Psychotherapies and digital interventions for OCD in adults: What do we know, what do we need still to explore?

BACKGROUND: Despite significant advances in the understanding and treatment of obsessive compulsive disorder (OCD), current treatment options are limited in terms of efficacy for symptom remission. Thus, assessing the potential role of iterative or alternate psychotherapies is important. Also, the potential role of digital technologies to enhance the accessibility of these therapies, should not be underestimated. We also need to embrace the idea of a more personalized treatment choice, being cognisant of clinical, genetic and neuroimaging predictors of treatment response. PROCEDURES: Non-syst

[Prevalence of depressive disorders in children and adolescents attending primary care. A survey with the Aquitaine Sentinelle Network].

UNLABELLED: Since depressive disorders in children and adolescents have not been widely studied in the context of gene-ral medicine, we conducted an epidemiological survey among general practitioners (GP's) consulted by young subjects aged 7 to 17 years for various reasons. OBJECTIVE: The aims were the following: to estimate the prevalence of depressive disorders in general practice, to detect the eventual existence of particular clinical forms, to assess the frequency of comorbid disorders and to determine to what degree these disorders were diagnosed by GP's. METHOD: The study was conducte

LonelyFans? Longitudinal trajectories of adult content subscription platform engagement over two years.

BACKGROUND: Subscription-based adult content creator platforms (ACCPs; eg, OnlyFans) represent a rapidly expanding form of computer-mediated sexual media that blends monetized interaction, personalization, and ongoing engagement between users and creators, yet little is known about the psychological correlates of involvement with these platforms over time. AIM: To examine longitudinal associations between paid engagement with ACCPs, loneliness, and symptoms of Compulsive Sexual Behavior Disorder (CSBD). METHODS: Associations between paid engagement with ACCPs, loneliness, and CSBD symptoms w

On discriminating and not discriminating between affect and representation.

The topic involves two issues. They can be related either to different clinical pictures or to divergent opinions about the same clinical facts. But the above mentioned opposition can be found in Freud's work. Listening in analysis differentiates between situations where the distinction of affect and representation is blurred within the general frame of communication and others where this distinction imposes itself because of the critical or chronic character of the predominance of affect. Problems about the discrimination between affect and representation in the unconscious are elucidated in

[Failure effects and gender differences in perfectionism].

Perfectionism is a dimension which has been studied very little as a separate entity. It is not even considered as a nosological factor. No classification of the medical sciences underlines its importance other than to speak of a personality trait, of an aspect, or of a parameter. Nevertheless, perfectionism is related to multiple disorders such as depression (18, 20, 36), suicide (8, 16, 55), nutritional problems (11, 28), anxiety (3), obsessive-compulsive personality disorder (53), social phobia (2), as well as insomnia (46). Certain authors stress the possible role of perfectionism in the d…

Examining the Roles of Technology Across the Health Care Journey for Individuals With Obsessive-Compulsive Disorder: Qualitative Interview Study.

BACKGROUND: As digital technologies become increasingly embedded in daily life, their roles in mental health care have expanded and diversified. Digital tools are being explored as interventions for obsessive-compulsive disorder (OCD) across the care continuum, including symptom recognition, access to care, treatment, and self-management. However, there is limited empirical understanding of how individuals living with OCD use digital technologies in situ to navigate their health care journeys or how they envision technology shaping future models of care. OBJECTIVE: This study aimed to explore

Transcranial Magnetic Stimulation in Obsessive-Compulsive Disorder and Adolescent Depression: A Systematic Review of Efficacy, Safety, and Predictors of Treatment Response.

Obsessive-compulsive disorder (OCD) and adolescent depression are debilitating conditions where standard treatments often yield suboptimal outcomes. Transcranial magnetic stimulation (TMS) is a non-invasive neuromodulation technique with established efficacy in adults with OCD, but its role in adolescent depression remains less defined. This systematic review aims to synthesize the current evidence on the efficacy, safety, and predictors of treatment response for TMS in adults with OCD and adolescents with depression. This review was conducted in accordance with the PRISMA (Preferred Reporting

Feasibility of Using ChatGPT to Generate Exposure Hierarchies for Treating Obsessive-Compulsive Disorder.

Obsessive-compulsive disorder (OCD) is a chronic, severe condition. Although exposure and response prevention (ERP), the first-line treatment for OCD, is highly effective, too few clinicians are equipped to deliver it. One barrier is the time and expertise required to develop personalized exposure hierarchies. In this study, we examined the feasibility and promise of using large language models (LLMs) to generate appropriate exposure suggestions for OCD treatment. We used ChatGPT-4 (Generative Pretrained Transformer, Version 4) to generate 10-item exposure hierarchies for simulated patient cas

Cognitive behavioural therapy and third-wave approaches for anxiety and related disorders in older people.

BACKGROUND: Cognitive behavioural therapy (CBT) is the most researched psychological therapy for anxiety disorders in adults, and known to be effective in this population. However, it remains unclear whether these results apply to older adults, as most studies include participants between 18 and 55 years of age. This systematic review aims to provide a comprehensive and up-to-date synthesis of the available evidence on CBT and third wave approaches for older adults with anxiety and related disorders. OBJECTIVES: To assess the effects of Cognitive Behavioural Therapy (CT, BT, CBT and third-wav

Stakeholder Criteria for Trust in Artificial Intelligence-Based Computer Perception Tools in Health Care: Qualitative Interview Study.

BACKGROUND: Computer perception (CP) technologies hold significant promise for advancing precision mental health care systems, given their ability to leverage algorithmic analysis of continuous, passive sensing data from wearables and smartphones (eg, behavioral activity, geolocation, vocal features, and ambient environmental data) to infer clinically meaningful behavioral and physiological states. However, successful implementation critically depends on cultivating well-founded stakeholder trust. OBJECTIVE: This study aims to investigate, across adolescents, caregivers, clinicians, and devel

Breaking the cycle: Exploring the relationship of metacognition beliefs, obsessive-compulsive symptoms, and psychosocial performance among individuals diagnosed with schizophrenia.

THE RELEVANCE TO MENTAL HEALTH NURSING: This research paper explores the intricate relationship between metacognitive dysfunctional beliefs, obsessive-compulsive symptoms, and psychosocial performance in patients diagnosed with schizophrenia. Understanding these dynamics can help mental health nurses identify and address each patient's needs more effectively. It can guide them in devising personalized care plans that not only manage the symptoms but also improve the underlying mechanism that exacerbates the psychotic symptoms and social functioning and the overall quality of life. Moreover, th

[Psychiatric and psychological aspects of premenstrual syndrome].

Numerous, but heterogeneous studies have been performed about premenstrual syndrome, with finally a lack of credibility and interest among practitioners. More recently with the diagnosis criteria generalization, psychiatrists were more concerned about this syndrome, because of anxiety and mood symptoms involved in social impairment and need of medical care. In 1983 in the United States, the National Institute of Mental Health conference devoted to this topic proposed the first diagnosis criteria, requiring a prospective and daily assessment of the symptoms. In 1987, the American Psychiatric As

Trichotillomania: Bizzare Patern of Hair Loss at 11-Year-old Girl.

Trichotillomania (TTM) is defined by the Diagnostics and Statistic Manual of Mental Disorders, 4th edition (DMS-IV) as hair loss from a patient`s repetitive self-pulling of hair. The disorder is included under anxiety disorders because it shares some obsessive-compulsive features. Patients have the tendency towards feelings of unattractiveness, body dissatisfaction, and low self-esteem (1,2). It is a major psychiatric problem, but many patients with this disorder first present to a dermatologist. An 11-year-old girl came to our department with a 2-month history of diffuse hair loss on the fron

[Post-traumatic psychosis: the contribution of an acute post-immediate trauma to a dynamic theorization].

SUMMARY: In his recent publication (2004), in which he favors the psycho-dynamic approach, M. Bertrand underlines the importance of traumatic matter at the heart of new clinical, methodological and epistemological challenges of psychopathology. TRAUMATIC PSYCHOSIS CONCEPT: Our contribution ("post-traumatic psychosis") comes within that prospect in reference to the semiology of traumas generated by extreme situations, such as the tsunami in South Asia (December 2004), the terrorist attacks in Madrid (March 2004) or the hostage taking in North Ossetia (September 2004). Four principle sets of th

Development of a protocol for videoconferencing-based exposure and response prevention treatment of obsessive-compulsive disorder during the COVID-19 pandemic.

BACKGROUND: The existing literature indicates that psychotherapeutic treatment, especially exposure and response prevention (ERP) is efficacious in treating obsessive-compulsive disorder (OCD). The coronavirus disease 2019 pandemic adversely impacted many patients with OCD and disrupted their usual treatment. Moreover, the pandemic forced a global switch to telemental health (TMH) services to maintain the standards and continuity of care. Consequently, clinicians are increasingly using TMH-based psychotherapeutic treatments to treat OCD. However, several challenges have made it difficult for t

Association of Perinatal Risk Factors With Obsessive-Compulsive Disorder: A Population-Based Birth Cohort, Sibling Control Study.

IMPORTANCE: Perinatal complications may increase the risk of obsessive-compulsive disorder (OCD). Previous reports were based on small, retrospective, specialist clinic-based studies that were unable to rigorously control for unmeasured environmental and genetic confounding. OBJECTIVE: To prospectively investigate a wide range of potential perinatal risk factors for OCD, controlling for unmeasured factors shared between siblings in the analyses. DESIGN, SETTING, AND PARTICIPANTS: This population-based birth cohort study included all 2 421 284 children from singleton births in Sweden from Jan

Dear Editor, The 11th revision of the International Classification of Diseases and Related Health Problems (ICD-11), including the chapter on mental, behavioural and neurodevelopmental disorders, has been adopted unanimously by the 72nd World Health Assembly in Geneva on May 25, 2019. The endorsement of the new classification will not come into effect until January 1, 2022. Until that date, the Member States of the World Health Organization (WHO) will keep on using the ICD-10 for reporting data. The most significant innovations in the ICD-11 chapter, and the most important differences from the

Artificial Intelligence in psychotherapy: Inevitable evil or developmental evolution? Complementation or parallel universe?

"This is an artificial intelligence chatbot and not a real person. Treat everything it says as fiction. What is said should not be considered facts or advice," these are the first words that Gharacter.Ai, a counseling chatbot, says to those who turn to it for help. Is it common for humans to turn to machines for psychotherapy? We do it for most of our daily lives, often taking it for granted. In fact, we often prefer to interact with algorithms rather than other homo sapiens. Is it just as easy or efficient to seek help from machines for mental health issues? The global market for chatbots for
