Performance of HAS-BLED and DOAC scores to predict major bleeding events in atrial fibrillation patients treated with direct oral anticoagulants: A report from a prospective European observational registry.

BACKGROUND: The DOAC score has been recently proposed for bleeding risk stratification of patients with atrial fibrillation treated with direct oral anticoagulants (DOAC). OBJECTIVE: To compare the performance of HAS-BLED and DOAC score in predicting major bleeding events in a contemporary cohort of European AF patients treated with DOAC. METHODS: We included patients derived from a prospective observational registry of European AF patients. HAS-BLED and DOAC scores were calculated as per the original schemes. Our primary endpoint was major bleeding events. Receiver operating characteristic

Impact of direct oral anticoagulants on target lesion revascularization rates in patients with chronic coronary syndrome.

BACKGROUND: Percutaneous coronary intervention (PCI) is a widely used treatment strategy for coronary artery disease. Optimal long-term antithrombotic therapy after PCI remains challenging. OBJECTIVES: This substudy of the Vienna PCI Registry assessed long-term effects of direct oral anticoagulant (DOAC) use on major adverse cardiac events (MACEs) in patients with chronic coronary syndrome (CCS) after PCI, exploring possible pleiotropic effects. METHODS: We analyzed patients with CCS from the Vienna PCI Registry treated with a drug-eluting stent (DES) between 2015 and 2020. The primary end p

Efficacy and safety of apixaban versus warfarin in new-onset atrial fibrillation post coronary artery bypass grafting: A retrospective cohort study.

New-onset postoperative atrial fibrillation (POAF) is a common complication following cardiac surgery, especially coronary artery bypass grafting (CABG), affecting up to 40% of patients, and is associated with increased risks of thromboembolic events and prolonged hospital stays. Traditionally, warfarin has been the standard anticoagulant for stroke prevention in atrial fibrillation. However, it requires regular international normalized ratio monitoring. Direct comparisons between apixaban and warfarin in the CABG population remain scarce. This study aimed to compare the effectiveness and safe

Management of an inferior vena cava thrombus with comorbid intracranial haemorrhage and cerebral amyloid angiopathy.

We highlight the complex management considerations in a patient with cerebral amyloid angiopathy who developed venous thromboembolism, explicitly navigating the balance between risks of thromboembolism and bleeding with anticoagulation therapy. This case highlights a man in his 70s who presented to the emergency department with an acute right frontal intracerebral haemorrhage secondary to cerebral amyloid angiopathy with an acute segmental pulmonary embolism managed with an inferior vena cava (IVC) filter. During hospitalisation, the patient developed a large expansile thrombus extending infer

Direct oral anticoagulants in cancer-associated venous thromboembolism: trial robustness and clinical trade-offs.

BACKGROUND: Although direct oral anticoagulants (DOACs) are often favored over low-molecular-weight heparin for cancer-associated thrombosis, the robustness of trial evidence supporting their use remains uncertain. OBJECTIVES: We evaluated the efficacy, safety, and statistical robustness of DOACs vs dalteparin in patients with cancer-associated venous thromboembolism (VTE). METHODS: We identified phase 3 to 4 randomized controlled trials comparing DOACs with dalteparin in PubMed, SCOPUS, and EMBASE up to August 2025. Primary efficacy was total recurrent VTE; secondary outcomes included recur

Balancing safety and efficacy: Assessment of a weight-based, anti-Xa-guided enoxaparin venous thromboembolism prophylaxis dosing strategy for traumatic brain injury patients.

BACKGROUND: Patients with traumatic brain injury (TBI) with intracranial hemorrhage (ICH) are at high risk for venous thromboembolism (VTE) but are also prone to hemorrhagic progression. The efficacy and safety of weight-based, anti-Xa-guided enoxaparin dosing for patients with ICH are unknown. Therefore, this study aimed to compare fixed chemoprophylaxis versus weight-based, anti-Xa-guided enoxaparin dosing in the setting of ICH, hypothesizing reduced VTE incidence with similar ICH progression with weight-based, anti-Xa-guided dosing. METHODS: This was a retrospective pre-post, quasi-experim

Use of Direct Oral Anticoagulants in Patients with Sickle Cell Disease and Venous Thromboembolism: A Prospective Cohort Study of 12 Patients.

Patients with sickle cell disease have an increased risk of venous thromboembolism (VTE) and with a mortality 2-fold higher. The anticoagulation of VTE in a young population is an important question. Indeed, hemorrhagic complications of anticoagulation may occur more frequently than in the general population. The use of a direct oral anticoagulant (DOAC) is not recommended for VTE in patients with sickle cell disease because those patients were not included in the clinical studies. We aimed to study the safety of using DOACs in a prospective cohort of patients with sickle cell disease and VTE.
