OBJECTIVE: Considerable overlap exists in nerve root innervation of various muscles. Knowledge of myotomal innervation is essential for the interpretation of neurological examination findings and neurosurgical decision-making. Previous studies relied on cadaveric dissections, animal studies, and cases with anomalous anatomy. This study investigates the myotomal innervation patterns of cervical and lumbar nerve roots through in vivo stimulation during surgeries for spinal decompression.
METHODS: Patients undergoing cervical and lumbar surgeries in which nerve roots were exposed in the normal c

We report a patient with Lewis-Sumner syndrome (LSS) who showed an improvement only with plasma exchange (PE). The patient, 32-yr old man, had progressive multifocal motor-sensory deficits with persistent, multiple conduction blocks and marked slowing of NCVs. Nerve pathology supported a diagnosis of demyelinating neuropathy by revealing marked loss of myelinated fibers with inter- and intrafascicular variation. Although the patient was refractory to treatment with corticosteroid and intravenous immunoglobulin, PE produced a dramatic improvement. Our experience strongly proposes that PE should

A POEMS syndrome is a rare disorder characterized by polyneuropathy, organomegaly, endocrinopathy, monoclonal gammopathy and skin abnormalities including hyperpigmentation and hypertrichosis. Here we report a 55-year-old female case of a POEMS syndrome associated with Waldenstrom macroglobulinemia. The patient had bed-bound polyneuropathy, splenomegaly, IgM-λ type monoclonal (M) protein, elevated λ-type free light chain (FLC), infiltration of CD20-positive lymphoplasmacytic cells in bone marrow, edema and hypertrichosis, and was diagnosed to have an 'atypical' POEMS syndrome associated with ma

Any injury affecting peripheral nerve continuity is followed by a degenerative reaction from nerve itself and its muscule. Nerve regeneration is possibly when the two ends are put in direct contact (direct suture), proximity (autologus or artificial grafts) or through muscle neurotization. The aim of this experimental study was to evaluate sciatic nerve degeneration and regeneration in rat after no suture (group 1), direct suture (group 2), nerve grafting (group 3) and muscle neurotization (group 4). Nerve samples were analyzed in optical microscopy at 8 and 16 weeks after surgery and nerve de

BACKGROUND: We previously reported the usefulness of neuromagnetic recordings for the diagnosis of disorders in peripheral nerves or the spinal cord. However, there have been no reports on incomplete conduction block of the spinal cord, which is clinically common in conditions such as cervical myelopathy. Here, we estimated the usefulness of measuring spinal cord evoked magnetic fields for evaluating incomplete conduction block.
METHODS: Incomplete conduction block models of the spinal cord of the rabbit were established using a Fogarty balloon catheter that was inserted into the epidural spa

OBJECT: The etiology of intraneural ganglia has been debated for centuries, and only recently a unifying theory has been proposed. The incidence of tibial nerve intraneural ganglia is restricted to the occasional case report, and there are no reported cases of these lesions in children. While evidence of the unifying theory for intraneural ganglia of the common peroneal nerve is strong, there are only a few reports describing the application of the theory in the tibial nerve. In this report the authors examine tibial nerve intraneural ganglia at the ankle and knee in an adult and a child, resp

Guillain-Barré syndrome (GBS) is an acute polyneuropathy consisting of different subtypes. Acute inflammatory demyelinating polyradiculoneuropathy, the classic demyelinating form of GBS, accounts for 90% of all GBS cases in the Western world. Acute motor axonal neuropathy (AMAN) and acute motor and sensory axonal neuropathy (AMSAN) are axonal forms of GBS that are more prevalent in Asia, South and Central America, often preceded by infection by Campylobacter jejuni. AMAN and AMSAN may be mediated by specific anti-ganglioside antibodies that inhibit transient sodium ion (Na+) channels. The effi

BACKGROUND: Different methods in performing nerve conduction studies (NCS) yield different results. The results of NCS can also be affected by factors such as gender, age, height, and weight. This study aimed (1) to survey the effects of such factors, (2) to determine the importance of these factors, and (3) to analyze them for building equations in NCS and late response studies.
METHODS: NCS from the neurological screening tests of 101 individuals without spinal cord, root, nerve, neuro-muscular junction, muscular, or systemic diseases were collected and analyzed.
RESULTS: Subjects with old

In mammals, transient exposure to hyperoxia for a period of weeks during perinatal life leads to impairment of the ventilatory response to acute hypoxia, which may persist long beyond the duration of the hyperoxia exposure. The impairment of the ventilatory response to hypoxia is due to hyperoxia-induced reduction of carotid chemoreceptor sensitivity to hypoxia. We previously demonstrated that hyperoxia exposure in rats, from birth to two weeks of age, profoundly reduced carotid chemoreceptor single axonal responses to acute hypoxia challenge. However, the time course and mechanisms of this im

To examine if the effect of temperature on neuromuscular jitter differs in myasthenics and normals, we performed single fiber electromyography (SFEMG) in 10 myasthenics and 10 healthy controls after heating the upper extremity to 37 and 42 degrees C. In each case, 10-24 mean consecutive difference (jitter) values were obtained at both temperatures. All jitters were pooled appropriately and the groups were compared with each other. The mean jitter was 33.4 +/- 10.4 micros at 37 degrees C and 28.2 +/- 11.4 micros at 42 degrees C in controls, and 48.3 +/- 18.3 micros at 37 degrees C and 54.8 +/-

To clarify the differences between the mechanisms of conduction slowing/block and accommodative processes in focal demyelinating neuropathies, this computational study presents the kinetics of the ionic, transaxonal and transmyelin currents defining the intracellular and electrotonic potentials in different segments of human motor nerve fibres. The computations use our previous double cable model of the fibres. The simulated fibres have focal demyelination of internodes, paranodes or both together. The intracellular potentials are defined mainly by the Na+ current, as the contribution of the K

A 36-year-old man had a 6-year history of progressive left hand and finger extension weakness. Extensive electrophysiological studies showed only a partial motor conduction block along the left radial nerve at the mid-arm. Several intravenous immunoglobulin treatments were clinically successful. A 3-year follow-up did not show any clinical or electrophysiological involvement of other nerves. A monofocal motor neuropathy, as a variant of multifocal motor neuropathy, was diagnosed.

BACKGROUND: We aimed to investigate the effects of two different types of mesh used in hernia repair on nerve transport and neural injury.
STUDY DESIGN: Forty-five Wistar-albino rats were randomly allocated to three groups. Basal neural transport on the index of the left sciatic nerve was performed in all groups before surgery. In the control group, only sciatic nerve manipulation was performed. Other groups received a cuff around the index sciatic nerve. The second group received a monofilament polypropylene and the third group received a polytetrafluoroethylene mesh. Effects of entrapment o

This study provides numerical simulations of some of the abnormalities in the potentials and axonal excitability indices of human motor nerve fibers in simulated cases of internodal, paranodal and simultaneously of paranodal internodal demyelinations, each of them systematic or focal. A 70% reduction of the myelin lamellae (defining internodal demyelination), or of the paranodal seal resistance (defining paranodal demyelination), or simultaneously both of them (defining paranodal internodal demyelination) was uniform along the fiber length for the systematically demyelinated subtypes.
