Machine learning can guide suitability of consultation and patient referral through telemedicine for hepatobiliary diseases.

BACKGROUND AND AIM: Telemedicine is an evolving tool to provide health-care services. We evaluated the suitability of telemedicine to deliver effective consultation for hepatobiliary disorders. METHODS: In this prospective study spanning over a year, we interviewed hepatologists delivering the teleconsultations through a pre-validated questionnaire. A consult was deemed suitable based on the physician's judgment in the absence of unplanned hospitalization. We evaluated factors determining the suitability through inferential statistics and machine learning models, namely, extreme gradient boos

Optimizing patient selection for primary prevention implantable cardioverter-defibrillator implantation: utilizing multimodal machine learning to assess risk of implantable cardioverter-defibrillator non-benefit.

AIMS: Left ventricular ejection fraction (LVEF) is suboptimal as a sole marker for predicting sudden cardiac death (SCD). Machine learning (ML) provides new opportunities for personalized predictions using complex, multimodal data. This study aimed to determine if risk stratification for implantable cardioverter-defibrillator (ICD) implantation can be improved by ML models that combine clinical variables with 12-lead electrocardiograms (ECG) time-series features. METHODS AND RESULTS: A multicentre study of 1010 patients (64.9 ± 10.8 years, 26.8% female) with ischaemic, dilated, or non-ischaem

Transverse endoplasmic reticulum expansion in hereditary spastic paraplegia corticospinal axons.

Hereditary spastic paraplegias (HSPs) comprise a large group of inherited neurologic disorders affecting the longest corticospinal axons (SPG1-86 plus others), with shared manifestations of lower extremity spasticity and gait impairment. Common autosomal dominant HSPs are caused by mutations in genes encoding the microtubule-severing ATPase spastin (SPAST; SPG4), the membrane-bound GTPase atlastin-1 (ATL1; SPG3A) and the reticulon-like, microtubule-binding protein REEP1 (REEP1; SPG31). These proteins bind one another and function in shaping the tubular endoplasmic reticulum (ER) network. Typic

Estimating survival in patients with operable skeletal metastases: an application of a bayesian belief network.

BackgroundAccurate estimations of life expectancy are important in the management of patients with metastatic cancer affecting the extremities, and help set patient, family, and physician expectations. Clinically, the decision whether to operate on patients with skeletal metastases, as well as the choice of surgical procedure, are predicated on an individual patient's estimated survival. Currently, there are no reliable methods for estimating survival in this patient population. Bayesian classification, which includes bayesian belief network (BBN) modeling, is a statistical method that explore

Development and external validation of a machine learning-based multimodal radiomics nomogram for predicting progression-free survival in triple-negative breast cancer.

BACKGROUND: Triple-negative breast cancer (TNBC) is a highly aggressive subtype characterized by significant heterogeneity, making accurate prognostic assessment essential for clinical management. However, existing predictive models predominantly rely on single-modality data and lack external validation, which may limit their generalizability and clinical applicability. In light of these limitations, this study sought to explore the feasibility of developing a multi‑center prognostic model incorporating clinical and multimodal imaging data, and to preliminarily examine its potential value in s

Secondary Prevention of AFAIS: Deploying Traditional Regression, Machine Learning, and Deep Learning Models to Validate and Update CHA2DS2-VASc for 90-Day Recurrence.

Backgrounds/Objectives: Atrial fibrillation (AF) confers a fivefold greater risk of acute ischaemic stroke (AIS) relative to normal sinus rhythm. Among patients with AF-related AIS (AFAIS), recurrence is common: AFAIS rate is sixfold higher in secondary versus primary prevention patients. Guidelines recommend oral anticoagulation for primary and secondary prevention on the basis of CHA2DS2-VASc. However, guideline adherence is poor for secondary prevention. This is, in part, because the predictive value of CHA2DS2-VASc has not been ascertained with respect to recurrence: patients with and with

Predicting Care Needs in Community-Dwelling Older Adults Using Explainable Machine Learning and a Multidimensional Approach Integrating Health, Social, and Environmental Factors: Cross-Sectional Study.

BACKGROUND: Rapid population aging and a worsening shortage of care workers necessitate the identification of older adults who require proactive interventions. Although machine learning (ML) has been increasingly applied in gerontology, existing studies have predominantly focused on social isolation, loneliness, depression, falls, and frailty in isolation rather than on the integrated construct of care needs. OBJECTIVE: This study aimed to develop and interpret an explainable ML model that identifies care needs in community-dwelling Korean older adults. Beyond physical health indicators such

Triglyceride-glucose index and hyperhomocysteinemia within a cardiovascular-kidney-metabolic biomarker network: insights from interpretable machine learning and epidemiological modeling.

BACKGROUND: Although the triglyceride-glucose (TyG) index is an established surrogate marker of insulin resistance and metabolic burden, its relationship with hyperhomocysteinemia (HHcy) within the cardiovascular-kidney-metabolic (CKM) framework remains insufficiently understood. This study aimed to investigate the association between TyG and HHcy and to prioritize CKM-related biomarkers using interpretable machine learning and epidemiological modeling. METHODS: This retrospective cross-sectional study included 31,116 adults undergoing health examinations. HHcy was defined as serum homocystei

Arctic deep-sea hydrothermal microbiomes as a natural niche for novel antimicrobial peptides.

BACKGROUND: The escalating threat of antimicrobial resistance (AMR) has created an urgent need for new antimicrobial agents. Antimicrobial peptides (AMPs) are promising alternatives to conventional antibiotics due to their broad-spectrum activity and reduced risk of resistance development. While most AMP discovery efforts have focused on terrestrial microbes, extreme environments remain largely untapped. Deep-sea hydrothermal vent biofilms, such as those from the Arctic Mid-Ocean Ridges (AMOR), are unique ecosystems characterized by high pressure, temperature gradients, and chemical extremes.

Machine learning performance for a small dataset: random oversampling improves data imbalances and fairness.

BACKGROUND: Percutaneous coronary intervention (PCI) can be complicated by major adverse cardiovascular events (MACE; death, myocardial infarction [MI], and target vessel revascularization). Statistical models facilitated by machine learning (ML) can improve prediction of MACE compared with conventional models. However, two key challenges impair ML performance: (1) class imbalance in outcome distributions; and (2) bias affecting fairness across social subgroups. These challenges are amplified in small datasets. Additionally, no published ML model specifically addresses the influence of social

Prediction of Cardiopulmonary Resuscitation Outcomes for Arrest in Surgical Settings.

IMPORTANCE: Perioperative cardiac arrest and cardiopulmonary resuscitation (CPR) are associated with significant morbidity and mortality. Despite a growing focus on goal-concordant surgical care and longstanding emphasis on preoperative code status discussions, clinicians lack tools to individualize risk estimates and inform shared decision-making (SDM) regarding perioperative CPR. OBJECTIVE: To generate and internally validate predictive models for 30-day mortality and nonhome discharge using routinely available preoperative data. Environmental, Health, and Psychological Factors Predict Alcohol Sipping in Childhood: A Machine Learning Analysis of the ABCD Study.

OBJECTIVE: Extant research has focused on the risk factors for alcohol use in adolescence, but little work has examined these in childhood. Early alcohol sipping, defined here as sipping alcohol by ages 9 to 10, may be a unique and informative developmental precursor to future problematic alcohol use. This study employed machine learning to rank risk factors linked to early alcohol sipping by importance. METHOD: Data from a subset of the Adolescent Brain Cognitive Development℠ (ABCD) Study sample were used (n = 10,707 children ages 9-10 years old; 48.1% girls). Two state-of-the-art machine le

Risk factors associated with skeletal-related events following discontinuation of denosumab treatment among patients with bone metastases from solid tumors: A real-world machine learning approach.

BACKGROUND: Clinical practice guidelines recommend the use of bone-targeting agents for preventing skeletal-related events (SREs) among patients with bone metastases from solid tumors. The anti-RANKL monoclonal antibody denosumab is approved for the prevention of SREs in patients with bone metastases from solid tumors. However, real-world data are lacking on the impact of individual risk factors for SREs, specifically in the context of denosumab discontinuation. PURPOSE: We aim to identify risk factors associated with SRE incidence following denosumab discontinuation using a machine learning
