Enterococcus faecalis is a microorganism commonly detected in asymptomatic, persistent endodontic infections. Its prevalence in such infections ranges from 24% to 77%. This finding can be explained by various survival and virulence factors possessed by E. faecalis, including its ability to compete with other microorganisms, invade dentinal tubules, and resist nutritional deprivation. Use of good aseptic technique, increased apical preparation sizes, and inclusion of 2% chlorhexidine in combination with sodium hypochlorite are currently the most effective methods to combat E. faecalis within th

A prospective in vivo investigation was conducted to determine radiographic and clinical factors associated with contemporary nonsurgical endodontic retreatments. Approximately 1,100 failing endodontically treated teeth were screened to determine an appropriate treatment plan. Using magnification, 337 consecutive retreatment cases were evaluated and treated. The vast majority of the retreated cases involved multiple factors. Eighty-five percent of the cases presented with periradicular radiolucencies. Sixty-five percent of the cases demonstrated poor radiographic obturation quality. Associated

A necrotic immature mandibular second premolar with periapical involvement in a 13-year-old patient was treated. Instead of the standard root canal treatment protocol and apexification, antimicrobial agents were used in the canal, after which the canal was left empty. Radiographic examination showed the start of apical closure 5 months after the completion of the antimicrobial protocol. Thickening of the canal wall and complete apical closure was confirmed 30 months after the treatment, indicating the revascularization potential of a young permanent tooth pulp into a bacteria-free root canal s

Experimentally-induced external inflammatory tooth-root resorption can be inhibited by therapeutic doses of calcitonin. Such doses can be delivered by an intrinsically slow diffusion pathway, from a reservoir in endodontically-debrided root canals, via the dentinal tubules. While the kinetics of this journey have been followed in an earlier report, the binding characteristics of calcitonin to the tooth mineral, which will be responsible, in part, for these kinetics, have not been reported before. The current study examines the binding potential of calcitonin to root mineral and addresses the p

Biochemically, there is usually much less elastase activity in gingival tissue than in crevicular fluid. The tissue distributions of active and inactive elastase and the endogenous inhibitors alpha-1-proteinase inhibitor (alpha 1PI) and alpha-2-macroglobulin (alpha 2M) were therefore compared. Inflamed tissue was obtained from chronic periodontitis patients, and cryostat sections were incubated with the histochemical elastase substrate MeOSuc-Ala-Ala-Pro-Val-MNA. Adjacent sections were examined immunocytochemically with antibodies to neutrophil elastase, alpha 1PI, alpha 2M, and leukocyte diff

Apical periodontitis (AP) is characterized by progressive osteoclast-mediated bone destruction. Although Nell-1 may inhibit osteoclast differentiation, the underlying mechanisms and its specific role in AP remain unclear. This study aims to determine the inhibitory effects of Nell-1 on osteoclast differentiation and bone resorption in apical periodontitis. It further explores whether Nell-1 exerts its function through activation of the Wnt/β-catenin signaling pathway. RAW264.7 cells were transfected with Nell-1 to evaluate osteoclast differentiation through TRAP staining, qRT-PCR, and Western

PURPOSE: The present case report aimed to present an immunohistological analysis of newly formed intracanal tissues in a traumatised immature permanent incisor with pulp necrosis and apical periodontitis following RET.
CASE DESCRIPTION: A 7-year-old girl sustained a combined traumatic dental injury, an uncomplicated crown fracture and lateral luxation, affecting an immature maxillary central incisor. The tooth subsequently developed pulpal necrosis and apical periodontitis and was treated with RET using calcium hydroxide as an intracanal medicament. A blood clot served as the scaffold, follow

Secondary/persistent endodontic infections (SPEIs) result from failed root canal therapy, causing persistent apical periodontitis. Current diagnostic methods for SPEIs predominantly rely on clinical and radiographic indicators, which often lack adequate sensitivity and specificity. Consequently, there is an urgent need to effectively detect SPEIs or monitor their progression. The aim of this study was to compare and characterize the microbiota of root canals and gingival sulci of teeth affected by SPEI to identify keystone pathogens as potential diagnostic biomarkers through advanced next-gene

INTRODUCTION: Postoperative pain remains a critical concern in endodontic treatment, influenced by multiple factors, including the choice of root canal sealer. Calcium silicate-based sealers like NeoSealerFlo offer biocompatibility and bioactivity, while resin-based sealers such as AH Plus are widely regarded as the gold standard in endodontics. This study aimed to compare postoperative pain and sealer extrusion between these two materials in a large-scale clinical setting.
METHODS: In this multicenter randomized clinical trial, 564 root canal-treated teeth were obturated with either NeoSeale

BACKGROUND: The main issues faced during the treatment of apical periodontitis are the management of bacterial infection and the facilitation of the repair of alveolar bone defects to shorten disease duration. Conventional root canal irrigants are limited in their efficacy and are associated with several side effects. This study introduces a synergistic therapy based on nitric oxide (NO) and antimicrobial photodynamic therapy (aPDT) for the treatment of apical periodontitis.
RESULTS: This research developed a multifunctional nanoparticle, CGP, utilizing guanidinylated poly (ethylene glycol)-p

AIM: The influence of hypercholesterolemia on the development of apical periodontitis (AP) is inconclusive. Recent studies revealed that cholesterol metabolite 27-hydoxycholesterol (27HC) can affect cellular responses to bacterial infections and oestrogen status and raloxifene may influence its action. Herein, we aimed to examine the impact of 27HC on production of inflammatory mediators by macrophages and the regulatory function of raloxifene. The contribution of 27HC to AP development and the therapeutic effect of raloxifene were evaluated in a rat model.
METHODS: Murine macrophages J774 ce

INTRODUCTION: Regeneration of the pulp-dentin complex is the penultimate goal of regenerative endodontic procedures (REPs). Histological outcomes have demonstrated reparative tissue formation in human teeth extracted post-REPs. However, lack of accurate characterization has precluded identification of the true nature of tissues formed post-REP.
METHODS: Here, we present 2 case reports of tooth #29 and #9 treated with REPs and demonstrate their clinical, radiographic, and histological outcomes.
RESULTS: Clinical outcomes revealed healing of apical periodontitis in both teeth and re-establishm

BACKGROUND: The purpose of the present study was to evaluate if the presence of granulation tissue in asymptomatic apical periodontitis compromised immediate implant placement.
METHODS: Patients requiring extraction of one tooth (maxillary and mandibular incisive, canine or premolar) with asymptomatic apical periodontitis, were recruited for this prospective study. They were randomly scheduled into two groups: in first group (A) including 30 teeth, reactive soft tissue was debrided before implant placement, and in second group (B) including 30 teeth, reactive soft tissue was left in the apica

INTRODUCTION: Although studies have recently shown that osteocytes embedded in mineralized bone matrix play an important role in bone diseases, the participation of cementocytes in apical periodontitis has not been evaluated. The aim of the present study was to evaluate the possible involvement of cementocytes in the development of apical periodontitis.
METHODS: Apical periodontitis was experimentally induced in the lower first molars of wild-type mice by pulp exposure to the oral environment. At 0, 7, 21, and 42 days after pulp infection, the animals were euthanized, and the jaws were prepar
