Analysis of root canal treated primary incisor after trauma: two year outcomes.

BACKGROUND: The low number of clinical studies of traumatized teeth submitted to root canal treatment is completely out of proportion to the seriousness that dental trauma imposes on children in early years. AIM: This study evaluates the outcomes of root canal treatment (RCT) in traumatized primary incisors and identifies the predisposing factors associated with therapy success. METHODS: This is a retrospective study conducted with all dental records of 704 patients who had one or more teeth with traumatic injuries. Patients with irreversible pulp changes in primary teeth leading to RCT with

The use of likelihood ratio methodology to find predictors of treatment outcome in patients with dental injury diagnoses.

The purpose of this prospective, cohort study of patients with dental injuries was to develop prediction rules to predict treatment response related to the management of dental injuries. The study comprised of 130 patients with a single permanent incisor affected by a dental displacement (n = 100) or fracture injury (n = 30). Laser Doppler flowmetry (LDF) measurements of pulpal blood flow (PBF) were taken 6 and 18 weeks after dental injury Treatment response (success or failure) was categorized based on findings of clinical and radiographical evaluation after 9 months. Forty-four (34%) subject

Clinical and Micromorphologic 29-year Results of Posterior Composite Restorations.

Prospective clinical studies of composite restorations revealed their safety and longevity; however, studies did not elucidate the dynamic mechanisms of deterioration caused by fractures and secondary caries. Therefore, the aims of this 29-y controlled study were 1) to follow up on the clinical behavior of posterior composite restorations annually and 2) to compare clinical outcomes with micromorphologic scanning electron microscopy features. After ethical approval, the single-arm study commenced in 1987 with 194 class I or II primary posterior composite restorations with glass ionomer cement

Effect of preparation designs and CAD-CAM materials on step-stress fatigue survival of premolar partial coverage restorations: An in-vitro study with fractographic analysis.

OBJECTIVES: The growing trend of minimally invasive approaches has encouraged the application of partial coverage designs in restorative dentistry. This study aimed to evaluate the cyclic fatigue performance of different CAD-CAM materials used in various partial coverage designs for premolar restorations. METHODS: A freshly extracted upper premolar was prepared using a high-speed handpiece to create mesio-occluso-distal (MOD) cavities with standardized dimensions. Three preparation designs were investigated: onlay, overlay, and vonlay. CAD software was used to create digital models of the res

The effects of ageing on the biomechanical properties of root dentine and fracture.

OBJECTIVES: Knowledge of the mechanical behaviour of root dentine can facilitate better understanding of spontaneous vertical root fracture (VRF), an age-related disease initiated mainly at the root apex. We tested the hypothesis that the biomechanical properties of root dentine change with ageing. METHODS: Sixteen human premolars were divided into "old" (17-30 years) and "young" (50-80 years) groups. The elastic modulus, nano-hardness, micro-hardness, elemental contents, tubular density/area of root dentine in cervical, middle and apical root regions were evaluated using atomic force microsc

[Use of osteoconductive materials in pediatric dental medicine].

Osteoconductive materials are well known to have the ability to form a characteristic scaffold for bone replacement. The use of these agents is mostly described in periodontal procedures. The present study deals with the use of two different osteoconductive materials used for repairing bone defects following traumatic injuries. The materials used were Bio-Oss (Geistlich) an allogenic demineralised bone and Fisiograft (Ghimas) a synthetic co-polymer of polyglicolic and polylactic acid. These materials were used in five different clinical cases with the objective to compare their typical propert

Fractographic analysis of separated endodontic file designs.

Endodontic rotary files are cutting instruments used to perform root canal procedures within a tooth interior. Focusing on quantitative fractographic analysis increases necessary, clinical performance understanding of file separation failure. This research employed controlled, dynamic testing to failure of commercial rotary files, analyzing the fractographic, forensic characteristics in relation to Weibull reliability determination, considering: (1) design analysis; (2) stress concentrations; (3) times to failure; (4) number of cycles to failure (NCF). Ex vivo testing included three file desig

Impacts of Contracted Endodontic Cavities on Instrumentation Efficacy and Biomechanical Responses in Maxillary Molars.

INTRODUCTION: Recently, we reported that in mandibular molars contracted endodontic cavities (CECs) improved fracture strength compared with traditional endodontic cavities (TECs) but compromised instrumentation efficacy in distal canals. This study assessed the impacts of CECs on instrumentation efficacy and axial strain responses in maxillary molars. METHODS: Eighteen extracted intact maxillary molars were imaged with micro-computed tomographic imaging (12-μm voxel), assigned to CEC or TEC groups (n = 9/group), and accessed accordingly. Canals were instrumented (V-Taper2H; SSWhite Dental, L

The immediate replacement of teeth by dental implants using homologous bone membranes to seal the sockets: clinical and radiographic findings.

Placement of dental implants into fresh extraction sockets has several advantages compared to the original protocol. However, the soft-tissue handling is often complicated and the aesthetic results less rewarding. A method to use a homologous bone membrane as the sole sealer of the extraction socket after immediate implant placement is introduced and the outcome of 34 cases is presented. In 4 cases early losses of the membranes occurred (2 of these iatrogenic damage) in all other cases the homologous membranes were covered by proliferation of the surrounding mucosa within 2-4 weeks. In 2 of th

A Novel Surgical Technique for Maxillary Alveolar Sealing: Use of a Tunneled Pedicled Connective Tissue Flap in Ridge Preservation Procedures.

This case report illustrates the use of a novel design for a tunneled connective tissue pedicled flap in socket sealing for guided bone regeneration procedures, specifically in the management of compromised sockets. The purpose of this flap is to optimize soft tissue healing and esthetics in these situations, permitting primary closure without buccal flap advancement. Initially, after creating the surgical access with a sulcular and a mesial releasing incision, extraction of the fractured tooth and immediate implant placement were performed. A xenograft and collagen membrane were then placed.

Dental Management of a Patient with Multiple Idiopathic Cervical Root Resorption.

Multiple Idiopathic Cervical Root Resorption (MICRR) is a rare condition. It initiates at the cemento-enamel junction of multiple teeth. The lesions continue to grow until they unite, thereby undermining the entire coronal structure of affected teeth. Its distribution can vary from a single region to the entire dentition and the number of teeth affected by resorption tends to increase as the condition is followed over time. The teeth themselves appear clinically normal. The aetiology of MICRR is unknown and it is considered to be a diagnosis of exclusion. The condition tends to be progressive.
