Hyponatremia, usually attributed to the syndrome of inappropriate secretion of antidiuretic hormone, typically occurs in a delayed fashion following transsphenoidal removal of a pituitary adenoma. In a series of 99 consecutive patients who underwent transsphenoidal surgery for pituitary adenoma, nine patients developed delayed hyponatremia, seven of whom were symptomatic. Of these seven patients, four had been discharged from the hospital and required readmission on postoperative Day 7 to 9. In the nine patients who developed hyponatremia, on the average, serum sodium levels began to fall on D

Chiasmal syndromes (characterized by bitemporal field defects, decrease of vision and simple optic atrophy) are not only caused by tumors within the chiasmal region, but also occur in inflammatory, toxic, demyelinizing and hereditary-atrophic processes of the chiasm, respectively the optic nerve. In the presence of classic bitemporal visual field defects the probability of a tumor within the sellar region is very great. Apart from the most frequent pituitary adenoma one has to consider the craniopharyngioma, the meningioma of the tuberculum sellae, the olfactory meningioma and the meningioma o

OBJECTIVE: Recently, 3-dimensional-computed tomography (3D-CT) imaging has been used in the diagnosis and surgical treatment planning of patients with craniofacial deformities. The present authors have developed a 3D-CT imaging procedure for a 3-dimensional coordinate point evaluation system to assess and diagnose patients with facial asymmetry.
STUDY DESIGN: The CT data of 16 subjects was selected retrospectively as the control group from patients who had undergone CT examinations to diagnose conditions other than maxillofacial deformities. Anatomical landmarks modified from orthodontic cran

Since the early days of pituitary surgery, a variety of transnasal approaches have been used to gain access to the sella turcica. Each of these approaches requires crossing the sphenoid sinus, hence the transsphenoidalnoidal designation of these methods. Since the growth and refinement of microsurgery as a distinct surgical discipline, there has been a coincidental maturation of transsphenoidal microsurgical techniques for the management of a variety of pituitary disorders. The present paper reviews the historical events leading to the current methods of transphenoidal pituitary surgery. Detai

PURPOSE: The purpose of this study was to examine bone healing after Le Fort I osteotomy in Class III patients.
PATIENTS AND METHODS: The study group consisted of 18 Japanese patients with mandibular prognathism with and without asymmetry, maxillary retrognathism or open bite. A total of 36 sides were examined. Le Fort I osteotomy was performed without a pterygoid osteotome, with an ultrasonic curette used to remove interference at the pterygomaxillary region. Titanium plates (Universal Mid-face fixation module, Stryker, Freiburg, German) were used for four patients, absorbable plates (poly-L

Various interpretations of clinical and cephalometric data lead to different diagnostic and surgical planning in the study of dentoskeletal malformations. Many authors have identified this discrepancy as an incorrect positioning between the skeletal structures and the Frankfurt plane (PFH)--a plane that is used routinely for the sagittal measurement of the upper and lower jaw. The aim of this study was to establish a control method to verify the correct positioning of the sella-nasion point plane (SN) and the PFH. To find a reference plane, the authors concentrated their attention on measuring

OBJECTIVE: To assess whether 2D cephalometrics is comparable with 3D imaging devices and whether 3D technology could replace traditional 2D image capture in posttreatment evaluation.
MATERIALS AND METHODS: The study is a prospective evaluation of superimposition techniques obtained from a cohort of 40 patients who underwent orthognathic surgery in a private practice environment. Surgical records were obtained from lateral cephalometric radiographs taken by a Kodak 8000C machine, and the 3D images were obtained from the 3dMD stereo photogrammetric camera capture system. The aims of this study were to compare measurements on three-dimensional (3D) models of human skulls derived from two different cone beam CT scanners (CBCT) and to evaluate if the used hardware can influence the performed measurements. CBCT scans of 40 dry human skulls with both the i-CAT and the Iluma cone beam CT scanners were made. From the CBCT scans, 3D models were constructed. One operator identified 19 landmarks five times on both types of 3D models with a time interval of 1 week. Intra-observer reliability was high for most measurements. There was a statistically significant and clinic

The purpose of this retrospective longitudinal study was to compare 7 cephalometric measurements of the cranial base in subjects with Class I and Class II skeletal patterns at ages 1 month, 2 years, and 14 years. A sample of 22 Class I and 21 Class II subjects was selected; the inclusion criteria were overjet, ANB, and Harvold unit difference. Analyses of head circumference, crown-rump length, and weight revealed no significant (P >.15) differences between the Class I and Class II infant subjects at the initial age (1 month). One angular and 6 linear measurements were first compared with a mul

OBJECT: The author's personal series included 148 patients who sustained traumatic frontobasal injuries and were treated between 1986 and 1999. Included in this study are 74 of 148 patients with acute injuries and complex frontobasal lesions involving the frontal sinus, the cribriform/ethmoid roof complex, one or both orbital roofs, and the planum sphenoidale.
METHODS: Surgery was delayed for up to 4 weeks postinjury in most patients (67 cases), whereas 17 with space-occupying hematomas and perforating injuries required early surgery. In 30 patients additional surgery was required to treat ma

OBJECTIVE: The infratemporal fossa (ITF) is a continuation of the temporal fossa between the internal surface of the zygoma and the external surface of the temporal bone and greater wing of the sphenoid bone that is sitting deep to the ramus of the mandible. The principal structure to understanding its relationships is the lateral pterygoid muscle. Other important structures are the medial pterygoid muscle, the maxillary artery, the pterygoid venous plexus, the otic ganglion, the chorda tympani nerve and the mandibular nerve. In this study, we describe the microsurgical anatomy of the ITF, as …

CONCLUSION: In a retrospective study of 239 cases of fungal sinusitis, noninvasive paranasal sinus aspergillosis was most common and successfully treated by endoscopic sinus surgery (ESS) with postoperative sinus irrigation. For the treatment of fungal sinusitis, ESS with or without antifungal agents and control of predisposing factors for secondary cases are recommended.
OBJECTIVE: The aim of this study was to investigate the clinical characteristics of fungal sinusitis and evaluate the treatment outcomes of primary and secondary fungal infections of the paranasal sinuses.

Sphenoid wing meningiomas are characterized by bony hyperostosis and dural invasion. Surgical resection is often extensive, giving rise to complex orbital defects. Orbital reconstruction is associated with improved structural and functional outcomes. Patient-specific cranioplasty implants facilitate the accurate orbital volume reconstruction, with polyetheretherketone (PEEK) exhibiting advantageous material properties. This retrospective case series evaluates the clinical outcomes of immediate PEEK-implant reconstruction of complex orbital defects in patients undergoing sphenoid wing meningiom

BACKGROUND: The tumor invasion of the skull base structures is very frequent in spheno-orbital meningiomas. The aim of the present study is to evaluate the invasion rate of skull base structures and the best surgical approach and management.
METHODS: The surgical series of 80 spheno-orbital meningiomas was reviewed. The tumors were classified according to the intraorbital location with respect to the optic nerve axes into three types: I-lateral: II-medial; III-diffuse. The invasion of the orbital apex, optic canal, superior orbital fissure, anterior clinoid, ethmoid-sphenoid sinuses, and infr
