Intrathecal baclofen therapy: challenges in patients with multiple sclerosis.

Intrathecal baclofen (ITB) is a recognized therapy for severe spasticity of both spinal and cerebral origin, with documented positive clinical outcomes for many patients with multiple sclerosis (MS). This article reviews some specific considerations concerning the use of ITB in the MS population, based on the author's 10 years of clinical experience. The unpredictable and progressive nature of the disease affects the identification of appropriate goals, the patient's decision-making process, and the evaluation of the response to the treatment over time. Patients with MS may be more sensitive t

Parent-training programmes for improving maternal psychosocial health.

BACKGROUND: Mental health problems are common, and there is evidence from a range of studies to suggest that a number of factors relating to maternal psychosocial health can have a significant effect on the mother-infant relationship, and that this can have consequences for both the short and long-term psychological health of the child. The use of parenting programmes is increasing in the UK and evidence of their effectiveness in improving outcomes for mothers is now required. OBJECTIVES: The objective of this review is to address whether group-based parenting programmes are effective in impr

Hyperactive Delirium Temporally Associated With Hydroxyzine in a Child With Succinic Semialdehyde Dehydrogenase Deficiency Presenting With Psychotic Symptoms: A Case Report.

BACKGROUND: Succinic semialdehyde dehydrogenase deficiency (SSADHD) is a rare autosomal recessive disorder of gamma-aminobutyric acid (GABA) metabolism caused by biallelic variants in ALDH5A1. The phenotype includes developmental delay, intellectual disability, seizures, sleep disturbance, anxiety, and other neurobehavioral symptoms. Psychotic symptoms are increasingly recognized in SSADHD but remain uncommon and incompletely characterized in children. In medically and neurodevelopmentally vulnerable patients, acute psychiatric decompensation may also be complicated by delirium, especially whe

Co-Design of a Depression Self-Management Tool for Adolescent and Young Adult Cancer Survivors: Rapid Qualitative Analysis of Interview Feedback on a Prototype.

BACKGROUND: Over 2.1 million adolescent and young adult cancer survivors (AYACS) live in the United States. Recent estimates suggest that up to one-third of AYACS experience major depressive disorder. Although several efficacious evidence-based interventions are available to manage symptoms of depression, these interventions are often inaccessible to AYACS who have many competing commitments. Digital mental health tools hold promise for this population; however, only a few have been tailored to meet the unique needs of AYACS, and findings to date have yielded mixed results. OBJECTIVE: This st

Possible Role and Function of AI Conversational Agents in Dialectical Behavior Therapy for Borderline Personality Disorder: Qualitative Interview Study.

BACKGROUND: Borderline personality disorder (BPD) is associated with substantial distress and a high risk for suicide. Individuals with BPD may be unable to access evidence-based treatments like dialectical behavior therapy (DBT). Artificial intelligence conversational agents (AI-CA) are increasingly discussed as scalable tools for mental health support, but little is known about how DBT clinicians understand the possible role of AI-CA in treatment. OBJECTIVE: This study aimed to explore DBT psychologists' perspectives on integrating AI-CA into DBT for BPD in the future. METHODS: Seventeen p

Adverse childhood experiences and risk of living with HIV among lesbian, gay, bisexual, transgender, and queer/questioning (LGBTQ+) people: a cross-sectional study.

BACKGROUND: Adverse childhood experiences and HIV represent interconnected public health issues. However, studies characterizing the prevalence and associations remain limited among sexual and gender minorities (i.e., lesbian, gay, bisexual, transgender, and queer/questioning individuals, collectively referred to as LGBTQ+), especially in China. This study aimed to examine adverse childhood experiences patterns, association with risk of living with HIV, and potential mediating factors among LGBTQ+ individuals in China. METHODS: An online survey was provided to LGBTQ+ people and the eligibilit

Informed consent and ethical issues pertaining to female sterilization-Scoping review.

BACKGROUND: Female sterilization, a safe, permanent method of contraception that blocks the fallopian tubes, has been in use since the 19th century. The procedure necessitates informed consent, a critical step that has been marred by reports of forced sterilization since World War II. These incidents often stem from inadequate consent processes where ethical principles are overlooked or deliberately flouted. The persistent issue of forced sterilization, primarily attributable to a flawed informed consent process, highlights significant ethical concerns. OBJECTIVES: This scoping review aimed t

Treatment Patterns for Incident Bipolar Disorder Among Nonrefugee Immigrants, Refugees, Second-Generation Immigrants, and Host Population in Sweden.

BACKGROUND: Deviations in treatment practices toward immigrant groups compared to host populations are common in mental disorders but unknown in bipolar disorder (BD). We aim to close this research gap by analyzing age-stratified use patterns of antidepressants, mood stabilizers, and antipsychotics following an incident diagnosis of BD in Swedish-born, second- and first-generation nonrefugee immigrants, and refugees. METHODS: Individuals with incident BD between 2006 and 2018 were identified through Swedish national registers. Medication use was followed up until 5 years after diagnosis. Use

Impact of Sports on Female Growth and Pubertal Development: A Cohort Study.

Introduction The participation of women in sports is increasing, and the rising training demands may impact growth and pubertal development. High-intensity sports are often linked to delayed growth and bone maturation due to energy deficits and intense regimens. These factors may increase the risk of injury and musculoskeletal issues. Additionally, elite athletes face higher incidences of eating disorders and the female athlete triad, affecting both physical and mental health. This article aims to examine the impact of various sports on female physiological development, particularly during pub

Developing a novel co-produced methodology to understand 'real-world' help-seeking in online peer-peer communities by young people experiencing emotional abuse and neglect.

BACKGROUND: Recent systematic reviews suggest mediating factors (barriers and facilitators) of help-seeking for children and young people (CYP) with a range of mental health problems but highlight the need for a more robust methodology underpinned by theoretical frameworks. Emotional abuse and neglect is the most prevalent and pervasive form of abuse, with many CYP remaining unknown to professional services. These CYP are underrepresented in help-seeking research yet seek help from their peers via anonymous online publicly available message communities. OBJECTIVES: To sensitively co-develop q

The Effect of Varying Test Administration and Scoring Procedures on Three Tests of (Central) Auditory Processing Disorder.

BACKGROUND: There is currently no widely accepted objective method used to identify (central) auditory processing disorder ([C]APD). Audiologists often rely on behavioral test methods to diagnose (C)APD, which can be highly subjective. This is problematic in light of relevant literature that has reported a lack of adequate graduate-level preparation related to (C)APD. This is further complicated when exacerbated by the use of inconsistent test procedures from those used to standardize tests of (C)APD, resulting in higher test variability. The consequences of modifying test administration and s

Long-term survival and cause-specific mortality of patients newly diagnosed with tuberculosis in São Paulo state, Brazil, 2010-15: a population-based, longitudinal study.

BACKGROUND: Long-term survival and cause-specific mortality of patients who start tuberculosis treatment is rarely described. We aimed to assess the long-term survival of these patients and evaluate the association between vulnerable conditions (social, health behaviours, and comorbidities) and cause-specific mortality in a country with a high burden of tuberculosis. METHODS: In this population-based, longitudinal study in São Paulo state, Brazil, we described the 5-year survival of patients who were newly diagnosed with tuberculosis in 2010. We included patients with newly-diagnosed tubercul

Incremental value of biomarker combinations to predict progression of mild cognitive impairment to Alzheimer's dementia.

BACKGROUND: The progression of mild cognitive impairment (MCI) to Alzheimer's disease (AD) dementia can be predicted by cognitive, neuroimaging, and cerebrospinal fluid (CSF) markers. Since most biomarkers reveal complementary information, a combination of biomarkers may increase the predictive power. We investigated which combination of the Mini-Mental State Examination (MMSE), Clinical Dementia Rating (CDR)-sum-of-boxes, the word list delayed free recall from the Consortium to Establish a Registry of Dementia (CERAD) test battery, hippocampal volume (HCV), amyloid-beta1-42 (Aβ42), amyloid-be

[Social skills training groups for children and adolescents with Asperger syndrome: A review].

BACKGROUND: First described in 1944 by Hans Asperger, Asperger syndrome (AS) is now considered in international diagnostic classifications as one of the pervasive developmental disorders (PDD) or autism spectrum disorders (ASD). The main symptoms of AS are severe impairment in social interaction and communication, and restricted interests, without significant delay in cognitive and language development. Its prevalence is not clearly established but might be around 0.26 per 1000. Symptoms of high-functioning autism (HFA), which is not an official diagnostic category, are quite similar. Children

Neighbourhood Deprivation Is Associated With Greater Mortality in Patients With Alcohol-Associated Liver Disease.

BACKGROUND AND AIM: Significant health disparities persist in alcohol-associated liver disease (ALD), largely driven by social determinants of health (SDOH). However, the influence of neighbourhood-level SDOH on ALD outcomes remains underexplored. In this study, we aimed to evaluate how neighbourhood-level SDOH are associated with disease burden, comorbid conditions, and mortality in individuals with ALD. METHODS: We conducted a retrospective cohort study of patients with ALD in the Banner Health System, representing hospitals across Arizona, California, Nevada, Wyoming and Colorado, from Jan

The AFFIRM Framework for gender-affirming care: qualitative findings from the Transgender and Gender Diverse Health Equity Study.

BACKGROUND: Transgender, nonbinary, and gender diverse (TGD) people experience stigma in healthcare settings impacting healthcare utilization, including avoidance of care due to anticipated discrimination. Gender-affirming care refers to care for medical gender affirmation, such as gender-affirming hormones and surgery, as well as general care that affirms and respects TGD patients. This study sought to explore the experiences of TGD adults to inform gender-affirming care delivery and develop an actionable framework for practice. METHODS: Between May-October 2021, one-time individual in-depth
