis is a signaling-driven process mediated through iron metabolism imbalance, polyunsaturated fatty acid (PUFA) exceeding oxidation, and defects in its protective systems like Xc-/GSH/GPx4. Specifically, this review establishes that iron-driven ferroptosis is a central underlying pathomechanistic factor in a broad range of human diseases. Significantly, whether its modulation is therapeutic, it

Rhododendron poisoning in alpacas (Vicugna pacos) in Northern Germany.

Poisoning is often suspected to be the origin of disease in South American camelids (SACs) by owners, but only in a few cases this assumption can be confirmed. In small ruminants, rhododendron poisoning is a common emergency for livestock veterinarians. However, this condition has rarely been reported in SACs so far. This paper provides information regarding clinical findings, hematology, clinical chemistry, and treatment of four alpacas after presumed intake of rhododendron leaves including pathological findings of one of the animals. Rhododendron leaves contain grayanatoxins that lead to hyp

Selfish conflict underlies RNA-mediated parent-of-origin effects.

Genomic imprinting-the non-equivalence of maternal and paternal genomes-is a critical process that has evolved independently in many plant and mammalian species1,2. According to kinship theory, imprinting is the inevitable consequence of conflictive selective forces acting on differentially expressed parental alleles3,4. Yet, how these epigenetic differences evolve in the first place is poorly understood3,5,6. Here we report the identification and molecular dissection of a parent-of-origin effect on gene expression that might help to clarify this fundamental question. Toxin-antidote elements (

HSP70 Is a Critical Regulator of HSP90 Inhibitor's Effectiveness in Preventing HCl-Induced Chronic Lung Injury and Pulmonary Fibrosis.

Exposure to hydrochloric acid (HCl) can provoke acute and chronic lung injury. Because of its extensive production for industrial use, frequent accidental exposures occur, making HCl one of the top five chemicals causing inhalation injuries. There are no Food and Drug Administration (FDA)-approved treatments for HCl exposure. Heat shock protein 90 (HSP90) inhibitors modulate transforming growth factor-β (TGF-β) signaling and the development of chemical-induced pulmonary fibrosis. However, little is known on the role of Heat Shock Protein 70 (HSP70) during injury and treatment with HSP90 inhibi

Regulation of NCOA4-mediated iron recycling ameliorates paraquat-induced lung injury by inhibiting ferroptosis.

Paraquat (PQ) is an irreplaceable insecticide in many countries for the advantage of fast-acting and broad-spectrum. However, PQ was classified as the most prevailing poisoning substance for suicide with no specific antidote. Therefore, it is imperative to develop more effective therapeutic agents for the treatment of PQ poisoning. In the present study, both the RNA-Seq and the application of various cell death inhibitors reflected that ferroptosis exerts a crucial regulatory role in PQ poisoning. Moreover, we found PQ strengthens lipid peroxidation as evidenced by different experimental appro

Detoxification of hydrogen sulfide by synthetic heme model compounds.

Hydrogen sulfide is a lethal toxic gas that disrupts cellular respiration in the mitochondrial system. Currently, no antidote is available for the clinical treatment of hydrogen sulfide poisoning. In this study, we investigated the function of iron(III)porphyrin complexes as hydrogen sulfide scavengers in water and evaluated their potential use as therapeutic agents for hydrogen sulfide poisoning. The compounds, named met-hemoCD-P and met-hemoCD-I, are composed of iron(III)porphyrin complexed with per-methylated β-cyclodextrin dimers that contain a pyridine (met-hemoCD-P) or imidazole axial fi

Deletion of Glyoxalase 1 Exacerbates Acetaminophen-Induced Hepatotoxicity in Mice.

Acetaminophen (APAP) overdose triggers a cascade of intracellular oxidative stress events, culminating in acute liver injury. The clinically used antidote, N-acetylcysteine (NAC), has a narrow therapeutic window, and early treatment is essential for a satisfactory therapeutic outcome. For more versatile therapies that can be effective even at late presentation, the intricacies of APAP-induced hepatotoxicity must be better understood. Accumulation of advanced glycation end products (AGEs) and the consequent activation of the receptor for AGEs (RAGE) are considered one of the key mechanistic fea

In situ investigation of detoxification and metabolic effects of polyfluoroalkyl substances on metal-organic frameworks combined with cell-cultured microfluidics.

Over 9000 types of per- and polyfluoroalkyl substances (PFASs) have been produced that exhibit environmental persistence, bioaccumulation and biotoxicity, and pose a potential hazard to human health. Although metal-organic frameworks (MOFs) are promising structure-based materials for adsorbing PFASs, the enormous structural diversity and variability of the pharmacologic action of PFASs present challenges to the development of structure-based adsorbents. To address this issue, we propose an in situ platform for the high-throughput identification of efficient MOF sorbents that can adsorb PFASs a
