An assessment of anxiety in patients with primary hyperhidrosis before and after endoscopic thoracic sympathicolysis.

OBJECTIVE: Endoscopic bilateral thoracic sympathicolysis (EBTS) is an effective and minimally invasive procedure used for patients with primary hyperhidrosis. The purpose of this study was to examine anxiety levels using standardized psychometric tools in hyperhidrosis patients before and after EBTS. METHODS: A total of 106 patients diagnosed with hyperhidrosis who underwent EBTS were asked to fill out a questionnaire before and 12 months after the procedure that elicited the following information: (a) symptoms associated with hyperhidrosis; (b) the patient's level of anxiety; and (c) the ext

Upper thoracic sympathetic chain neurolysis under CT guidance. A two year follow-up in patients with palmar and axillary hyperhidrosis.

PURPOSE: To evaluate the efficacy, safety and middle-term results of upper thoracic sympathetic chain neurolysis performed under CT guidance in patients with palmar/axillary hyperhidrosis. MATERIALS AND METHODS: From April 1999 to July 2000 we performed 30 upper thoracic sympathetic chain neurolysis in 15 patients presenting with palmar/axillary hyperhidrosis (6 M, 9 F, mean age 29.5 years). Neurolysis was performed under CT guidance injecting 2-10 ml of a solution of phenol 8%, glycerine 20% and saline through 22 G needles placed in the paraspinal space at T3 level through a paraspinal extra

Successful treatment of facial blushing by endoscopic transthoracic sympathicotomy.

Facial blushing is one of the cardinal symptoms of social phobia and has a strong negative impact on the quality of life. Traditional therapeutic options are psychotherapy and pharmacological treatment. The results of these treatments on facial blushing are poorly documented. To investigate whether endoscopic bilateral transection of the upper thoracic sympathetic chain is efficient in the treatment of facial blushing, 244 consecutive patients were treated with bilateral endoscopic transthoracic sympathicotomy (ETS). The results were evaluated by questionnaire and symptoms assessed with visual

Norepinephrine depletion in the amygdala inhibits CRF-41, ACTH, and corticosterone responses following photic simulation.

The effects of amygdaloid norepinephrine depletion by 6-hydroxydopamine on changes in corticotropin releasing factor-41 (CRF-41) and serum adrenocorticotropic hormone (ACTH) and corticosterone levels, following neural stimuli were investigated. In intact animals, photic or acoustic stimulation caused CRF-41 depletion from the median eminence and a rise in serum ACTH and corticosterone levels. In rats with amygdalar norepinephrine depletion there were no changes in basal CRF-41, ACTH, or corticosterone levels. However, the above responses of the hypothalamo-pituitary adrenocortical axis were bl

Changes in cardiocirculatory autonomic function after thoracoscopic upper dorsal sympathicolysis for essential hyperhidrosis.

Essential hyperhidrosis (EH) is caused by an unexplained overactivity of the sympathetic fibers which pass through the upper dorsal sympathetic ganglia D2 and D3. Since the D2 and D3 ganglia are also involved in the sympathetic cardiac innervation, cardiocirculatory autonomic function may also be abnormal in EH. In order to study the function of the sympathetic nervous system in EH, and to assess the effects of thoracoscopic sympathiocolysis, cardiocirculatory autonomic function tests were performed in 13 consecutive patients with EH, before (baseline) and 6 weeks after the thoracoscopic inter

Destruction of the vesicoureteric plexus for the treatment of hypersensitive bladder disorders.

OBJECTIVE: To determine whether bilateral vesicoureteric ganglion excision is effective in the treatment of women with hypersensitive bladder disorders. PATIENTS AND METHODS: Utilizing neuroconductive studies, 175 women with hypersensitive bladder complaints of burning and frequency were selected by the presence of abnormal pudendal nerve sensory latency studies. Cystoscopy under anaesthesia revealed petechial haemorrhaging peritrigonally with bladder neck involvement, consistent with interstitial cystitis. RESULTS: Laser obliteration of the vesicoureteric plexus bilaterally resulted in comp

Effect of central norepinephrine depletion on renovascular hypertension and on the renin system.

Some reports have stated that central norepinephrine (NE) depletion inhibited the development of hypertension in the rat. On the other hand, this pharmacological treatment induces changes on the central renin-angiotensin system. The present study was designed to follow the development of 2 kidney-2 clip (2k-2c) renovascular hypertension in rats depleted of central NE and to analyze the central and peripheral renin-angiotensin system. Male Wistar rats (n = 40) were used. Half of the animals was injected, intracisternally, with 6-hydroxydopamine (6-OHDA), the remaining rats only received the veh

Sympathetic nerve modulation of regional cerebral blood flow during asphyxia in newborn piglets.

Regional cerebral blood flow (rCBF) during asphyxia suggests a reflex vasoconstrictor mechanism active principally in brain cortex. Present studies in newborn piglets investigate sympathetic modulation of the cerebrovasculature both during and after acute asphyxia. Unilateral superior cervical sympathetic ganglionectomy (SCSG) was performed in 13 newborn piglets, after which asphyxia was produced by discontinuing ventilation. In 8 animals, blood flow was measured during control and sequentially 1, 2, and 3 min after ventilation was stopped. In 5 piglets with unilateral SCSG, cortical flow decr

Reflex sympathetic dystrophy syndrome: a retrospective pain study.

While reflex sympathetic dystrophy syndrome (RSDS) research is lacking and the pathophysiology remains obscure, it is known that it affects all age groups with the common overriding complaint of severe, unrelenting, burning pain. It seems to be triggered by trauma (major or minor), including more central events such as myocardial infarction, cerebrovascular accident and tumours. Diagnostic characteristics of RSDS are: spontaneous burning pain, hyperalgesia, vasomotor disturbances, exacerbations by emotional upset, occurrence either spontaneously or after minor injury, occasional spontaneous re

Reinnervation of primate ciliary muscle following ciliary ganglionectomy.

Ciliary ganglionectomy and/or postganglionic ciliary neurectomy in the cynomolgus monkey was followed by supersensitivity to intramuscular (i.m.) pilocarpine and lack of response to topical eserine and to electrical stimulation of the Edinger-Westphal nucleus. Normal responsiveness to pilocarpine and eserine returned in most instances after about 6 months. An accommodative response to stimulus of the Edinger-Westphal nucleus was also present and, as in control eyes, could be blocked by hexamethonium. Collectively, these findings demonstrate that parasympathetic innervation to the ciliary muscl

Inhibitory effects of superior cervical ganglionectomy on dimethylbenz(a)anthracene-induced mammary tumors in the rat.

The role of the pineal in regulating the oncogenic processes was explored in Sprague-Dawley female rats by comparing incidence and growth of mammary tumors in animals subjected to superior cervical ganglionectomy (SCGx) or blinding and anosmia (BAs) with that of intact rats treated with 7-12-dimethylbenz(a)anthracene (DMBA). The surgery was performed at the age of 56 days, 1 day following the administration of the carcinogen. Growth of mammary tumors was studied, and 15 weeks later the rats were sacrificed by decapitation and the activity of the pineal hydroxyindole-O-methyltransferase (HIOMT)

[Polymyalgia rheumatica: a systemic vasculitis. Review of the literature and presentation of 2 clinical cases].

A brief historical account of the nosological and pathogenetic classification of rheumatic polymyalgia (RPM) is followed by an indication of the importance of vasculitis during its course. Elective involvement of the temporal artery is not always observed, though there may also, or exclusively, be involvement of other arterial districts, such as those of the subaxillary and subclavicular, that of the thoracic and abdominal aorta, and of the large branches of the lower limbs. Two clinical cases are presented in support of these assertions. It is also shown that arteritis may appear several mont

[Changes in the activity of the intracardiac postganglionic apparatus during the interaction of the regulatory sympathetic and parasympathetic effects on the pacemaker rhythm].

Chronotropic effect on the isolated sinus node of the frog heart and postganglionic impulse activity of the intracardiac pathways were studied during separate and joint stimulation of extracardiac sympathetic and parasympathetic nerves. The joint stimulation attenuated or augmented the impulse activity. This effect depended on the "energy" of initial parasympathetic and/or sympathetic activity. beta-Adrenoblocking inderal abolished the parasympathetic impulse activity augmentation. A possible role of intracardiac ganglionic system in the sympathetic-parasympathetic interaction is discussed.

Cardiovascular reactivity and neurogenic tone in unilateral renovascular hypertension in the rat.

The role of some mechanisms in the development of hypertension due to unilateral renal artery stenosis is influenced by the presence or absence of the intact opposite kidney. In this paper: a) the cardiovascular reactivity (CR) to norepinephrine (NE) and b) the effect of a ganglionic blocker (pentolinium, P) during the early (first two weeks) and later periods (ten weeks) of hypertension elicited by unilateral renal ischemia in the presence of the untouched contralateral kidney in the rat have been reported. Neither the threshold doses nor the dose-pressor response curves have shown a greater

Renal denervation for resistant hypertension: A review of current literature.

Resistant hypertension is defined by persistently elevated blood pressure (BP) despite being treated with three or more antihypertensive medications including a diuretic, leading to an increased risk of cardiovascular events and mortality. It presents a substantial global health challenge, impacting about 10 % of individuals with hypertension. Renal denervation (RDN) has come up as a promising interventional strategy to address this condition by modulating the renal sympathetic nervous system. This review article explores the mechanisms underlying RDN, its historical evolution, recent procedur
