CASE: A 32 year old woman was referred because of secondary amenorrhea, hirsutism, and voice deepening.
OUTCOME: The blood testosterone level was markedly high. A transvaginal ultrasound revealed a small region in the left ovary, but whether or not it was a tumor was unclear. Therefore, selective ovarian venous sampling was performed. Steroidal and gonadotropic secretion were studied in infertile men with varicocele. Hormonal values were compared with those from control groups. No significant difference was found between control and pathologic groups in relation to prolactin secretion. FSH and LH showed increased basal values in 16% and 8% these cases in the group studies. This study reviews our experience with the presentation of occult echo-poor lesions of the testis in patients who were seen with problems such as infertility, prostatitis or orchialgia. A range of pathologic problems revealed by scrotal ultrasonography in these patients is presented, and we compared surgical and pathological findings to those of preoperative scrotal ultrasonography. Scrotal ultrasound appears to be a highly sensitive but nonspecific diagnostic technique.
