Management of post-menopausal vaginal atrophy and atrophic vaginitis.

The involution of the female genital tract seems to reflect a built-in biological life expectancy, inter-related with the hypothalamic-hypophyseal-ovarian axis. Lower levels of oestradiol have a number of adverse effects, including on the lower urinary tract. The major universal change is vaginal atrophy. The vaginal mucosa becomes thinner and dry, which can produce vaginal discomfort, dryness, burning, itching, and dyspareunia. The vaginal epithelium may become inflamed, contributing to urinary symptoms such as frequency, urgency, dysuria, incontinence, and/or recurrent infections. Moreover,

[Retinal and pineal melatonin--from a circadian signal to therapeutic use].

Melatonin, the major product of the pincal gland, is also synthesized in the retina of several vertebrate species. Retinal melatonin levels show a marked daily variation, with high levels at night and low levels during the light period. Melatonin rhythm generating system includes photodetector, circadian clock and melatonin synthesis machinery and they are located in pincal organ and retina. In both, light regulates daily variations in melatonin secretion by controlling the activity of arylalkylamine N-acetyltransferase (AANAT). In mammals, circadian system is comprised of three major componen

[Monosymptomatic hyperthyroidism and TSH-producing adenoma: successful therapy with octreotide].

HISTORY AND FINDINGS: Magnetic resonance imaging (MRI) of the central nervous system was performed on a 72-year-old woman who was hyperthyroid without suppression of the thyroid-stimulating hormone (TSH) and had complained of a recent onset of headaches. MRI demonstrated a space-occupying lesion, 1 cm in diameter, in the anterior pituitary. The clinical symptoms were marked by a long-standing monosymptomatic illness of rapidly changing mood swings with depressive and manic phases. INVESTIGATIONS: Endocrinological-biochemical tests showed hyperthyroidism (fT3 10.55 pmol/l and fT4 39 pmol/l) bu

Morphogenesis of the hypothalamus and hypophysis: their association, dissociation and reassociation before and after "Rathke".

Recent progress in the experimental morphology of the development of amphibian pituitary gland is reviewed. A series of transplantation experiments were carried out using wild-type embryos of the toad as a donor and albino embryos as a recipient. Melanin granules in the wild-type cells allowed tracing of the developmental fate of the grafts as a visible cell marker. These studies have demonstrated that the pituitary gland is not a stomodeal derivative, as it has long been believed to be under the name of "Rathke's pouch". The adenohypophysis is of neural origin. The anterior part of the neural

[Prolonged pregnancy--prostaglandins as the cause of labor onset].

The causes of prolonged pregnancy are still largely unknown and their investigation requires a detailed observation of potential birth-initiating stimuli on the endocrine and biomolecular level. A large number of clinical and biochemical studies point to the central importance of prostaglandins for the beginning of human birth. The main places of origin of the intensified prostaglandin formation and release are the amnion and the decidua which has "macrophage-like" properties and functions. The superordinate regulation and trigger mechanisms for intensified uterine prostaglandin production has

[Primary empty sella syndrome. Report of 6 cases].

The extension of the suprasellar subarachnoid space through an incompetent diaphragma sellae into the sella turcica is defined as empty sella syndrome (ESS). The primary form arises in the absence of previous pituitary surgery or irradiation. Predominance of obese, middle aged, often multiparous women are generally observed; clinically headaches and slight endocrine alterations are frequent but not characteristic symptoms. Rarely liquor rhinorrhea or visual campimetric defects may occur. The authors report six cases observed in their Departments of Internal Medicine during the last two years;

[How frequent are pathologic ENT findings in patients with obstructive sleep apnea syndrome?].

Several studies have been reported suggesting a relationship between pharyngeal obstruction due to ENT pathology and the sleep apnea syndrome (SAS). To determine the incidence of pathological ENT findings that may present symptoms similar to SAS, we performed ENT examination, fiberoptic nasopharyngoscopy, rhinomanometry and partial audiometry and electronystagmography in 431 patients who had undergone polysomnography for clinically suspected SAS. 336 patients were referred for ENT examination; 95 patients had some kind of ENT disease and therapy before polysomnography. In the first group 31% s

Structural changes in nuclear chromatin in rat pituitary after chronic stress of low intensity.

Acute, intense sources of "psychogenic" stress clearly modify the structure and function of the hypophysis, and there are concomitant changes in many peripheral physiological systems. Less dramatic sources of stress yield more equivocal results. An experiment is reported in which nuclear morphology of adenohypophyseal cells from 49 male rats exposed to a chronic, low-intensity stressor was examined both by conventional histological and computer-assisted-image-processing methods. The hypothesis tested was that an unequivocal pattern of morphological changes in the nucleus and nuclear chromatin

Hypothalamic-pituitary function in growth hormone-deficient patients with pituitary stalk transection.

We compared 1.5 T magnetic resonance (MR) image findings with hypothalamic-pituitary function in 11 patients with idiopathic pituitary dwarfism, each of whom had a history of perinatal abnormalities, and 1 patient with posttraumatic pituitary dwarfism. MR imaging revealed transection of the pituitary stalk in all patients and the formation of an ectopic posterior lobe at the proximal stump in 9 patients, none of whom had polydipsia or polyuria. Three patients without an ectopic posterior lobe had diabetes insipidus. The 5 patients who had small pituitary glands (less than 2 mm in height) had h

Effect of hypothalamic ventromedial lesions on plasma growth hormone response to growth hormone-releasing factor in rats.

The effect of ventromedial-arcuate (VMH-ARC) nuclei lesions on plasma growth hormone (GH) response to human growth hormone-releasing factor (GRF, 1 microgram/kg b.wt., i.v.) was studied in conscious rats after they had received chlorpromazine (CPZ) or CPZ plus antiserum against somatostatin (ASS). When rats were pretreated with CPZ alone, there was no difference in basal plasma GH level between VMH-ARC lesioned rats and controls. The magnitude of plasma GH response to GRF in 5 out of 6 VMH-ARC lesioned rats exceeded that of controls. When the same observation was repeated using the same rats a

Role of the autonomic nervous system in the mediation of LHA electrical stimulation-induced effects on insulinemia and glycemia.

The short-term effects of bilateral electrical stimulation of the lateral component of the lateral hypothalamic/medial forebrain bundle area (LHA) on peripheral plasma levels of glucose (G), immunoreactive insulin (IRI) and glucagon (IRG) were measured in food-derived, anesthetized rats in the presence of continuous low i.v. glucose infusion. In normal rats LHA stimulation induced a rapid but moderate rise in IRI in the presence of a simultaneous rapid rise in G. At the end of the 5 min stimulation period the glucose-induced IRI response was clearly attenuated. In rats receiving a continuous p

[Pulsation echoencephalography--a bloodless method of measuring intracranial pressure].

By means of a special procedure an echo encephalograph can be modified in such a manner that changes in the amplitude of an echo peak can be detected and prepared for registration. In this iaay it is possible to single out the medium echo pulsations of the one-dimensional echo encephalography which have been known for a long time and prepare them for recording. In a number of zna persons without an indication of an increase in the intracranial pressure, medium echo pulkmon and ECG were simultaneously recorded. The quotient of the time T1 and R--T of the ECG was calculated 120 times and the res

Epigenetic mechanisms in stress and the hypothalamic-pituitary-adrenal axis.

The hypothalamic-pituitary-adrenal (HPA) axis is central to regulating the body's stress response. While HPA axis function enables physiologic and behavioral adaptation to stress, dysregulation of its feedback mechanisms - driven by early-life adversity, chronic stress, trauma, or illness - contributes to the pathophysiology of various physical and neuropsychiatric disorders. Gene expression is controlled by chromatin plasticity, which involves epigenetic mechanisms such as DNA methylation, histone modifications, and noncoding RNA regulation. Animal models demonstrate that early-life stressors

Emotional Eating and Abdominal Obesity: A Narrative Review of the Potential Mechanisms Underlying Their Relationship and Emerging Interventions for Their Management.

Abdominal obesity (AO), assessed through waist circumference (WC), has become a validated complementary anthropometric marker for cardiometabolic risk assessment. Growing evidence suggests that emotional eating (EE), characterized by food intake in response to emotions rather than physiological hunger cues, may be linked to AO by promoting dysregulated eating patterns rich in palatable and energy-dense foods. This behavior may contribute, directly or indirectly, to excess visceral fat accumulation. An analysis of the current evidence was conducted to examine the psychological, physiological, n

Depression and breast cancer: research progress and prospects from an interdisciplinary perspective.

Depression and breast cancer are two major health concerns in the medical field today. Not only do they mutually influence the risk of onset and disease progression, but they also collectively lead to poorer clinical outcomes and reduced quality of life. This review elaborates on how depression, through the overactivation of the hypothalamic-pituitary-adrenal (HPA) axis, may be associated with persistently elevated levels of glucocorticoids (GCs). This, in turn, causes dysregulation of the sex hormone axis, prolactin imbalance, and insulin resistance, collectively promoting the initiation and

A multistable slow-fast model of affective state switching under circadian drive.

Rhythmic mood fluctuations are often associated with circadian timing, but it remains unclear how daily rhythms that normally organize physiological affective variation also shape transitions into longer pathological episodes in bipolar disorder. We introduce a phenomenological slow-fast model of affective dynamics inspired by hypothalamic-pituitary-adrenal-axis homeostatic feedback. A fast affective coordinate evolves on a multistable landscape whose geometry is shaped by a slow homeostatic variable, producing four stable wells: depression-like, physiological low, physiological high, and mani

From Mechanisms to Medicine: Astrocyte Dysfunction in Stress-Related Neuroinflammation and Alzheimer's Disease.

Chronic stress is increasingly acknowledged as a pivotal precipitating factor in the pathogenesis of neuropsychiatric and neurodegenerative disorders, notably including depression and Alzheimer's disease (AD). Astrocytes, which constitute the predominant population of glial cells involved in the maintenance of synaptic homeostasis, the recycling of neurotransmitters, and the provision of metabolic support, display a pronounced susceptibility to sustained exposure to stress. The deleterious effects of astrocytic dysfunction instigate a series of neuroinflammatory and synaptic modifications that

"Feeding the Rhythm"-Effects of Food and Nutrients on Daily Cortisol Secretion: From Molecular Mechanisms to Clinical Impact.

Daily rhythms define physical, mental, and behavioral changes that the body experiences over a 24 h cycle. The light-dark cycle plays a crucial role in regulating daily rhythms, but other factors such as food intake, stress, and physical activity also affect them. Cortisol secretion exhibits one of the largest endocrine amplitudes, with an early morning peak and late-evening nadir driven by the suprachiasmatic nucleus and hypothalamus-pituitary-adrenal axis, representing the most robust endocrine output of the circadian system. Beyond photic cues, feeding is a potent non-photic zeitgeber that

Hormonal and Behavioral Consequences of Social Isolation and Loneliness: Neuroendocrine Mechanisms and Clinical Implications.

Social isolation and loneliness represent critical psychosocial stressors associated with profound hormonal dysregulation and adverse behavioral outcomes. This review synthesizes current evidence on neuroendocrine mechanisms linking perceived and objective social disconnection to health consequences, emphasizing hypothalamic-pituitary-adrenal axis dysfunction, altered glucocorticoid signaling, and inflammatory pathways. Loneliness activates conserved transcriptional responses with upregulated proinflammatory gene expression and downregulated antiviral responses, mediated through sustained cort
