Retrosigmoid versus translabyrinthine approach to acoustic neuroma resection: A comparative cost-effectiveness analysis.

OBJECTIVES/HYPOTHESIS: Approach-specific economic data of acoustic neuroma (AN) resection is lacking. The purpose of this study was to analyze and compare adjusted total hospital costs, hospital and intensive care unit (ICU) length of stay (LOS), and associated factors in AN patients undergoing resection by translabyrinthine (TL) approach versus retrosigmoid (RS) approach. STUDY DESIGN: Retrospective chart review. METHODS: A total of 113 patients with AN undergoing TL (N = 43) or RS (N = 70) surgical resection between 1999 and 2012 were analyzed. Data including age, health status, preoperati

Improvement in autophony symptoms after superior canal dehiscence repair.

OBJECTIVE: Autophony, or the unusually loud or disturbing sound of a patient's own voice, can be a prominent and disabling symptom of superior canal dehiscence (SCD) syndrome. The current study measures autophony symptoms before and after SCD plugging to quantify the benefits of surgery. STUDY DESIGN: Patients undergoing SCD plugging between September 2007 and October 2008 completed a questionnaire before and 3 months after surgery. The questionnaire consisted of 26 statements to assess the disability caused by the sound of the patient's own voice. Patients graded each item on a scale from 0

[Modeling vestibulo-ocular reflexes in everyday activities].

Fundamental data for the establishment of a new clinical testing procedure for patients suffering from equilibrium disorders were developed. A mathematical model that predicts vestibulo-ocular reflex responses (eye velocities) to three-dimensional head movements was investigated. An experimental set-up permitting the simultaneous recording of the head and eye movements was developed to investigate the vestibulo-ocular reflex (VOR) during everyday activities. The test results show that computer simulation of the VOR occurring during complex movements is indeed possible. The model was able to pr

Corticofugal projections to the vestibular nuclei in squirrel monkeys: further evidence of multiple cortical vestibular fields.

Single- and multiple-unit recordings were made from nerve cells located in the different nuclei of the brainstem vestibular nuclear complex (VNC) of anaesthetized squirrel monkeys (Saimiri sciureus) by conventional stereotaxic techniques. After neurons responding to semicircular canal stimulation in a yaw, roll, or pitch direction or to otholith stimulation were identified, small amounts of retrograde tracer substances were deposited at the recording sites. Up to three different tracers were administered to different parts of the VNC in the same animal (Fast Blue, HRP-WGA, and Rhodamine-dextra

Histopathological findings in the inner ear caused by measles.

An otopathological analysis of three cases of viral labyrinthitis was performed. Six temporal bones cut in serial sections were available for this study. According to the degree of degenerative changes in various parts of the inner ear two types of morphologically distinct labyrinthitis after measles are presented: the first one with the port of entrance through the internal auditory meatus and characterized by, first and most significantly, changes in the spiral ganglion cells; and the second, as described previously by Lindsay, with the port of entrance of the virus in the inner ear through

Two cases of Tullio phenomenon in which oval and round window reinforcement surgery was effective.

AIM: To report the effect of oval and round window reinforcement surgery performed in two patients with the Tullio phenomenon. Case 1: A male with bilateral superior canal dehiscence syndrome. Downbeat nystagmus with leftward horizontal eye movement was recorded in an electronystagmogram using a pure-tone sound of 110dB at 2000 or 4000Hz in the right ear. Case 2: A female who had undergone stapes surgery. Computed tomography revealed an ossicular prosthesis in the vestibule. An audiogram indicated mild to moderate hearing impairment in the right ear. Leftward horizontal nystagmus was recorded

X-ray microtomography study of otic capsule deficiencies: three-dimensional modelling of the fissula ante fenestram.

BACKGROUND: The postulated sites of perilymph fistulae involve otic capsule deficiencies, in particular, at the fissula ante fenestram. Histological studies have revealed this to be a channel extending from the middle ear, and becoming continuous with the inner ear medial to the anterior limit of the oval window. The relationship between a patent fissula and symptoms of perilymph fistula is contentious. OBJECTIVE: The understanding of the anatomy of the fissula ante fenestram is incomplete. Histopathology is inherently destructive to the delicate ultrastructure of the middle and inner ear. Co

Clinical testing of otolith function.

The subjective visual horizontal and vestibular-evoked myogenic potentials are simple, robust, and reproducible tests of otolith dysfunction that can prove clinically useful diagnostic information in patients with vertigo and other balance disorders. While they appear to have high specificity for unilateral otolith dysfunction, further clinical research will be required to establish their sensitivity.

Hearing results following modified radical versus canal-up mastoidectomy.

Modified radical mastoidectomy (MRM) provides relatively safe surgical access for the removal of chronic middle ear and mastoid disease and gives reproducible results. However, it had been suggested that hearing may not be as good as that after "intact canal wall mastoidectomy" (ICWM). This paper reviews 153 tertiary referrals suffering from extensive disease who underwent MRM and compares their hearing results with those obtained by other authors using ICWM and MRM and a variety of reconstructive techniques. In this study there were no dead ears and no significant changes in bone conduction d

Cranial nerve preservation after stereotactic radiosurgery of intracanalicular acoustic tumors.

We reviewed our initial stereotactic radiosurgery experience in 10 patients with intracanalicular acoustic tumors managed by radiosurgery during a 5-year period. These patients constitute 4.7% of acoustic tumor patients who underwent Gamma Knife radiosurgery during this period. Tumor volume stabilization was achieved in 8. Two patients had initial growth followed by delayed growth arrest. Preservation of preoperative hearing was achieved in all patients in the immediate postoperative period and in 8 of 10 at 1 year. No patient had developed facial or trigeminal nerve dysfunction at the last fo

A personal history of stapedectomy.

Aristotle has said the essential ingredient of tragedy is first hubris. Fame leads to the hubris that offends the gods, who send great punishment. This is so true in the history of stapedectomy. The three distinct eras of surgery for otosclerosis teach us a lot about what happens in science and in life. The first stapes era began in Europe, ahead of its time, and in those halcyon days before the turn of the century, the Belle Epoch, proceeded, uncorrected to its tragic extreme, and then was stopped suddenly, quite rightly, by the establishment. The fenestration era proceeded to an extreme, whe

Vestibular Atelectasis: A Narrative Review and Our Experience.

Vestibular atelectasis (VA) is a rare clinical entity characterized by a collapse of the endolymphatic space resulting in vestibular loss with the possible onset of positional and/or sound/pressure-induced vertigo. It could be idiopathic or secondary to other inner-ear diseases including Meniere's disease (MD). A collapse of the membranous labyrinth involving the semicircular canals (SCs) and the utricle represents its distinctive histopathological feature. While specific radiological patterns consistent with VA have been described on contrast-enhanced MRI with delayed acquisitions, an impairm

Prevalence, Determinants, and Consequences of Vestibular Hypofunction. Results From the KORA-FF4 Survey.

Objective: Uni- or bilateral vestibular hypofunction (VH) impairs balance and mobility, and may specifically lead to injury from falls and to disability. The extent of this problem in the general population is still unknown and most likely to be underestimated. Objective of this study was to determine the prevalence, determinants, and consequences of VH in the general population. Methods: Data originates from the cross-sectional second follow-up (FF4) in 2013/14 of the KORA (Cooperative Health Research in the Region of Augsburg)-S4 study (1999-2001) from Southern Germany. This was a random sam

Evaluation of the stability of the stereotactic Leksell Frame G in Gamma Knife radiosurgery.

The purpose of this study was to evaluate the stability of the Leksell Frame G in Gamma Knife radiosurgery (GKR). Forty patients undergoing GKR underwent pretreatment stereotactic MRI for GKR planning and stereotactic CT immediately after GKR. The stereotactic coordinates of four anatomical landmarks (cochlear apertures and the summits of the anterior post of the superior semicircular canals, bilaterally) were measured by two evaluators on two separate occasions in the pre-treatment MRI and post-treatment CT scans and the absolute distance between the observations is reported. The measurement

Study of a temporal bone of Homo heildelbergensis.

CONCLUSIONS: The characteristic features of the Hh specimen conformed to those of other Pleistocene human fossils, indicating strong cranial structures and a heavy mandible. The mastoid was large and suggested a powerful sternocleidomastoid muscle. The inner ear and tympanic cavities were similar in size and orientation, suggesting that their functions were probably similar. Our observations suggest that the left ear of this Hh specimen was healthy. The large canaliculo-fenestral angle confirms that this ancestor was bipedal. It also strongly suggests that Hh individuals were predisposed to de
