Final results of the southwest German pilot study on cystic fibrosis newborn screening - Evaluation of an IRT/PAP protocol with IRT-dependent safety net.

BACKGROUND: Previous studies suggest that PAP-based CF protocols are suitable for newborn screening (NBS) for cystic fibrosis (CF) when newborns designated as CFSPID should not be detected. However, there are still discussions about the performance of IRT/PAP algorithms. We present the final results of a pilot study evaluating a IRT/PAP protocol with an IRT-dependent safety net (SN) conducted from 2008 to 2016 in southwestern Germany on nearly 500,000 newborns. METHODS: To achieve reliable data, all newborns were screened using both the PAP-based and a DNA-based CFNBS algorithm. PAP quantific

Identification of paternal germline mosaicism by MicroSeq and targeted next-generation sequencing.

BACKGROUND: Prezygotic de novo mutations may be inherited from parents with germline mosaicism and are often overlooked when the resulting phenotype affects only one child. We aimed to identify paternal germline mosaicism in an index family and provide a strategy to determine germline mosaicism.' METHODS: Whole-exome sequencing was performed on an Alport syndrome-affected child. Variants were validated using Sanger sequencing in the pedigree analysis. An apparent de novo mutation was tested by next-generation sequencing (NGS) following chromosome microdissection of the mutant region (MicroSeq

Automatic Extraction of a Reference Region for the Noninvasive Quantification of Translocator Protein in Brain Using 11C-PBR28.

Brain inflammation is associated with various types of neurodegenerative diseases, including Alzheimer disease (AD). Quantifying inflammation with PET is a challenging and invasive procedure, especially in frail patients, because it requires blood sampling from an arterial catheter. A widely used alternative to arterial sampling is a supervised clustering algorithm (SVCA), which identifies the voxels with minimal specific binding in the PET images, thus extracting a reference region for noninvasive kinetic modeling. Methods: We tested this algorithm on a large population of subjects injected w

A set of microRNAs coordinately controls tumorigenesis, invasion, and metastasis.

MicroRNA-mediated gene regulation has been implicated in various diseases, including cancer. This study examined the role of microRNAs (miRNAs) during tumorigenesis and malignant progression of pancreatic neuroendocrine tumors (PanNETs) in a genetically engineered mouse model. Previously, a set of miRNAs was observed to be specifically up-regulated in a highly invasive and metastatic subtype of mouse and human PanNET. Using functional assays, we now implicate different miRNAs in distinct phenotypes: miR-137 stimulates tumor growth and local invasion, whereas the miR-23b cluster enables metasta

Receptor tyrosine kinase amplified gastric cancer: Clinicopathologic characteristics and proposed screening algorithm.

Although targeted therapy for receptor tyrosine kinases (RTKs) of advanced gastric cancers (AGCs) has been in the spotlight, guidelines for the identification of RTK-amplified gastric cancers (RA-GCs) have not been established. In this study, we investigate clinicopathologic characteristics of RA-GCs and propose a screening algorithm for their identification. We performed immunohistochemistry (IHC) for MLH1, MSH2, PMS2, MSH6, key RTKs (EGFR, HER2, MET), and p53, in situ hybridization for Epstein-Barr virus encoding RNA, and silver in situ hybridization (SISH) for EGFR, HER2, and MET using tiss

Allele and extended haplotype polymorphism of HLA-A, -C, -B, -DRB1 and -DQB1 loci in Polish population and genetic affinities to other populations.

Human leukocyte antigen (HLA)-A, -C, -B, -DRB1 and -DQB1 alleles were typed in 200 Polish healthy volunteers recruited for stem cell donor registry, using sequence-specific primer (SSP) and direct sequencing-based methods. Enhanced Bayesian approach of expectation maximization algorithm provided by phase platform was used for extended HLA haplotype inferences. The numbers of identified alleles (four-digit resolution) were 23, 23, 44, 27 and 18 alleles in HLA-A, -C, -B, -DRB1 and -DQB1 loci, respectively, of both northern and southern European frequency characteristics. The most frequent extend

KISA% index: a quantitative videokeratography algorithm embodying minimal topographic criteria for diagnosing keratoconus.

PURPOSE: To formulate and test an algorithm using minimal topographic criteria for accurately diagnosing clinical keratoconus. SETTING: Subspecialty cornea practice and Keratoconus Genetic Research Project. METHODS: Both eyes of 86 keratoconic patients who had never worn contact lenses and 195 normal participants were studied with the TMS-1 videokeratoscope to evaluate the KISA% index, an algorithm that topographically quantifies the phenotypic features of keratoconus. The diagnostic efficacy of the KISA% index was compared with that of the modified Rabinowitz/McDonnell (K- and I-S values) a

Adaptation of gastrointestinal nematode parasites to host genotype: single locus simulation models.

BACKGROUND: Breeding livestock for improved resistance to disease is an increasingly important selection goal. However, the risk of pathogens adapting to livestock bred for improved disease resistance is difficult to quantify. Here, we explore the possibility of gastrointestinal worms adapting to sheep bred for low faecal worm egg count using computer simulation. Our model assumes sheep and worm genotypes interact at a single locus, such that the effect of an A allele in sheep is dependent on worm genotype, and the B allele in worms is favourable for parasitizing the A allele sheep but may inc

Genetic Predictors of Antipsychotic Efflux Impairment via Blood-Brain Barrier: Role of Transport Proteins.

Antipsychotic (AP)-induced adverse drug reactions (ADRs) are a current problem of biological and clinical psychiatry. Despite the development of new generations of APs, the problem of AP-induced ADRs has not been solved and continues to be actively studied. One of the important mechanisms for the development of AP-induced ADRs is a genetically-determined impairment of AP efflux across the blood-brain barrier (BBB). We present a narrative review of publications in databases (PubMed, Springer, Scopus, Web of Science E-Library) and online resources: The Human Protein Atlas; GeneCards: The Human G

Multilocus fragment analysis of Cryptosporidium parvum from pre-weaned calves in Colombia.

The intra-species genetic diversity of Cryptosporidium parvum in dairy cattle farms in the central area of Colombia was investigated using a multilocus fragment typing approach with nine variable-number tandem-repeat (VNTR) loci and the gp60 gene. Genomic DNA of 70 C. parvum isolates from pre-weaned calves in 32 farms was analysed. Most markers showed two (ML1, MSB, CP47, and MSC6-7) or three alleles (5B12, Cgd2_3850, and Cgd6_5400), although they exhibited a major allele accounting for more than 69% of specimens, which explains their low discriminatory index. The TP14 microsatellite was monom

Fragmented Thresholds, Unified Vision: Advancing Salivary Diagnostics for OSCC and OPMD.

Salivary diagnostics for oral squamous cell carcinoma (OSCC) and oral potentially malignant disorders (OPMD) have advanced significantly, yet clinical translation remains limited. A key barrier is the lack of harmonized thresholds for candidate biomarkers, ranging from cytokines to genetic and epigenetic markers, across studies. This variability undermines biosensor calibration, impedes algorithmic integration, and restricts the scalability of AI and eHealth tools. Paradoxically, saliva is well-established in systemic disease diagnostics but remains undervalued in oral oncology. Recent initiat

Investigating Leptomeningeal Anastomoses' Role in Collateral Blood Flow using SPECT and 4D Flow MRI.

Leptomeningeal anastomoses (LMAs) are vital components of cerebral collateral circulation, but their small size and inter-patient variability hinder direct quantitative assessment in patient-specific cases. This study investigates the functional role of LMAs and their vessel-level interactions with the cerebral vasculature, using patient-specific flow data and accounting for uncertainty in peripheral vessel anatomy. Synthetic vascular trees were generated via a stochastic, anatomically informed sampling process. Assuming that discrepancies between 4D Flow MRI and Single-Photon Emission Compute

Personalized treatment pathways for adult osteomalacia.

Osteomalacia, characterized by impaired mineralization of the bone matrix in adults, arises from various causes, with vitamin D deficiency being the most prevalent. Additional etiologies include genetic disorders of vitamin D metabolism and excessive renal phosphate wasting. If left untreated, osteomalacia can result in bone pain, hypocalcemia, fractures, and skeletal deformities. This review provides an overview of current therapeutic strategies for osteomalacia, encompassing both pharmacological and non-pharmacological treatments, as well as a step-by-step management algorithm. We conducted

Diagnostic Methods and Biomarkers in Inflammatory Bowel Disease.

Inflammatory bowel disease (IBD) refers to a chronic inflammatory condition involving the GI tract that includes Crohn's disease (CD) and ulcerative colitis (UC). These conditions are believed to arise in genetically predisposed individuals who develop an exaggerated immune response to the intestinal microbiota. A timely and accurate diagnosis of IBD is essential because diagnostic delays can result in intestinal damage that is irreversible, leading in some cases to intestinal dysfunction and the need for surgery. Diagnostic delays are common in cases when GI symptoms are mild and nonspecific.

Update in the diagnosis of complement-mediated thrombotic microangiopathy/atypical hemolytic uremic syndrome.

Complement-mediated thrombotic microangiopathy (C-TMA), also referred to as atypical hemolytic uremic syndrome (aHUS), is a rare but severe cause of microangiopathic hemolysis and organ injury. Despite advances in understanding its pathophysiology and new nomenclature distinguishing C-TMA (aHUS) from other TMAs, diagnosis remains difficult because there is no single definitive diagnostic test. This update focuses on emerging data for functional complement assays (eg, modified Ham assay and human microvascular endothelial cell (HMEC-1) C5b-9 deposition assays) as adjunctive tests that may direc

[Blood eosinophilia in patients with severe bronchial asthma and chronic polypous rhinosinusitis treated with dupilumab: A review].

Bronchial asthma and chronic polypous rhinosinusitis are diseases associated with a T2-inflammatory immune response. These nosologies can be combined, creating the preconditions for a more severe course of multimorbidity, requiring the use of genetic engineering biological therapy. Dupilumab is a monoclonal antibody that can specifically bind to the alpha subunit of the interleukin-4 receptor and block the action of interleukins 4 and 13, which play a key role in the development of T2 inflammation. Numerous studies have demonstrated the high effectiveness of this medicament. The use of dupilum

General anesthesia using propofol infusion for implantation of an implantable cardioverter defibrillator in a pediatric patient with Andersen-Tawil syndrome: a case report.

Andersen-Tawil syndrome (ATS) is a rare genetic disease characterized by a triad of episodic flaccid muscle weakness, ventricular arrhythmias, and physical anomalies. ATS patients have various cardiac arrhythmias that can cause sudden death. Implantation of an implantable cardioverter-defibrillator (ICD) is required when life-threatening cardiac arrhythmias do not respond to medical treatment. An 11-year-old girl underwent surgery for an ICD implantation. For general anesthesia in ATS patients, anesthesiologists should focus on the potentially difficult airway, serious cardiac arrhythmias, suc

An automatic reduction method of 3D bone fragments based on a novel section contour point descriptor.

Comminuted fractures are orthopedic traumas with greater surgical difficulty. In clinical treatment, a great challenge is precise reduction of multiple broken bone fragments; Another great challenge is personalized and precise internal fixation after reduction. For these two issues, we designed an automated method framework for precise reduction and internal fixation of comminuted fractures. First, the Gaussian mixture model (GMM) is used to distinguish section points and noise points in a broken bone model; Second, ellipse fitting is carried out to achieve section points matching and a descri
