Wrist kinematic indices and scapholunate distance play an essential role in diagnosing and treating injury and disease of the carpus. It can be challenging to measure and diagnose scapholunate ligament injury in the immature skeleton. The aim of this study was to measure the normal ranges of the carpal indices on radiographs in the Turkish population aged 5-14 years. This retrospective study examined children aged 5-14 years between 2021 and 2022. Children aged 5-14 years with a wrist anteroposterior X-ray, children admitted to the hospital for nontraumatic reasons, and children who had not ha

A model for multiple diagnostic tests, applied repeatedly over time on each subject, is proposed; gold standard data are not required. The model is identifiable with as few as three tests, and correlation among tests at each time point in the diseased and nondiseased populations, as well as across time points, is explicitly included. An efficient Markov chain Monte Carlo scheme allows for straightforward posterior inference; sample R code is available in the Supporting Web Materials for this paper. The proposed model is broadly illustrated via simulations and an analysis of scaphoid fracture d

BACKGROUND: The aim of this study was to assess the inter observer and intra observer reliability of acute scaphoid fracture classification methods including a novel 'long axis' measurement, a simple method which we have developed with the aim of improving agreement when describing acute fractures.
METHODS: We identified sixty patients with acute scaphoid fractures at two centres who had been investigated with both plain radiographs and a CT (Computed Tomography) scan within 4 weeks of injury. The fractures were assessed by three observers at each centre using three commonly used classificati

BACKGROUND: Headless screw is frequently used for scaphoid fracture fixation. Aim of the study was to assess the correlation between the axial length of the scaphoid and the axial length of the middle phalanx of index, middle, ring and little finger so as to provide an indirect method to assess the length of the scaphoid and thereby the length of the screw.
METHODS: Thirty five fresh frozen cadavers with seventy wrists and hands were dissected. The age, sex and side were recorded. The axial length of the scaphoid, axial length of the middle phalanx of index, middle, ring and little finger wer

PURPOSE OF THE STUDY: Scapholunate (SL) ligament disruption frequently occurs together with distal radius fractures. If it is not diagnosed and treated, instability develops and will eventually be manifested as arthritic lesions known as scapholunate advanced collapse (SLAC) of the wrist. The aim of this study was to record the occurrence of SL ligament disruption in the patients treated conservatively for displaced fractures of the distal radius and to find out which fracture types are most often associated with injury to the SL ligament.
MATERIAL AND METHODS: A total of 75 patients treated

OBJECTIVE: To report the effect of Russe technique in treating old scaphoid fracture.
METHODS: From January 1987 to February 1999, 11 cases of old scaphoid fractures were treated with Russe technique. The follow-up period averaged 5 years and 7 months.
RESULTS: The fracture healing rate was 100%, and wrist pain was completely relieved in all cases. The ranges of motion averaged 41.2 degrees of flexion, 40.5 degrees of extension, 8.7 degrees of radial deviation, and 15.4 degrees of ulnar deviation. The grip strength reached 82.9% of the normal side.
CONCLUSION: Russe technique is an effectiv

Salvage of the ununited scaphoid may not always be feasible or advisable, hence the availability of a number of alternative techniques which prioritize restoration of pain relief and range of motion. The role of intercarpal fusion reflects the importance of midcarpal stabilization when the scaphoid is resected. While scaphocapitate and scaphocapitolunate fusions have been described, these techniques provide less pain relief than midcarpal fusion with scaphoid excision, and are associated with an increased risk of radioscaphoid arthritis. In this article, the author will address the rationale f

We have used the principles of extracorporeal shock wave therapy (ESWT) in the treatment of nonunion of fractures in 44 patients (49 bones). There were 35 males and 9 females with a mean age of 34 years(range14-70). Clinical and radiological assessment was performed at regular time intervals with a minimum follow up of 18 months. Most common sites involved were the femur and tibia. The average time from initial fracture treatment to intervention with ESWT was 11.9 months (6 months to 5 years). Thirty eight non-union sites had one session of treatment and the rest (11) had more than one session.

INTRODUCTION: Avascular necrosis (AVN) of the bones of the wrist most commonly involves the lunate followed by the proximal pole of the scaphoid and the capitate. Trapezium avascular necrosis is extremely rare with only two cases reported in the literature, both of which were treated surgically. In this article, we report a unique case of trapezium avascular necrosis treated conservatively.
CASE PRESENTATION: A 38-year-old man complaining of a 4-month history of mild pain on the base of his right thumb. MRI scan was performed. The clinical presentation and the imaging findings indicated avasc

This paper reports on pilot laboratory experiments with a recently proposed surgical procedure for percutaneous screw insertion into fractured scaphoid bones using ultrasound guidance. The experiments were intended to determine the operator variability of the procedure and its performance in comparison with a traditional pinning procedure using fluoroscopy. In the proposed procedure, a three-dimensional surface model is created from pre-operative computed tomography images and intra-operatively registered to the patient using ultrasound images. A graphical interface that communicates with an o…

Isolated capitate fractures that result in the development of aseptic necrosis of the proximal pole are uncommon. Five patients were treated who developed aseptic necrosis of the proximal pole. Four had a history of trauma with radiographic evidence of nonunion. In vitro arterial injection studies were done to correlate clinical problem with intraosseous capitate vascularity. Palmar vessels were found to contribute the majority of the blood supply to the capitate. Three patterns of intraosseous blood supply were seen. In each pattern, the proximal pole received its blood supply exclusively in

Twelve patients had limited arthrodesis between the scaphoid and the lunate for chronic static scapholunate dissociation using internal plate osteosynthesis. The median time between the injury and surgery was 50 months (range 9-180). They were followed up for a year postoperatively. Preoperative symptoms were pain, functional impairment, and restricted movement. During operation the scapholunate interosseous ligament was completely torn and the scaphoid malrotated in all patients. The range of motion was measured preoperatively and postoperatively, and the unaffected side used for control. For

We present the case of a 14-year-old patient who suffered fracture dislocation of the pisiform bone (PB) along with fractures of the scaphoid, proximal radius, and proximal phalanx of the thumb due to high-energy trauma directly to the extended wrist. This combination of fractures has not been previously reported in the literature. Currently, there is no consensus in the literature regarding the optimal treatment approach for such cases. In our management, initial attempts at closed and open reduction were unsuccessful, leading to the decision for primary pisiformectomy. Our report includes a

A 12-year-old boy was tackled mid-air, resulting in a fall on the outstretched hand without fracture. The patient was treated conservatively but developed sharp pain and stiffness 6 months later. Imaging revealed distal radius avascular necrosis with physeal involvement. Owing to the injury chronicity and location, we treated the patient conservatively with hand therapy. After 1 year of therapy, the patient returned to normal activities without pain and with a resolution of findings on imaging. Avascular necrosis is more frequent in carpal bones (eg, Kienböck disease of the lunate and Preiser

We present the case of a young patient with a severely comminuted, malunited, intra-articular distal radius fracture and complete disruption of the sigmoid notch. We reconstructed the malunited distal radioulnar joint by osteotomy and repositioning the displaced sigmoid notch fragments through a combined dorsal and volar approach. At the same time, we carried out a radioscapholunate arthrodesis with distal scaphoid excision. We used a free vascularized corticoperiosteal flap from the medial femoral condyle to span the massive bone defect in the radius to obtain union. At the 2.5-year follow-up

With the development of better and smaller equipment, arthroscopy of the wrist offers the same benefits achievable with arthroscopy of the knee, shoulder, or elbow - not only diagnostic information but also a therapeutic option. Standardized techniques of performing wrist arthroscopy have been developed to evaluate the treat various wrist disorders, such as lesions of the triangular fibrocartilage complex, intra-articular distal radius fractures, and scaphoid fractures. Arthroscopy is now performed in the treatment of dorsal-wrist ganglion cysts and interosseous ligament disruptions, as well a

Rupture of the scapholunate interosseous ligament can cause the dissociation of scaphoid and lunate bones, resulting in impaired wrist function. Current treatments (e.g., tendon-based surgical reconstruction, screw-based fixation, fusion, or carpectomy) may restore wrist stability, but do not address regeneration of the ruptured ligament, and may result in wrist functional limitations and osteoarthritis. Recently a novel multiphasic bone-ligament-bone scaffold was proposed, which aims to reconstruct the ruptured ligament, and which can be 3D-printed using medical-grade polycaprolactone. This s
