[Non-infectious unilateral wing lameness of pigeons, so called "Schieffliegersyndrom". First clinical and pathological results].

OBJECTIVE: After medium- and long-distance flights and following the first training units of the year, a unilateral injuring of the shoulder joint is observed in racing pigeons. The objective of the study was to discuss the pathogenesis and aetiology of the damage. MATERIAL AND METHODS: In 35 pigeons suffering from unilateral wing lameness, the affected shoulder joints were examined microbiologically and histopathologically. Additionally, both shoulder joints of 12 affected pigeons were examined pathologically and histopathologically. RESULTS: Joint capsule, articular cartilage, tendons and

Shoulder instability with concomitant bone loss in the athlete.

Thorough evaluation of the athlete with persistent shoulder instability and appropriate use of imaging modalities, such as 3-dimensional computed tomography, can help quantify the severity of bony deficiency. Based on obtained imaging and examination, surgical and nonsurgical methods can be considered. In many situations both the humeral- and glenoid-sided bone loss must be addressed. Depending on the extent of bone loss, athletic demands, and surgeon experience, arthroscopic or open surgical options can provide shoulder stability and return athletes to their prior level of activity.

Imaging update on the glenoid labrum: variants versus tears.

This article reviews the normal labral variants of the shoulder and describes imaging features, including secondary signs, to help discriminate tears from normal variants. The labral variants can be divided into two main groups. One group is composed of nonstandard appearances in the labral shape, signal intensity, or the site of affixation to the glenoid rim. The other group consists of classic labral variants, where the labrum is partially or completely unattached to the subjacent bone, or a segment of the labrum is absent.

Subacromial pressures in vivo and effects of selective experimental suprascapular nerve block.

Subacromial impingement has been related to increased subacromial pressures. High subacromial pressures may, therefore, have a negative effect on tendon healing after rotator cuff repair, but avoidance of high pressures during healing is only possible if pressures in different positions of the arm are known. The purpose of this study was to determine the subacromial pressures for different positions of active shoulder movement and to determine the effects of supraspinatus and infraspinatus failure on subacromial pressures, as it is currently held that rotator cuff weakness might increase subac

[Isokinetic strength measurement and training of the shoulder: methodology and results].

OBJECTIVE: To evaluate the contribution of isokinetic methods of shoulder strength measurement and training. METHOD: A Medline search of English and French publications, including referenced articles, allowed us to analyse non-indexed publications. Eighty-seven articles were retained for analysis. RESULTS: The isokinetic evaluation of the shoulder is valid. Although reproducibility of shoulder evaluation is inferior to that of the knee, it is nevertheless satisfactory when a rigorous test method is used. Normal values for the rotators, abductors-adductors, and extensors-flexors depend on div

Superior labrum anterior posterior (SLAP) repair using the Neviaser portal.

Superior labrum anterior posterior (SLAP) repair using suture anchors requires successful passing of sutures through the labrum. The optimal angle for passing a penetrating suture retriever is perpendicular to the superior labrum. The Neviaser (superior-medial) portal, without a cannula, is ideally suited for this procedure. The authors demonstrate their technique and review the anatomy of this portal. The Neviaser portal is illustrated as an important working portal and not merely for inflow or outflow. The authors demonstrate that this is a safe and easy technique for SLAP repair.

Magnetic resonance imaging of the shoulder.

Magnetic resonance imaging (MRI) is proving to be an effective means for evaluating the shoulder. The use of a surface coil and high resolution scanning techniques have allowed detailed analysis of the normal anatomy of the shoulder. When evaluating for shoulder pathology, the choice of imaging plane and pulse sequencing will be determined by the suspected pathology. In patients with impingement syndrome, subacromial bursitis, supraspinatus tendinitis, and supraspinatus tendon tear can be differentiated, and the offending component of the subacromial are traumatizing these soft tissues can be

Shoulder position sense during passive matching and active positioning tasks in individuals with anterior shoulder instability.

BACKGROUND: Altered neuromuscular control due to compromised joint position sense may contribute to recurrent shoulder instability. OBJECTIVE: The purpose of the present study was to examine whether individuals with anterior shoulder instability exhibit larger shoulder position sense errors than those with healthy shoulders in both passive matching and active positioning. DESIGN: This was a between-groups study with repeated measures. METHODS: Ten people with anterior shoulder instability and 15 people with healthy shoulders participated in the study. Shoulder position sense was examined wi

Seatbelt use to save money: Impact on hospital costs of occupants who are involved in motor vehicle crashes.

OBJECTIVE: Seatbelt use is the single most effective way to save lives in motor vehicle crashes (MVC). However, although safety belt laws have been enacted in many countries, seatbelt usage throughout the world remains below optimal levels, and educational interventions may be needed to further increase seatbelt use. In addition to reducing crash-related injuries and deaths, reduced medical expenditures resulting from seatbelt use are an additional benefit that could make such interventions cost-effective. Accordingly, the objective of this study was to estimate the correlation between seatbel

The operative outcomes of displaced medial-end clavicle fractures.

BACKGROUND: Nonoperative treatment of displaced medial clavicle fractures often leads to poor functional outcomes and painful nonunions. This study investigates the functional outcomes of patients undergoing operative fixation of these fractures. METHODS: We investigated 27 patients undergoing operative fixation of a medial clavicle fracture; 24 had an acute, displaced fracture and 3 had fixation for nonunions. Preoperative radiographs or computed tomography scans were obtained, and data collected included age, sex, mechanism of injury, and fixation method. Follow-up included physical examina

Combined injury of the accessory nerve and brachial plexus.

BACKGROUND: Stretch-induced spinal accessory nerve palsy has been considered extremely rare, with only a few cases reported. OBJECTIVE: In 357 patients with stretch lesions of the brachial plexus, we investigated the prevalence, course, and surgical treatment of accessory nerve palsy. METHODS: Accessory nerve palsy was ascertained when the patient was unable to shrug the ipsilateral shoulder. Patients underwent brachial plexus reconstruction between 6 and 8 months after trauma. To confirm paralysis, during surgery, the accessory nerve was stimulated electrically. RESULTS: Accessory nerve pa

Simultaneous bilateral posterior dislocation of the shoulder: diagnostic problems and management. A case report.

We present the case of a patient who sustained simultaneous bilateral posterior dislocation of the shoulder after a possible epileptic fit. The confirmation of the diagnosis was reached only by a computed tomography (CT) scan, after the clinical suspicion. Under general anesthesia, close reduction of both shoulder dislocations was done. Posterior dislocation of the shoulder-especially the bilateral one-is very rare. When the history describes an electric shock or convulsive seizure, any shoulder injury demands a careful clinical and radiological evaluation. It is usually associated with revers

Rehabilitation of the shoulder in tennis players.

The tennis player places unique demands on the shoulder by creating a high risk for overuse and overloading of the soft tissues. Tennis requires concentric work to position and move the arm, eccentric work to stabilize the shoulder, effective depression of the humeral head to avoid impingement in the overhead position, and normal stability to prevent secondary impingement. The tennis serve produces enormous angular velocities about the shoulder joint. A comprehensive rehabilitation program has been described in which the therapist, trainer, player, and physician alike need to have an understan

Common conditions in the overhead athlete.

The overhead athlete is at unique risk for injury because of the mechanics associated with rapid shoulder elevation, abduction, and external rotation. Angulation of the humeral head against the posterosuperior glenoid can cause rotator cuff tendon and labral impingement. The throwing or striking motion of baseball, softball, water polo, tennis, racquetball, and volleyball may result in scapular dyskinesis, partial articular-sided supraspinatus avulsions, and posterosuperior labral tears. The SICK scapula syndrome (scapular malposition, inferior medial border prominence, coracoid pain and malpo

The efficacy of electroacupuncture for cervical nerve edema and movement disorder caused by the brachial plexus injury: a case report.

UNLABELLED: The brachial plexus injury (BPI) is one of the most severe types of peripheral nerve injuries, often caused by upper limb traction injury. In clinic, the surgery is widely used to treat the BPI. However, surgery may need to be performed multiple times at different stages, which carries risks and brings heavy economic burden. In non-surgical treatment, splinting, local injection of corticosteroids, and oral corticosteroids can achieve significant short-term benefits, but they are prone to recurrence and may cause complications of mechanical or chemical nerve damage. In this report,
