Initial experience with a novel robotic surgical system in abdominal surgery.

The emergence of robotics in surgery has led to a recent boom in the acceptance of such technology. This technology has been rapidly adopted in various fields, with the most prominent being general surgery. The number of abdominal procedures being performed using robotics has increased to around 15.1% as potential advantages continue to be highlighted. We herein report the use of the Versius surgical robotic system for use in common abdominal procedures. Three experienced robotic surgeons first gained experience and became familiar with the robotic system through both online modules and cadave…

Robotic repair of perineal hernias: a video vignette and review of the literature.

BACKGROUND: Perineal hernias can be secondarily acquired following abdominoperineal resection of the rectum. While transabdominal minimally invasive techniques have traditionally used laparoscopy, there are few studies published on the robotic platform, which has been gaining popularity for other types of hernia repairs. We review the existing literature, share a video vignette, and provide practical tips for surgeons interested in adopting this approach. METHODS: A literature search in Pubmed was performed to include all articles in English describing robotic repair of perineal hernias with

Robotic lateral pancreatico-jejunostomy for chronic pancreatitis Puestow procedure.

Chronic pancreatitis is a benign inflammatory disorder, which can lead to severe and persistent symptoms. Patients with recurrent episodes of abdominal pain often require endoscopic treatments, which are frequently associated with inadequate symptom relief. In selected patients, surgical treatment allows for a greater long-term pain reduction. As one of the first teams reporting this approach, we present a didactical video with a stepwise technique to perform a robotic Puestow procedure. This technical report details a robotic approach to complete a latero-lateral pancreatojejunostomy in patient

Robotic transanal minimally invasive surgery: a single institutional experience.

Robotic transanal minimally invasive surgery (R-TAMIS) is a novel and evolving technique with limited reported outcomes in the literature. Compared to the laparoscopic approach, R-TAMIS provides enhanced optics, increased degrees of motion, superior ergonomics, and easier maneuverability in the confines of the rectum. We report a single institution experience at a large quaternary referral academic medical center with R-TAMIS using the da Vinci Xi® platform. This is a retrospective review of electronic medical records at the Mayo Clinic from September 2017 to April 2020. It includes all availa

Learning garment manipulation policies toward robot-assisted dressing.

Assistive robots have the potential to support people with disabilities in a variety of activities of daily living, such as dressing. People who have completely lost their upper limb movement functionality may benefit from robot-assisted dressing, which involves complex deformable garment manipulation. Here, we report a dressing pipeline intended for these people and experimentally validate it on a medical training manikin. The pipeline is composed of the robot grasping a hospital gown hung on a rail, fully unfolding the gown, navigating around a bed, and lifting up the user's arms in sequence

The diffusion of robotic surgery: Examining technology use in the English NHS.

This paper examines the adoption and diffusion of medical technology as associated with the dramatic recent increase in the surgical use of robots. We consider specifically the sequential adoption and diffusion patterns of three interrelated surgical technologies within a single healthcare system (the English NHS): robotic, laparoscopic and open radical prostatectomy. Robotic and laparoscopic techniques are minimally invasive procedures with similar patient benefits, but the newer robotic technique requires a high initial investment cost to purchase the robot and carries high maintenance costs

Identifying Mild Cognitive Impairment by Using Human-Robot Interactions.

BACKGROUND: Mild cognitive impairment (MCI), which is common in older adults, is a risk factor for dementia. Rapidly growing health care demand associated with global population aging has spurred the development of new digital tools for the assessment of cognitive performance in older adults. OBJECTIVE: To overcome methodological drawbacks of previous studies (e.g., use of potentially imprecise screening tools that fail to include patients with MCI), this study investigated the feasibility of assessing multiple cognitive functions in older adults with and without MCI by using a social robot.

Robotic-assisted extraction of giant gastric trichobezoar: a case report.

Trichobezoar is a rare condition, almost exclusively seen in young females with certain psychiatric disorders. Trichobezoars are usually confined within the stomach and the complications include ulceration, perforation, intussusception and obstruction for which surgery is usually required. Most of the reported cases of giant gastric trichobezoar extraction underwent an exploratory laparotomy with only a few reported cases that underwent a successful laparoscopic approach. This case report details the surgical management of the first case of a giant obstructing gastric trichobezoar extraction u

Robotic esophagectomy and 3-field lymphadenectomy with intraoperative nerve monitoring.

The standard of care for esophageal malignancies has evolved over the years from open transthoracic esophagectomy to a minimally invasive approach due to the reduction in surgical trauma and significant impact on postoperative outcomes. Minimally invasive approaches include video-assisted thoracoscopic surgery and robot-assisted thoracoscopic surgery. These minimally invasive approaches have an attendant learning curve that early-career surgeons are required to negotiate before achieving proficiency in the procedure. Recurrent laryngeal nerve injury is a particularly significant problem, espec

Sensor-Driven Human-Robot Synergy: A Systems Engineering Approach.

Knowledge-based synergistic automation is a potential intermediate option between the opposite extremes of manual and fully automated robotic labor in agriculture. Disruptive information and communication technologies (ICT) and sophisticated solutions for human-robot interaction (HRI) endow a skilled farmer with enhanced capabilities to perform agricultural tasks more efficiently and productively. This research aspires to apply systems engineering principles to assess the design of a conceptual human-robot synergistic platform enabled by a sensor-driven ICT sub-system. In particular, this pape

A Case of Robotic Posterior Rectopexy for Full-thickness Rectal Prolapse.

We describe our experience with robotic posterior rectopexy for a patient with full-thickness rectal prolapse. To our knowledge, this is the first report of such a case in the literature. A 94-year-old woman presented with a history of gradually worsening rectal prolapse. On examination, we found that the rectum was completely prolapsed, and we observed a prolapsed intestinal tract. Surgery was indicated and robotic rectopexy was performed without intraoperative complications. The postoperative course was uneventful, and she was discharged 10 days after the operation. One year later, there wer

Simple Height Reduction of the Posterior Leaflet in Robotic Mitral Annuloplasty.

Systolic anterior motion is occasionally encountered during mitral valve repair using neochordal reconstruction for mitral regurgitation owing to large posterior leaflet prolapse. In cases with a redundant posterior leaflet, the risk of systolic anterior motion increases if the redundant posterior leaflet is unresected to decrease the leaflet height. The posterior leaflet was fixed at the anterior annulus, cut near the posterior annulus, and resected into a spindle shape. The excised part of the posterior leaflet was subsequently closed with a continuous suture. Herein, we report our simple he

Robot-Assisted Surgery of an Iliac Artery Aneurysm: A Case Report.

Robot-assisted surgery has not yet been able to establish itself for vascular surgery. However, the preconditions for robot-assisted vascular interventions have changed fundamentally over the past years because of technological advances and extensive experience in other surgical disciplines. Hence, we describe a robot-assisted repair of an iliac artery aneurysm using a late-generation robotic platform. A 63-year-old male patient was diagnosed with an asymptomatic 30 mm aneurysm of the right common iliac artery. The operation was performed with the Da Vinci Xi system (Intuitive Surgical, Inc.,

Robotic Platform for Horticulture: Assessment Methodology and Increasing the Level of Autonomy.

The relevance of the study is confirmed by the rapid development of automation in agriculture, in particular, horticulture; the lack of methodological developments to assess the effectiveness of the introduction of robotic technologies; and the need to expand the functionality of mobile robots. The purpose of the study was to increase the level of autonomy of a robotic platform for picking apple fruits based on a new method, develop a system of factors to determine the effectiveness of the introduction of robots in horticulture, and develop a control system using integrated processing of onboa

Multi-Pesticide Residue Analysis Method Designed for the Robot Experimenters.

Robots replacing humans as the executioners is crucial work for intelligent multi-pesticide residue analysis to maximize reproducibility and throughput while minimizing the expertise required to perform the entire process. Traditional analysis methods are predicated on manual execution, so we configured our robot experimenter, automated the analytical workflow, and achieved the goal of robotics execution. Our robot experimenter with an X-Y-Z axis robotic arm was interfaced with seven modules and ultra-performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS) for automated standar…

The Use of Peripheral Nerve Blockade in Laparoscopic and Robotic Surgery: Is There a Benefit?

PURPOSE OF REVIEW: The purpose of this review is to synthesize recent literature investigating the use of regional anesthesia for minimally invasive surgery. RECENT FINDINGS: Recent studies investigating the use of newer peripheral nerve blocks such as erector spinae plane (ESP) and quadratus lumborum (QL) block are very limited. Evidence supporting the use of peripheral nerve blockade in laparoscopic or robotic surgery is very limited and of low-moderate quality. While transverse abdominal plane (TAP) block may decrease opioid and pain scores after laparoscopic cholecystectomy, bariatric sur
