Synovial involvement assessed by power Doppler ultra-sonography in systemic sclerosis: results of a cross-sectional study.

OBJECTIVES: To characterize hand synovial manifestations assessed by power Doppler ultrasonography (PDUS) in a cohort of unselected patients … two main PDUS features were Doppler-positive/inflammatory synovitis (n = 18, 17.5%) and sclerosing tenosynovitis (TS) (n = 19, 18.4%). Inflammatory synovitis was more frequent in the wrist and MCP joints. Sclerosing TS was more frequent in men [odds ratio (OR) = 6.32, 95% CI: 2.17, 18.41; P = 0.001] and was associated with anti-RNA polymerase III antibodies (O …

Repetitive differential finger motion increases shear strain between the flexor tendon and subsynovial connective tissue.

Non-inflammatory fibrosis and thickening of the subsynovial connective tissue (SSCT) are characteristic in carpal tunnel syndrome (CTS) patients. These pathological changes have been linked to repetitive hand tasks that create shear forces between the flexor tendons and SSCT. We measured the relative motion of the flexor digitorum superficialis tendon and SSCT during two repetitive finger tasks using color Doppler ultrasound. Twelve participants performed flexion-extension cycles for 30 min with the long finger alone (differential movement) and with all four fingers together (concurrent moveme

Hand infections in the trauma patient.

Infections are a frequent, unfortunate complication seen in orthopedic trauma patients. The specific anatomy of the upper extremity makes it an easy target for opportunistic organisms, especially after an acute traumatic event. In addition, pathogenic factors and host factors can contribute to the severity of the infection. Meticulous wound management, identification of the offending organism(s), and tailoring of care to the special needs of the very sick trauma patient limit complications.

[Results of early intervention in acute finger infections].

In the examination of 66 patients treated for acute felons, we classified the infections of fingers as (1.) cutaneous/subcutaneous felons; (2.) paronychia/subungual felons, (3.) thecal whitlow; (4.) articular/bony felons. Primarily, the therapy was based on early aggressive surgical treatment, and secondly on antibiotics. With the management described we achieved a good to very good result in 76%.

Enhanced diagnosis of hand and wrist disorders by triple phase radionuclide bone imaging.

Triple phase radionuclide bone imaging (TPI), a diagnostic study, consists of a blood flow evaluation by means of an intravenous bolus injection of radionuclide tracer and rapid sequential images which show the arterial pattern; an immediate static image reflecting the pooling of activity in the soft tissues; and delayed static images taken about one hour later which reflect bone localization of the radiotracer. TPI is not specific for a particular condition, but it will confirm or rule out the presence of disease.

Carpal tunnel syndrome, humeral epicondylitis, and the cervical spine: a study of clinical and dimensional relations.

Forty-three patients with idiopathic carpal tunnel syndrome, confirmed by nerve conduction studies and treated by surgery, were compared clinically and radiologically with 43 age- and sex-matched control patients. Patients with carpal tunnel syndrome had a significantly greater prevalence of lateral humeral epicondylitis (tennis elbow) (33%) than controls (7%). Randomised reading of the cervical spine radiographs in ignorance of the groups to which they belonged showed no significant difference in the prevalence of either intervertebral disc degeneration or intraforaminal osteophyte protruion

Musculoskeletal ultrasound findings in GCA with PMR and isolated PMR.

OBJECTIVES: To compare musculoskeletal ultrasound (MSUS)-defined musculoskeletal involvement between patients with isolated PMR and those with PMR symptoms in GCA and to explore the utility of MSUS findings combined with CRP levels for identifying GCA. METHODS: We analysed 20 patients with GCA-PMR and 40 with PMR from a prospectively maintained MSUS registry before treatment initiation. Both shoulders and knees were assessed using greyscale (GS) and power Doppler (PD) ultrasound. Tenosynovitis, bursitis, tendinitis and ligamentitis were scored semiquantitatively. The association between ultra…

Comparative effectiveness of extracorporeal shock wave therapy and high-intensity laser therapy in trigger finger: a randomized controlled trial.

Trigger finger (TF) is stenosing tenosynovitis causing pain and impaired hand function. To compare extracorporeal shock wave therapy (ESWT) and high-intensity laser therapy (HILT) as adjuncts to home exercise in TF. In this three-arm randomized controlled trial, patients were allocated to ESWT, HILT, or home exercise alone. Active modalities were applied for 3 weeks (nine sessions), and all groups followed the same exercise program. Assessments were performed at baseline, week 3, and week 12. The primary outcome was week 12 QuickDASH; secondary outcomes were VAS, Quinnell grade, grip strength,…

Refining Human Phenotype Ontology (HPO) to enable better phenotype-genotype integration in systemic autoimmune rheumatic diseases.

BACKGROUND: The Human Phenotype Ontology (HPO) provides a standardised framework for disease-phenotype associations. Given the complexity of … ti-chromatin antibody, autoimmune hepatitis, scleroderma renal crisis, shrinking lung syndrome, and tenosynovitis. The HPO term "scleroderma" was misdefined, and six HPO terms did not include six linked SARDs in the respective disease-association section. In a random sample of 10 SARD patients, we also demonstrated that HPO terms affect the gene variant priorit …

Dorsal Approach for Corticosteroid Injection in Trigger Finger Management: A Scoping Review.

Introduction: Stenosing tenosynovitis (trigger finger) is a common condition caused by inflammation and hypertrophy of flexor tendons. Corticosteroid injection (CSI) is an effective and safe treatment option. A palmar approach for CSI is typically used; however dorsal injection, which may be less painful, is not well-studied. Methods: A 6-stage scoping review was conducted to characterize outcomes associated with dorsal CSI for trigger finger management. We searched Ovid MEDLINE, EMBASE, and Web of Science for eligible articles in English from inception to July 2024. Data regarding study chara…

A Novel Variant of Avian Reovirus Is Pathogenic to Vaccinated Chickens.

Avian reovirus (ARV) infections, characterized by severe arthritis, tenosynovitis, pericarditis, and poor weight gain, have become increasingly serious in recent years. The economic impact is significant as it causes growth inhibition and immunosuppression. Some commercial poultry in China have been widely vaccinated with available ARV vaccines; however, infections continue to occur even after vaccination. This study aimed to isolate a novel variant, ARV-SD19/11103, from the joint tissues of infected broiler chickens vaccinated with ARV vaccines in Shandong Province. Genetic evolution analysis…
