Auditory Effects of Acoustic Noise From 3-T Brain MRI in Neonates With Hearing Protection.

BACKGROUND: Neonates with immature auditory function (eg, weak/absent middle ear muscle reflex) could conceivably be vulnerable to noise-induced hearing loss; however, it is unclear if neonates show evidence of hearing loss following MRI acoustic noise exposure. PURPOSE: To explore the auditory effects of MRI acoustic noise in neonates. STUDY TYPE: Prospective. SUBJECTS: Two independent cohorts of neonates (N = 19 and N = 18; mean gestational-age, 38.75 ± 2.18 and 39.01 ± 1.83 weeks). FIELD STRENGTH/SEQUENCE: T1-weighted three-dimensional gradient-echo sequence, T2-weighted fast spin-echo

Monitoring anesthesia depth with patient state index during pediatric surgery.

BACKGROUND: Monitoring anesthesia depth in children is challenging. Pediatric anesthesiologists estimate general anesthesia depth using indirect methods such as pharmacokinetic models and neurovegetative reflexes. The application of processed electroencephalography may help to identify the correct anesthesia depth (i.e., patient state index between 25 and 50). AIMS: To determine the median values of patient state index and spectral edge frequency 95% in children undergoing general anesthesia conducted according to indirect evaluation of depth. The relationships between patient state index and

Case Report: Bilateral pes planovalgus: an uncommon pediatric orthopedic presentation of Charcot-Marie-Tooth disease type 4B1.

Charcot-Marie-Tooth (CMT) disease is an inherited motor and sensory neuropathy that typically presents with cavovarus foot deformity. Pes planovalgus is uncommon in CMT and, while noted incidentally in multicenter cohort studies of CMT4B subtypes, has not been described as the predominant orthopedic manifestation in a published case report of genetically confirmed CMT4B1. We report the case of a 7-year-old girl who presented with progressive painful bilateral pes planovalgus, hallux valgus, frequent falls, and gait imbalance, despite otherwise normal developmental milestones. Neurological exam

Unusual Presentation of Spinal Osteoid Osteoma: A Case Report.

INTRODUCTION: Osteoid osteoma (OO) is a benign osteogenic tumor that most commonly affects long bones, accounting for 2-3% of primary bone tumors. Spinal involvement is relatively rare, representing 6-20% of cases, with the lumbar spine being the most frequently affected region. Typical spinal OO presents with painful scoliosis and nocturnal pain relieved by non-steroidal anti-inflammatory drugs (NSAIDs). However, atypical presentations may occur when lesions are located adjacent to neural structures, leading to radicular symptoms and poor NSAID response. This case report describes an unusual

Marcus Gunn jaw-winking ptosis and contralateral lid retraction: management with Müller muscle-conjunctival resection with tarsectomy.

PURPOSE: Marcus Gunn jaw-winking syndrome (MGJWS) is a rare congenital cranial dysinnervation disorder characterized by synkinetic eyelid elevation during jaw movement. Standard surgical management often involves levator excision with bilateral frontalis suspension, which abolishes levator function but carries morbidity and variable cosmetic outcomes. Posterior ptosis approaches such as Müller muscle-conjunctival resection (MMCR) are well established for phenylephrine-responsive ptosis, but their role in MGJWS remains unclear. METHODS: A 12-year-old male with congenital left upper eyelid ptos

Return of spontaneous circulation after cardiopulmonary arrest in an adult horse recovering from anaesthesia.

This report describes successful cardiopulmonary resuscitation of a healthy, 525 kg, 8 years 9 months-old Quarter horse gelding. After successful pelvic limb orthopaedic surgery to excise a metatarsal bone under general anaesthesia, with the horse in left lateral recumbency, the horse was hoisted into recovery. After the horse was positioned in recovery, a routine cardiac check by thoracic auscultation was performed. Cardiopulmonary arrest (CPA) was identified by absence of cardiac sounds and pulse, as evaluated by digital palpation of the facial artery, absence of the right palpebral reflex,

Impaired cortical autonomic responses during sympathetic activation in neurogenic orthostatic hypotension characterized by postganglionic autonomic dysfunction.

Neurogenic orthostatic hypotension (NOH) is a cardinal feature of autonomic dysfunction. The cortical autonomic network (CAN) is a network of brain regions associated with autonomic function. Therefore, our objective was to investigate whether impairment of CAN structures is involved in the pathophysiology of NOH. Fifteen controls (63 ± 13 yr) and 15 NOH patients (67 ± 6 yr; P = 0.2) with peripheral autonomic dysfunction completed standard tests of parasympathetic [deep breathing (DB)] and sympathetic [Valsalva maneuver (VM)] activation during a functional MRI. Blood-oxygen-level dependent (BO

[A case of acute sensory neuropathy associated with cytomegalovirus infection].

We report a non-compromised patient with acute sensory neuropathy (ASN) developed following cytomegalovirus (CMV) hepatitis. A 67-year-old man admitted to a hospital because of acute hepatic dysfunction accompanying fever and skin eruption. One month later, when hepatic function normalized, numbness and clumsiness started acutely first in the right upper limb next to all the extremities. He found difficulty in walking in a couple of weeks. One month after the commencement of neurological illness, he was referred to us. On examination, he had sensory limb ataxia. His gait was wide-based, and Ro

[A 54-year-old man with action myoclonus, parkinsonism and epilepsy].

We describe 3-year clinical course of a 54-year-old Japanese man who presented with action myoclonus, parkinsonism and epilepsy. There was no family history or consanguinity. The patient was well until the age of 51 years (in 1986), when he noted slow movements, memory disturbance and left hand tremor. He was treated with anti-Parkinson drugs without any improvements. Soon thereafter, he developed a gait disturbance and generalized tonic clonic seizures. He was admitted to our service at the age of 53 years. General physical examination revealed no hepatosplenomegaly. Neurological examination

Circulatory response evoked by a 3 s bout of dynamic leg exercise in humans.

1. The mechanisms underlying the pronounced transient fall in arterial blood pressure evoked by a 3 s bout of bicycle exercise were investigated in twenty healthy young adults and four patients with hypoadrenergic orthostatic hypotension. 2. In healthy subjects a 3 s bout of upright cycling induced a 28 +/- 3 mmHg fall in mean arterial pressure at 12 s. The fall in mean arterial pressure was preceded by a 12 +/- 2 mmHg rise in right atrial pressure at 3 s and accompanied by a 54 +/- 7% increase in left ventricle stroke volume at 6 s. Systemic vascular resistance dropped 48 +/- 2% at 7 s after

The effect of discrete stimulation of carotid body chemoreceptors on atrial natriuretic peptide in anaesthetized dogs.

1. In seven chloralose-anaesthetized and artificially ventilated beagles, the carotid sinus regions were vascularly isolated and perfused with either arterial or mixed (arterial and venous) blood (PO2 46.4 +/- 1.5 mmHg, mean +/- S.E.M.) to stimulate the chemoreceptors at constant flow and pressure. Cervical vagosympathetic trunks were ligated in all dogs, and gallamine triethiodide (2.0 mg kg-1 h-1, I.V.) was given in five dogs. Right atrial pressure was measured in all dogs, and left atrial pressure in four dogs. Mean aortic pressure was held constant (91.0 +/- 3.0 mmHg) by means of a reservo

Pattern of cardiorespiratory afferent convergence to solitary tract neurons driven by pulmonary vagal C-fiber stimulation in the mouse.

The central integration of signals from pulmonary vagal C-fibers (or type-J receptors) with those arising from cardiac, peripheral chemoreceptor, and baroreceptor afferents to neurons within the nucleus of the solitary tract (NTS) was studied in an arterially perfused working heart-brain stem preparation of adult mouse. Pulmonary vagal C-fibers were excited by right atrial injection of phenylbiguanide (PBG) while cardiac receptors were stimulated by left ventricular injection of veratridine (1-3 micrograms/kg) or mechanically by distension of the left ventricle (20-50 microl perfusate) using a

"An urge to take action": Motivational pathways of crisis-activated social entrepreneurs.

This study examines the motivations underlying the rapid emergence of social entrepreneurship in Israel following the October 7, 2023, Hamas attack and the ensuing war. While formal emergency services and volunteer organizations played important roles, many immediate responses were led by civilians who independently created new ventures to address urgent unmet needs. The study explored how these grassroots actors led community-based initiatives amid national trauma and institutional gaps. Drawing on interviews with social entrepreneurs, reflexive thematic analysis identified a dynamic interpla

Building Toward Better Housing for Canadians With Fetal Alcohol Spectrum Disorder.

BACKGROUND: Individuals with disabilities have a right to live independently, be included within their communities, and have adequate standards of living and social protections. However, individuals with fetal alcohol spectrum disorder (FASD) experience elevated obstacles to obtaining and maintaining housing. In addition to brain-based differences that can create barriers to successful navigation of daily demands, many individuals with FASD experience mental health concerns, substance use, or trauma, and the experience of being houseless is, in itself, traumatic. These factors often compound,

Effects of Long-Term Contact Lens Use on Vestibulo-Ocular Reflex Using Rotary Chair and Video Head Impulse Testing.

PURPOSE: This study investigated whether long-term contact lens wear influences vestibulo-ocular reflex (VOR) gain and suppression across a range of stimulus frequencies. Despite frequent use of contact lenses in clinical populations, it remains unclear how optical correction affects VOR assessment outcomes. METHOD: Twelve myopic adults (four men, eight women; 21-25 years) who wore contact lenses at least 12 hr per day, 5 days per week, participated. VOR gain and suppression were measured using sinusoidal harmonic acceleration (SHA) rotary chair testing at 0.04-0.64 Hz, and VOR gain at high f

[Chronic Cough].

Here we summarize the most important new scientific developments in the field of chronic cough (CC). Many have been already considered in the new German Cough Guidelines published 2025. We explain the difference between chronic cough on the one hand as a symptom of almost all respiratory and other diseases and on the other, cough as a disease on its own right: refractory chronic cough and unexplained chronic cough (RCC/UCC). In the German version of ICD 10 a new code was introduced for these entities: U69.6. Every patient with CC should undergo a basic diagnostic procedure including history, p

Metformin-Associated Functional Vitamin B12 Deficiency Presenting as Subacute Combined Degeneration in a 57-Year-Old Man With Diabetes Mellitus.

BACKGROUND Functional vitamin B12 deficiency, also known as metformin-associated cobalamin deficiency, can occur in patients receiving metformin due to impaired gastrointestinal absorption of vitamin B12. This condition can coexist with diabetic neuropathy and may result in impaired myelin formation in the spinal cord and peripheral nerves, leading to subacute combined degeneration of the spinal cord. This report describes a patient with diabetes mellitus and metformin-associated functional vitamin B12 deficiency who presented with subacute combined degeneration of the spinal cord and gastroin
