Urrets-Zavalia syndrome is a surgical complication in which the pupil remains fixed and dilated after a procedure. It is believed to be caused by brief periods of high intraocular pressure. Although the syndrome was originally associated with penetrating keratoplasty, it has subsequently been associated with other procedures. We report the case of a 13-year-old boy with congenital glaucoma who developed Urrets-Zavalia syndrome after goniotomy. After 2 years of follow-up, the pupil remained fixed and dilated in that eye. A diode laser, which emitted infrared radiation at a wavelength of 810 nm, was used to perform trabecular photocoagulation in a human eye due for enucleation for malignant melanoma. For comparison, burns were applied with an argon blue-green laser (488-514.5 nm). With each laser, the treatment spot size was 100 microns and the pulse duration was 0.20 sec. Examinations of the visual field in 25 patients with Stages I and II primary open-angle glaucoma have shown that in the majority of cases surgery results in improvement of stabilization of the visual field status in respect of all its parameters (retinal sensitivity threshold, deficiency of the field and sensitivity). Increase of the number of defects and reduction of the retinal ability to discern light were observed in 5 patients: in 2 with postoperative hypotension and 3 with progressive cataracts.

INTRODUCTION: Glaucoma in ocular cicatricial pemphigoid (OCP) is an important blinding complication. Glaucoma in eyes with OCP is most often managed medically as surgical management is difficult in eyes with cicatrised conjunctiva and often end up with complications or scarring and failure. In some cases where the topical antiglaucoma medications are not tolerated or worsen the OCP and in eyes with medically uncontrolled glaucoma, surgical management would be needed.
